MyActiveHealth offers integrated, cutting-edge resources and programs to members at work, at home and through their doctor.

Wellness Screenings provide opportunities toimprove members’ overall health and energy so they can better enjoy every area of their life.

YOU ARE REQUIRED to participate if you are enrolled in the PEEHIP Blue Cross Blue Shield hospital/medical plan.

PEEHIP Wellness Program is a “ParticipatoryProgram,” which means members and eligiblespouses must only participate, not meet anyspecific health standard.

For example: A member will still get thefinancial incentive even if they have anabnormal body mass index (BMI).

For those who do NOT participate, a $50.00 monthly wellness premium will be applied tomembers and their spouses for a possibletotal $100.00 per month.

Next year: Single coverage = $65 per month Currently - $15 (180) VS. $65 (780) = +$600

Next year: Family coverage = $277 per monthCurrently - $177 (2,124) VS. $277 (3324) = +$1200

This will become effective 10/1/2015 if youdo not complete the annual wellness program requirements.

1. Complete a Wellness Screening – This must be completed between 8/1/14 – 5/31/15.

Your choices……. ◦ Attend a workplace wellness screening by ADPH

nurses. ◦ Visit the County Health Departments which are

offering screenings throughout the state. ◦ Visit your Doctor’s office for a wellness screening.

There is no copay for 1 annual routine preventative visit. (Diagnostic services,treatments and some lab tests other than what is required may cause a copay charge orfee.)

Members should provide the doctor with theHealthcare Provider Screening Form.

Members should NOT send the forms to PEEHIP. The doctor’s office must mail or fax the fully completed form to ADPH at 334-206-0385.

Additional Requirements are ONLY for thosewho get a letter notifying them that they are a candidate for coaching.

3. Participation in Wellness Coaching – The Health Coach works with members to reduce health risks of certain diseases and to manage weight, stress and eat healthier.

5. Participate in Disease Management

The five chronic diseases identified for wellness coaching are: asthma, diabetes, coronary arterydisease, congestive heart failure, COPD.

Coaching minimum requirements include: 1. 1 phone call to ActiveHealth OR 2. 100 Heartbeats of online coaching online

coaching credit (takes 4 weeks) ActiveHealth nurses and coaches will contact

candidates or can be reached at 855-294-6580, 8:00 a.m. to 8:00 p.m., M-F, 8:00 a.m. to 1:00 p.m.,Saturdays.

Sign up for a Wellness Screening!

Go to www.MyActiveHealth.com/PEEHIP or call to complete your Health Questionnaire.

Participate in Coaching if identified by PEEHIPand MyActiveHealth.

Go to www.MyActiveHealth.com/PEEHIP. Create an account with a user name and

password. Check the Rewards Center to check on your

status of completion towards earning themonthly wellness premium waiver.

Don’t ignore any mail or emails or phone calls from MyActiveHealth. They will try to reach out to members who have not completed their requirements.
