This is an exciting time to be at Humber River Hospital as we prepare for our move to the new hospital. As I talk to staff, physicians and volunteers I am learning about how everyone is getting ready: learning new skills, implementing new processes and using new equipment.

Our construction team is making excellent progress in the building of the new hospital. The exterior is almost complete and soon we’ll be installing new equipment. All department, transition and planning teams are working hard to ensure we are ready to deliver high quality, safe, respectful patient care on day one.

As we reach development and transition milestones thank you for sharing your stories; they are bringing our new hospital to life.

ur new hospital is being designed with a focus on our future. All the planning, design and digital features were developed for what our community will need for the next 30 years and beyond. The centerpiece of our plans is patient and family-centred care. It will revolutionize health care by maximizing the benefits of technology, lean design and environmental planning.

Within our new building we will enable our hospital team to further enhance the high quality, safe, efficient care they provide in our current facilities. Care providers will have more time at the bedside, nurturing the personal interactions that form the basis of patient and family-centred care.

Our commitment to deliver patient and family-centred care is about more than the equipment we use, the processes we implement or great clinical CARE: it’s about CARING. All staff and physicians will be empowered to deliver respectful, compassionate, expert care. The Humber Experience is about Reinventing Patient Care by putting our patients at the centre of their care.

To help deliver patient and family- centred care, we are launching Communicate with H.E.A.R.T., a training program for staff, physicians and volunteers with a focus on effective and empathetic communication. Patient satisfaction depends on their entire experience at our hospital, including the care they receive as well as how they are treated. Any one individual can effect a patient’s whole impression of the organization.

It takes everyone to be committed to delivering exceptional care and service. You all are an essential part of the team. Everyone who works at Humber River Hospital, regardless of job title, is a caregiver and has an important responsibility to positively impact the patient experience.

This is why over the next three years, all staff, physicians and volunteers at every level will participate in the Communicate with H.E.A.R.T. program. We will use the Humber Experience Learning Map as a tool in this program to understand how we all work together to deliver care to each patient that comes to our hospital. It illustrates how our patient strategy is inextricably connected into every aspect of our care delivery.

This program was developed by the world-renowned Cleveland Clinic. Communicate with H.E.A.R.T. successfully transformed their organization into a culture of sustained service excellence. Despite their long history of clinical excellence, their patient satisfaction scores were very low and they wanted to find a way to fix this. They went to patients and families to understand their needs and expectations for customer service. Communicate with H.E.A.R.T. was born out of this process and it has become the model for patient and family-centred care around the world.

The move to our new hospital will change how we deliver healthcare and how we work as a team. With a patient and family-centred focus, we will use the capabilities of our new hospital and deliver enhanced high quality, safe, respectful care. One of the most important keys to our success will be in the way we communicate with our patients, their families and with each other. I look forward to your feedback on the effectiveness of the Communicate with H.E.A.R.T. program and how it contributes to enhancing the Humber Experience.

Father Stanislaw Moszkowicz was Humber River’s Chaplain since 2009. He passed away on Friday, July 18, 2014, during a visit to his native Poland.

Lucy worked at Humber River’s Chuch site on B500 and A3 South as a Registered Nurse since the start of her career in 1988. She passed away on July 25, 2014.

I support cancer patients and their families to help them understand their diagnosis and expected outcomes.

What is the most rewarding part of your job?

Earning trust from patients and families as they go through a very difficult time.

The staff and physicians in our Cancer Clinic practice to the high standard of patient care and public safety established for all clinics by Cancer Care Ontario.

Interesting Fact:

I’m a frequent traveller and my favourite place to visit is Singapore.

I provide anesthesiology care for all surgical and obstetric patients. This includes acute pain management for post-operative patients and the Critical Care, Paediatrics and Emergency Departments.

I like to build trusting relationships with patients and families and make their experience here more calming and comfortable.

What would you like patients, families and visitors to know about quality, safety andservice at Humber River?

HRH delivers high quality patient care with a focus on excellence in clinical care, patient safety, and comfort for patients and families.

Interesting Fact:

I play the guitar to relax and I have an interest in high performance driving.

I provide safe, high quality physiotherapy treatment to post-surgical and orthopedic patients. I also promote physiotherapy to universtiy students and coordinate student volunteers as they observe and assist with patient exercises.

What is the most rewarding partof your job?

I enjoy seeing patients reach their goals of greater physical capabilities and improve their lifestyles.

