    Palomar Pomerado Health System islaunching an e-health initiative that will allowits medical staff to access virtually all patientmedical records and telemetry via Android-

    powered smartphones and tablets.The two-hospital system in northeast San

    Diego County introduced its in-houseapplication, Medical Information, Anytime,Anywhere (MIAA), at the HealthcareInformation and Management SystemsSociety annual conference in Orlando earlierthis week.

    "Physicians who are on the go, at home oron call, will have the ability to access realtime information in context, said Ben Kanter,M.D., a Palomar Pomerado radiologist andchief medical informatics ofcer. This willlead to more accurate medical judgments,

    ultimately improving patient care."Palomar Pomerado developed MIAA at a

    cost of about $250,000, and will test it inpilots with about a dozen physicians over thenext year. Ofcials claim it is the rst Android-powered patient records system developed bya healthcare system in the U.S.

    Physicians will be able to use MIAA toaccess patient records and graphically-oriented tests such as x-rays. They may alsocheck patient vital signs in real time and willbe warned of potential drug interactions. Theapplication will work on smartphones and onCius, a new tablet manufactured by Cisco.

    Partnerships are also being developedwith Rady Childrens Hospital, KaiserPermanente and other providers in the SanDiego area to share patient medical records,

    according to Palomar Pomerado spokespersonAndy Hoang.

    The more sharing of information, themore effective this will be for treatment,Hoang said.

    Hoang added that Palomar Pomeradoofcials rejected the iPhone and iPad as viableplatforms because Apple could not providethe support needed to ensure full functionality

    MIAA stands in contrast with the leaningsof most of the provider community, which hasbeen favoring applications for Apples iPhoneand iPad.

    The Food and Drug Administration

    approved the use of the iPad to performradiological diagnoses earlier this month,while iPads are quickly becoming a staple ofmedical schools.

    To date, most medical-oriented Androidapplications are reference rather thandiagnostic tools. However, sales of Android-powered smartphones have recently elipsedthose of the iPhone. Korean electronics giantSamsung also recently announced it wouldenter the EHR market via the Androidplatform, although such products are at leastmany months from introduction.

    Hoang indicated Palomar Pomerado will

    eventually market the system elsewhere.

    Two Hospitals On Healthgrades ListGlendale Memorial, Saint Johns Named Top 50

    MPTF Strikes Deal With ProvidencePact Is Expected To Avoid Nursing Facility Closure

    Glendale Memorial Hospital and Saint JohnsHealth Center in Santa Monica were the twoCalifornia facilities named to HealthGrades2011 list of the best 50 hospitals in the UnitedStates.

    Hospitals that made the HealthGrades listhad risk-adjusted mortality and complicationrates that were in the top 5% in the countryfor the past 11 years. On average, eachhospital had a nearly 30% lower risk of deathand a 3% lower risk of complications.

    Altogether, the hospitals were surveyed intheir delivery of 26 categories of care,including treatment for heart attacks, hip

    fracture repair, strokes, sepsis and coronarybypass surgery.

    According to HealthGrades, if all U.S.hospitals performed at that level, the deaths 500,000 Medicare patients between 1999 an2009 could have been prevented.

    !!!!!!When the Health InsurancePortability and Accountability Act wasenacted 15 years ago, few had any ideawhether it would serve as healthcaresversion of omerta (the Mafia Codeof Silence), or another ineffectualintrusion by the Feds. We alsoheard and still hear grumblingfrom doctors that all this medicalrecord preservation and protection

    has brought innumerable newcosts to their practices while theirpatients grumble about the extra burden ofsigning multiple HIPAA and other consentforms.

    !!!!!!Fortunately, HIPAA works much ofthe time. Most patients can rest safe atnight knowing their medical history is bothprotected from prying eyes and accessibleupon request. However, there have beenjust enough egregious violations to keepeveryone on their toes.! Among them:!!!!!!In April 2007, a formeradministrative assistant at Ronald Reagan

    UCLA Medical Center was indicted by afederal grand jury for selling patientinformation to the National Enquirer. Sixty-eight current and former staff, includingnine doctors, had been sneaking peeks atthe records of famous patients. This is oneof the most commonplace breeches: thecelebrity admitted to the hospital and thestaff unable to resist their prying eyes.

    !!!!!!There are also the occasionalreports of a health plan employees laptopthat containing the medical recordsthousands of enrollees stolen from a car or

    storage medium with millions of records sfor destruction and lost in transit.

    !!!!!!In virtually all of those cases, the has been caught and disciplined. Many halost their jobs. Those who have sold medicrecords such as the UCLA Healthcareemployee and another at New YorkPresbyterian/Weill Cornell Medical Centefaced criminal prosecution.

    !!!!!!The Health Information Technologfor Economic and Clinical Health (HITECHAct, the federal legislation that providesfunding for providers to switch to electron

    health records, also gives HIPAA a shot inarm by expanding who is covered by HIPAwhile setting very high fines for acts ofomission. (Such as the lost laptops andresponsible parties who ignored their HIPAand omerta oaths) Many experts believe thwill add the crucial level of security andfinancial penalties to ensure such inciden

    !!!!!!Nevertheless, we fear that suchincidents contribute to reluctance on behalf of patients and caregivers to nofully participate in the process ofdialogue, recording, and analysis nee

    to build and maintain the patient medrecord. Such reluctance could provepotentially harmful down the line if cruciaaspects of patient information were notconveyed by the patient or recorded by thcaregiver. Compounding this is the fact thcomplete patient record is already a logistchallenge to compile, given that multipleproviders must often contribute their recoto create a whole portrait of that patientsmedical history and overall health.

    !!!!!!However, we do believe thatproviders and their colleagues who compand keep track of medical records do a

    terrific job, even under the worst ofcircumstances. Well discuss that in detailnext week.!
