2. The nurse (named above) contacted will direct staff to transport individual to the ER, if not

already done, or instruct staff to contact the individual’s PCP.

3. Incident reporting processes remain as is.

4. Nurse Coordinator must assure that the Director is informed of the PCP directive or nurse’s

decision to transport the individual to the ER.

5. Nurse Coordinator must assure that Rose Kehoe, Director of Nursing is also informed and

documents this information the 7:30am Report.

6. The identity of the individual displaying COVID 19 symptoms is to be held confidential in line with

HIPPA standards

7. Staff will follow the instructions of the individual’s PCP or as directed at the emergency room

ICF Calls routinely go to the physician

CMH Calls routinely go to the nurse house manager

In the event that a call is received to provide COVID-19 test results, staff receiving that call need

to connect immediately with the nurse coordinator. All positive test results must be reported

directly to the nurse coordinator OR, if reported second hand the nurse coordinator will need to

validate the test results. The nurse coordinator will then initiate an action plan. All routine

notifications to families around the care of the individual will remain in place and would include

this illness as well.

Incident reporting processes remain as is.

ACTION PLAN

1. The NURSE COORDINATOR will notify

a. Director of Nursing, Rose Kehoe

b. Program Director

c. Cammie Benford, Transitional Nurse Coordinator

2. The PROGRAM DIRECTOR will identify all staff, including temp agency staff, who worked during

the 14 days since the individual displayed any one of the COVID 19 symptoms. The DIRECTOR

will inform Crystale, Scheduling Supervisor, of the names of the temps. The Director will assure

that all staff receive the FAQs.

a. FAQ used and given to all staff who may have been in contact with infected person

i. Staff must take their temperature twice daily. Report temperatures equal

to or greater than 100.4

ii. Advise staff if they work at another company/organization, they need to

follow the other employers’ guidance including communication with the

employer about exposure.

iii. Staff will only work in the KenCrest community home where they were

exposed to COVID 19. Mask and gloves will be provided. Normal use of

gloves and masks, no extraordinary requirement.

iv. Staff are expected to work. There are only three reason that someone will

be authorized PTO:

Staff provide a doctor’s note that they are at risk if they report to

work

Staff provide a doctor’s note that someone in their home is at risk if

they report to work.

The staff is displaying one or more symptoms of COVID-19.

If the employee declines a work assignment, the employee will be taken off

the schedule for 14 days from the day of exposure. The employee is not

permitted to use PTO because work in that home is available. In the event

that the employee experiences a symptom while they are off the schedule,

the employee must notify the HR Department. HR will advise regarding next

steps.

This is considered a non-disciplinary suspension from the schedule.

The employee is placed back on the schedule after 14 days unless the

employee experiences symptoms of COVID 19 and informs the KenCrest HR

Department. The HR Representative will advise the employee of the next

steps.

IF AN EMPLOYEE declares their intention to file for unemployment, HR

needs to be contacted. If asked, we will advise on the employee’s

unemployment claim that work is available

3. To support individual with C0VID-19, PROGRAM MANAGERS must:

i. Attempt increase social isolation in bedroom ii. Support client to wear mask, if tolerated

iii. One staff per shift will be assigned to support the individual to limit exposure

iv. Reinforce handwashing with all staff in the home v. Put disinfectant close at hand and wipe all surfaces

vi. Assigned staff must accompany the individual to the bathroom and disinfect all surfaces after use.

vii. Thorough cleaning is needed of all surfaces using soap and water and then disinfectant.

viii. Ensure that dishes, drinking glasses, utensils are not shared. When possible, use disposable eating and drinking items. Dispose of these items in the trash receptacle in individual’s bedroom using a trash bag liner. Ensure that bedding and towels are not shared.

ix. A trash can should be kept inside the individual’s room, close to their door. All disposable items from this room must be thrown out in this trash can. Staff should wear gloves when handling and disposing of trash from the infected individuals’ room. When the trash needs to be removed from the room, the bag must be tied in the room. This bag is then dropped into another trash bag being held open by another staff person outside of the room. The trash bag is tied again and immediately disposed of outside into the garbage can. Everyone should wash their hands thoroughly afterwards. ANYTHING THAT GOES INTO THE ROOM CANNOT COME OUT OF THE ROOM (EXCEPT TRASH FOLLOWING PROCEDURE) WITHOUT PROPER CLEANING!!! Magazines, books, puzzles, toys, radios, iPads, etc. all must stay in the room until terminal cleaning occurs when isolation is discontinued.

x. Regarding laundry 1. Handle their laundry with disposable gloves and dispose of gloves

after handling the infected person’s laundry 2. Wash their clothing separately 3. Launder items using the warmest appropriate water setting for the

items and dry items completely. 4. Do not shake dirty laundry to minimize dispersing virus in the air.