What would you like patients, families and visitors to know about quality, safety andservice at Humber River?

Humber River has excellent and dedicated rehabilitation staff with a lot of expertise who provide high quality patient care.

Interesting Fact:

I have a strong passion for ballroom and Latin dance and I recently won first place in an amateur competition.

Dr. Hagen receives Mentor

of the Year AwardCongratulations to Dr. John Hagen for winning the Mentor of the Year Award for Region Three from the Royal College of Physicians and Surgeons of Canada.

This award recognizes Dr. Hagen’s significant impact on the career development of students, residents and Fellows. He has proven to be an excellent role model for his trainees, taking great pride in their progress over the course of their service with him. He is committed to the highest standards of ethical and compassionate care.

Dr. Hagen introduced the teaching of residents, fellows and medical students into the culture of Humber River, which now has a rapidly expanding academic profile.

Excellence in wait time targets

for our patientsCancer Care Ontario (CCO) has recognized Humber River as the Top Performing Hospital in Ontario for Pathology Post-Surgical Report Turn-Around Times for Colorectal Cancer for the 2013/14 fiscal year and for exceeding the provincial annual improvement target. Our Surgical Department was also recognized with the award for Meeting Target for Cancer Surgery Wait Times-Time from Decision to Treat to Treatment.

This recognition is a result of many teams working together, including staff and physicians from the Surgical Department, and Laboratory and Pathology Services, to improve the patient experience at Humber River Hospital.

CCO is the organization that oversees quality and accessibility for cancer care services across the province. HRH is a member of the Central LHIN Cancer Care Steering Committee and is affiliated with CCO.

Clinical Practice Leaders and Professional

Practice LeadersOur clinical program areas are supported by an interdisciplinary group of professionals including physicians, nurses, IPAC practitioners, clinicians, and professional allied health staff who all work together to deliver high quality, safe, respectful patient care.

Clinical Practice Leaders (CPLs) and Professional Practice Leaders (PPLs) are part of the leadership team, providing support to all clinical areas in our hospital by promoting excellence in each of their practice disciplines.

CPLs work within programs and units to facilitate excellence in nursing and patient- centred care. They work collaboratively with program managers and various other discipline leaders to assess, plan, implement and evaluate educational programs for staff within and across programs. They provide leadership to the interdisciplinary team in the development and evaluation of clinical outcomes and standards of care. There are currently 20 CPLs at HRH who work across the various programs throughout the hospital.

The Professional Allied Health disciplines of Social Work, Speech Language Therapy, Dietitians, Occupational Therapy, Physiotherapy, and Registered Respiratory Therapy (RRT) at HRH are represented by five PPLs, an Allied Health Manager and an RRT Supervisor. They work to identify, establish and implement a standard of practice that is evidence-based and aligns with government legislation and their respective college guidelines.

CPLs and PPLs take an active role in collaborating with physicians and staff from program areas to improve patient care and safety. Healthcare professionals working together with patients support and enhance patient and family-centred goals and values. Such collaboration also increases communication among caregivers, optimizes staff participation in clinical decision making and fosters respect for the contributions of all the professionals.

The new Humber RiverThe new Humber RiverBecoming a Lean HospitalLean principles are used to analyze and find efficiencies in building design and in work, patient and material flow to ensure resources are being used effectively for quality patient care. In 2009, we partnered with GE Healthcare and worked with staff and physicians in our Medicine and Emergency units to understand our current workflow practices and processes. We then developed a future state based on Lean principles.

The aim was to understand the impact that the changes in unit layout, configuration and size, would have on our clinical workflow and efficiencies when we moved to the new building. Once this was understood, we modeled the impact that changes to design and technology would have in each unit and improving efficiency and care delivery.

Staff and physicians were involved in this work from the beginning and they participated in the process to brainstorm and validate ideas. We discovered that a nurse in a medical unit currently walks about 5.4 kilometres each day to provide care. Given the size of our new facility that walking time would almost double. We are using our findings from this process to become a truly Lean hospital. This includes elements built into the design of both inpatient and outpatient units such as the design of patient rooms, the location of utility rooms, and the development of equipment storage rooms. We are implementing technological initiatives including: BMV/eMAR; a fully digital bedside health record; patient room monitoring and vital sign devices that automatically record vital signs into the patient’s chart; a unified communications system providing alerts directly to handheld devices; tracking devices on frequently hunted equipment; and a pneumatic tube system. These initiatives will help staff and physicians deliver patient- centred care, communicate efficiently, and spend more time with patients.

