A geriatric care manager (GCM) is a health and human services professional, such as a nurse or social worker or other professional with geriatric experi-ence, that has specialized knowledge and experience related to aging and senior care issues. GCMs assist and advocate with older adults and their families to plan for and implement ways to allow for independence, safety, and comfort. GCMs are individuals who evaluate your situation, identify solutions, and work with you to design a plan , for maximizing your elder’s independence and well being.

What a Mature Solutions Two Geriatric Care Manager does: Identifies Needs by Conducting a Comprehensive Assessment

Which Includes:

Medical/Physical Emotional Financial Social Mental Safety

Develops a Care Plan Recommends or Arranges Services by Working With a Network of

Service Providers in the Community Monitors and Follows Up on Services Communicates with Older Adult, Family, and Service Providers

Mature Solutions Two also assists and advocates for the following services:

Information/Assistance/Referral Care Transitions Caregiver Services, Counseling Chronic Disease Self Management/Diabetes Self Management Pharmaceutical (SHAP), Benefits Access Daybreak-Get Connected Education and Awareness Transportation Employment Services Nutrition/Legal Services/Referral

Mature Solutions Two Geriatric Care Management Program is a private pay option to assist in allowing for and maximizing one’s

independence and planning for long-term care needs.