5. Clean and disinfect clothes hampers; If possible, consider placing a bag liner that is either disposable (can be thrown away) or can be laundered

xi. Isolation will be maintained until staff is notified to discontinue.

xii. The doors to the home need to be locked at all times.

xiii. Any visitors to the home must wear a mask.

4. NURSE CARE MANAGER in support of other clients in a home:

a. Nurse Care Manager will inform the other individuals’ PCP that a housemate has

tested positive for COVID 19, follow physician’s direction

b. Nurse Care Manger or Nurse Coordinator informs the emergency contacts of the

other consumers that someone in the home has tested positive for COVID 19.

KenCrest staff must comply with HIPPA guidelines and maintain confidentiality.

Advise them regarding physician’s advice and new precaution.

c. Increase temp monitoring to twice a day or more if directed by physician

d. Ask other clients not to the visit the person

5. DIRECTOR ADVISES:

a. Crystale Ebron, Scheduling Supervisor, to adjust schedules

i. Staff who work at the site or have come into contact with the ‘individual’ in

question cannot work at another site

b. Day Program Coordinator, if reassigned day program staff worked at this site and

were exposed. No client name will be shared. HIPPA guidelines must adhered to

and confidentiality must be maintained.

c. Wanda Ritter, Director of Personal Outcomes Champions

6. WANDA RITTER, Director of Personal Outcomes Champion

a. Identifies the assigned Personal Outcome Champion assigned to the individuals and

instructs the Champion to notify the assigned Supports Coordinator.

7. Director of Nursing, ROSE KEHOE advises:

a. Marian Baldini, President/CEO and Pam Schuessler, COO

8. Marilyn Everling, Director of Special Projects OR Melinda South, Executive Director OR Steven

Wolfe, Executive Director:

a. Notify the state DD authority and any other authority

If a KenCrest Supported Individual is identified as a confirmed case,

and remains in a health care facility

Positive test result received:

d. The PROGRAM DIRECTOR will identify all staff, including temp agency staff, who

worked during the 14 days since the individual displayed any one of the COVID 19

symptoms. The Program Director will notify Crystale Ebron, Scheduling Supervisor of

the temp agency staff that were exposed to the individual

The Day Program Coordinator will inform the reassigned staff can only work

in the KenCrest community home where they were exposed to COVID 19

until the 14 day waiting period is over.

iv. Advise staff if they work at another company/organization, they need to

In the event that a KenCrest Staff is suspected or test positive for

Covid-19.

SUSPECTED

Calls may come into immediate supervisors, schedulers or HR. The staff who is made aware of a

KenCrest employee who is displaying any symptoms of COVID 19 must assure that these steps are

taken:

1. If a staff member has any one of the symptoms associated with COVID-19

Direct them to call their primary doctor immediately. Instruct the staff to inform their

doctor they are an essential health worker….this applies to anyone who is in direct care,

may be working direct care, or supervising direct care

Give the employee the phone number to call our HR representative to report their test

results or status

Ensure the employee’s supervisor and or Program Director are alerted

KenCrest supervisor must notify the HR Department. HR notifies the Director of Nursing.

2. The staff person is not allowed to work and must self-quarantine until one of the following is

met:

The person may return to work IF:

The employee has been at home for 7 days with 3 consecutive days having no

symptoms and no medication to control symptoms.

The employee has a documented negative test for the virus.

The employee has a doctor’s note to return

The employee’s doctor may require the employee to remain out of work for longer. This is acceptable.

3. Contact the Director of Nursing PA, Rose Kehoe or Executive Director in DE, Melinda South or

Executive Director in CT, Steven Wolfe.

The Rose Kehoe Director of Nursing will assure that HR has the employee on our

tracking list

The Rose Kehoe, Director of Nursing will advise the Nurse Coordinator (PA) or House

Manager(DE or CT) to increase client monitoring to twice a day

CONFIRMED

The employee is not allowed to work and must self-quarantine.

The employee may return to work IF:

The employee has been at home for 7 with 3 consecutive days having no symptoms and no medication to control symptoms.

The employee has a documented negative test for the virus

The employee has a doctor’s note to return