First green and white

roof installations

The first green and white roof sections have been installed on our new hospital. We are using both green and white roofs to reduce the impact of the facility on the environment and to save on energy costs. There will be more than ten distinct sections of green roofs comprising 50% of the total roof area with a variety of highly drought tolerant plants such as ground covers, sedum plants and perennials. The remaining 50% of the roof area will be white roofs.

Green roofs help reduce the heat island affect and contribute to reduced cooling requirements and significantly reduced energy use. Green roofs also capture and absorb or evaporate large amounts of water, managing rain runoff. Any excess water will be captured in two 50,000 litre cisterns for site irrigation. The white roofs also help reduce energy used in managing building temperatures. They reflect sunlight back into the atmosphere directly reducing local heat island effect and global warming impacts.

The community amphitheatre, piazza and landscaping at our new hospital is almost finished. This will be a great place in the summer to grab a coffee, read a newspaper or watch the kids play on the terraced lawns. The campus team is also refurbishing the Ridge Park playground next door for the neighbourhood.

We want to welcome the whole community to visit the hospital even if they are not a patient or visiting one. Our new hospital was built on vacant lots and has renewed a significant area along Wilson Avenue with over 750 trees being planted on the grounds.

The amphitheatre and the campus will promote healthy lifestyles for patients and our community through increased physical activity in appealing, secure and comfortable walking areas and gardens.

On July 24th the construction team building our new hospital celebrated a safety milestone of over 3.5 million hours of Zero Lost Time Injuries with an on-site BBQ.

This is a huge project, and our construction team has put tremendous effort and energy into the building. The progress they’ve made since groundbreaking is impressive, but to have done it safely, with over 3.5 million hours of Zero Lost Time Injuries, that is the most impressive part of all.

It shows their regard for their fellow workers and that they are looking out for each other and themselves as they work. It shows that the work is being approached professionally and that there is pride behind their work.

We are proud of the construction team for this remarkable achievement.

The Humber Experience

At Humber River Hospital we are committed to Reinventing Patient Care. Creating a great place in which to work and to provide patient and family-centred care is important to that effort. To support our staff, physicians and volunteers in this area we are launching Communicate with H.E.A.R.T., a program designed to help everyone at Humber play their role more effectively in our shared responsibility to create a workplace of distinction, while enabling the delivery of high quality, patient and family-centred care.

Developed by the world-renowned Cleveland Clinic, Communicate with H.E.A.R.T. will provide additional tools to help increase awareness of the impact we have in our interactions with every patient, visitor and co-worker. The Humber Experience Learning Map, shown on the left, is an important tool in the training. It highlights the importance of teamwork and effective cross-functional workflow. It also shows how each person’s role in our hospital contributes to the larger picture of care, and caring.

Patient satisfaction depends on one’s entire experience at our hospital. It not only includes the care one receives but as well the caring with which that care is delivered. A kind word, a smile, a listening ear – each may make all the difference in turning a difficult hospital experience into a positive one. And, empathetic communication helps patients, families, staff, physicians and volunteers work together in a busy hospital.

We are focusing on two specific aspects of the program:

Forty staff members have been trained by the Cleveland Clinic to lead workshops with individuals from all disciplines and sites. HRH is committed to train 400 staff, physicians and volunteers by the end of March 2015. Our goal is to have everyone who works, learns or volunteers at HRH receive Communicate with H.E.A.R.T. training in the next three years.

Emergency preparedness at our new hospitalAdapting our existing emergency codes and procedures for the new building is a critical project. By October 18, 2015, they must all be complete, comprehensive and thoroughly tested to ensure 100% reliability.

To ensure moving day goes smoothly, Mike Orrico, Director of Emergency Preparedness, and his team have been hard at work on emergency

preparedness, but they’ve had help. “We’ve had a great partnership with Toronto Fire, Toronto Police Services and Toronto EMS, as well as other experts,” he explains, “developing policies and procedures to help ensure the safety and security of everyone in the new hospital.”

Given the size and complexity of our new hospital, Mike’s team is building a new Code Manual for staff, physicians and volunteers. “This manual is their

guide on how to react in different code and emergency situations,” says Orrico. “There will be extensive on-site training starting in May of 2015, once we have possession of the building, so that when doors open on October 18, 2015, everyone is prepared to respond to an emergency situation in a seamless and coordinated effort.”

The new building will leverage state-of-the-art technology to minimize the risks in emergency situations, to ensure that affected units, staff, physicians, security and EMS personnel are connected and notified immediately of emergency location and details, as well as for up-to-the-minute updates. Staff and physicians, security and code response teams will be alerted through PDA and computer alarms, while the

public will be informed though electronic monitors throughout the hospital when appropriate.

Code Response teams will provide the support necessary to effectively respond to medical emergencies within the hospital. A Real Time Locating System (RTLS) with a ‘Nurse Call’ system will be implemented to further enhance safety in the hospital, including newborn foot monitors, and wrist monitors for at-risk adult and paediatric patients.

For community disasters – explosions, derailments etc. - HRH will be better equipped to respond thanks to its large decontamination capability. There will be a designated decontamination suite, as well as the ability to use the enclosed ambulance bay to decontaminate over 100 people at one time. In addition, if a local incident impacts the external air quality, the supply control system will switch from using external air to purified recirculated air.

“Ensuring life safety within the new hospital – for patients, for staff and physicians, for volunteers and visitors - is an absolute priority for my team,” says

Orrico. “We all hope it never happens, but if that day does come, we will be ready. We will keep everyone safe and informed.”

Planning SessionOn July 31st we held a day-long planning meeting for the transition to our new hospital with a number of partners to review progress, identify areas for further discussion, and share what different groups are doing. Participants included PCL Constructors, Health Care Relocations, Johnson Controls Inc. and the hospital’s planning team. Everyone involved is clearly focused on the task at hand, and working collaboratively to ensure a safe, efficient move to our new building.

Using Electronic Health Records and Computerized Provider Order Entry (CPOE) will help us achieve our digital vision. Our goal is to share electronic data seamlessly and efficiently across the system, to all appropriate users, in real time.

Dr. Adam Krajewski is supporting this vision by changing completely to electronic patient records and updating them during his rounds, rather than at the end of the day. To do this, he needed to learn how to type.

He was initially anxious to start typing, but he knew that only

through practice would his technique improve. After a few months of perseverance he is now able to complete patient notes as he consults with patients and have a seamless digital workflow. “I am grateful that Dr. Ali Ghafouri introduced me to typingweb.com to improve my typing skills,” Dr. Krajewski says. “Hard work and motivation are my only secrets.”

Our digital vision leads to better communication and instant access to the right information. It will enhance patient care by improving efficiency, accuracy, reliability and safety.

Greening Health Care Sustainability Forum

On May 28th, Rueben Devlin, President & CEO, spoke at the Toronto and Region Conservation Authority’s 5th annual Greening Health Care Sustainability Forum, highlighting Humber River’s innovative sustainability plans for our new hospital.

Humber River Hospital is a Greening Health Care member for our leadership role in reducing energy use in healthcare. Humber River’s new hospital has been designed and is being constructed with a goal of achieving Leadership in Energy and Environmental Design (LEED) Silver certification and we are on track to achieving LEED Gold status.

Some of the features of the hospital that will contribute to improved efficiency include using natural and LED lighting, low flow toilets, faucets, and showers, automated lighting, white and green roofing, and windows made of electrochromic glass, which allow full control over how much light is let through and eliminates the need for blinds.

Showcasing excellence in bariatric surgery to the worldHumber River’s bariatric surgical team, led by Dr. John Hagen and Dr. Laz Klein, performed two live bariatric surgeries through video feed from Humber’s Finch site for the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) conference, held in Montreal on August 30th.

Five hundred international surgeons watched Drs. Hagen and Klein perform these operations to learn from them how these intricate procedures are handled at Humber River. According to one of the conference organizers, Dr. Michel Gagner of Hôpital du Sacré Coeur in Montréal, the HRH surgical team “delivered the most amazing cases, and made Canada proud about its contribution to bariatric surgery. I only heard excellent comments about Humber River.”

This event was the first time we used the new operating room video conference technology. This technology, combined with the larger operating rooms in the new hospital, will make it that much easier to collaborate remotely with other surgeons

and medical experts during an actual surgery. It will also greatly enhance the ability of students, residents and clinical fellows to learn from Humber’s surgical team.

A sustainable health care facility has significant benefits for patients. "We need to understand that environmental sustainability means a better quality of life for patients, for staff and physicians, for everybody,” says Rueben Devlin “I'm in the people care business; every dollar I save on energy resources, I put into providing patient care."

The Healing Harmonies Music Program was piloted this summer on A3 South, Church site and 6W, Finch site. The group played a mix of classical and semi-classical music for patients and family members.

This very talented group of students consisted of pianists, a violinist, and a vocalist who study at the Ontario Academy of Music. The program was well received and has been music to everyone’s ears – patients, family members and staff!

The students were busy all summer selling samosas and organizing fundraising events. This has been the most successful summer for fundraising from our students yet, with a total $1,667 raised!

This year, the Summer Student Volunteer Program held their first ever Yard Sale at the Church site. It was a great success raising $640 for the Volunteer Association to help purchase equipment for our hospital.

Another new initiative that was piloted this summer was Volunteer Rounding. Experienced summer students went to the Departments with summer volunteers to help them settle into their new roles, bring forward their questions, and ask them about their experiences.

There were three Volunteer Rounding Team Leaders at the Church site: Thurga, Chris and Jacky, and three at the Finch site: Tiana, Zohal and Lina. Many thanks to our Volunteer Rounding Team Leaders – you did a great job!

Family Campaign RaffleOur summer Cash Raffle raised over $5,000 for the new hospital! We appreciate everyone’s tremendous support; we more than doubled our goal!

Thank you to Louisa Ceci and Mary Latter, our Family Campaign Co-Chairs, and all the committee members who were enthusiastic about the raffle from the beginning and who worked really hard in selling tickets. 100% ClubCongratulations to our Environmental Services Department – Housekeeping staff at all three sites for being the first Department to reach 100% in staff giving. In total, this group has donated $21,207 towards the Environmental Services Department at our new hospital. Thank you for leading the charge!

2014

On August 18th the 31st Annual Awesome Invitational Golf Tournament took place at Eagles Nest Golf Club. The day was a tremendous success, raising over $200,000 to help build and equip the new Humber River Hospital! The tournament, presented by Plenary Group, PCL and Johnson Controls, was completely sold out. In addition to their round of golf, guests enjoyed gourmet meals, special activities on the course, live and silent auctions, and a raffle. Golfers heard from Rueben Devlin, President & CEO on the progress of our new hospital and the many lean, green, and digital features that will make our hospital patient and family- centred.

I was treated in the Emergency Room on Sunday and

later admitted into the hospital. A very special thanks

to Agnes, the nurse in the ER, as well as Dimitri, the

ultrasound technician and Jan, the CT-scan technician.

Also a special thanks to Sal who transported me to my

tests. Your professionalism and kind ways made it

easier for me to deal with my health troubles. May all

of you receive great blessings.

I took my mother to Emergency with severe abdominal pain it was 2:30 am. Early that morning, Molly, the Head Nurse arrives and tells me she is very concerned about my mother. She quickly starts to make phones calls, makes sure all the appropriate tests are performed and gets the doctor ASAP to make sure that the pre operating forms are ready for signing. Molly was amazing and I thank her from the bottom of my heart. If it wasn't for her passion and caring disposition, I don't think my mother would be with us today. It's people like Molly that truly makes the difference at Humber River Hospital.Mary

I would like to express my deep appreciation for the wonderful and humane care

I received during my six day stay in the B-5 wing. Juyoung, Marcia, Helen,

Rosalie and Leilanie went about their duties with great professionalism and

dedication. They are a true credit to their profession and my family and I feel

blessed by their presence in our lives. Ernesto and Opoku cheerfully delivered my

food every day and always put a smile on my face. Merlinda always cheered me

up with her smile when she came to clean the room. All of these wonderful

people made my stay at Humber River Hospital a memorable experience.

12 Division officers responded to a threaten suicide call. Officers immediately requested a rush for

DAS to attend. 12 Division Medical Crisis Intervention Team’s Officer and Nurse Adele Stavro

arrived on the scene prior to DAS. They immediately attempted to control the victim’s breathing

as he was hyperventilating and had become extremely restless.The victim continued to voice his

suicidal intent and was unreceptive to care from DAS. The officers and Nurse Stavro remained at

the hospital with the gentleman until he was in a more cooperative state and receptive to care. On

behalf of the Toronto Police Service and 12 Division, I commend Nurse Stavro for not only her

medical assistance, but her efforts to assist in diffusing what was a very emotionally charged situation

and helping to bring about a positive outcome.
