Pathways offers people an opportunity to engage in a thorough programme of mental health and daily living rehabilitation.

Welcome.

Pathways Northwest is an exclusively designed specialist resource offering a recovery programme to people with complex mental health needs.

The scheme endeavours to meet the requirements of service users with challenging and multifaceted mental health and behavioural problems. Pathways offers people an opportunity to engage in a thorough programme of mental health and daily living rehabilitation, using the recovery model and wellness recovery action planning, coupled with independent living experiences.

Pathways Northwest will do this in a manner which is responsive to the individual needs of service users taking account of ethnicity, culture and gender.

Pathways Northwest is committed to developing its staff team so that they have the knowledge and skills to be able to deliver high quality care and enhance their role.

Our aims can be summarised via our mission statement:

Pathways Northwest will work with our partners to provide the best mental health care to service users with complex and challenging needs, within a specialised environment. We will provide intensive mental health rehabilitation and facilitate access to a full range of services within the community.

Our Philosophy

The philosophy of Pathways Northwest is to provide a safe, happy, supporting and healthy environment for residents within a homely setting.

We provide intensive one to one care, and, if required, two to one support. Programmes are delivered in highly structured and consistent environments. We focus our work towards addressing the complex presenting issues and needs that have led to the referral, whilst ensuring that the broader development needs of people are also fully realised. Each service user is involved in the decision

making process regarding daily living arrangements. We like all facilities, resources, activities and services to remain service user led, where possible. We aim to work with the residents and their family members to formulate and implement a focused care plan reflecting their social, physical and spiritual needs.

We work alongside the principles of the Health and Social Care Act and other relevant and current legislation and guidance and aim to meet all standards for our service users.

Mental Health ServicePathways assists adults with mental health difficulties to live as independently as they can within their local communities.

Pathways works closely with other professionals and extended families to ensure our services compliment the overall support provided to individuals within their home.

Our approach is to assist individuals to gain or relearn skills which will assist them to achieve higher levels of independence and to reach their own personal goals in life whilst receiving the support they need to live safely within the community.

Our priorities are Work, Recovery and Inclusion, as we feel these themes are key to achieving better mental health.

We support people in order to assist them in planning their care with hope and optimism and work towards independence and autonomy even when long term support may be needed.

We endeavour to help service users manage mental health crisis/relapse, when safe, within the home rather than moving to a hospital environment.

This allow us to carry out invaluable work with the service user in terms of managing symptoms.

Pathways Northwest is a modern service which engages Service Users in forward thinking and creative manner.

Community SupportWe ensure individuals can exercise choice and have control over the way our services are provided.

Personal and ‘relationship-centred’ care and support should be at the heart of the services offered by residential care homes.

Pathways NW is a community - both of itself and within the existing community in which it is located.

Residents and staff actively

find ways to engage with the wider public to personalise the services offered.

Our management team ensures all staff members ‘live and breathe’ a culture that actively promotes personalised services - with maximum choice and control for people living within our three schemes.

We ensure residents have access to all the information and advice they need to make informed decisions - including support services.

This challenge is met using the Recovery Star model.

Recovery Star tracks individual development and ensures the support we provide is constantly improving.

It helps service users become optimistic about what they can achieve while motivating our dedicated staff members even further.

By continually providing proof that our services work to commissioners and referring agents, we can continue to improve our services.

The Mental Health Recovery Star makes it possible to provide evidence while enabling users, and workers, to focus on the important issues – providing a platform for development.

This exciting model demonstrates our core values - to work for a society in which all people with mental health concerns have the right to take control of their lives.

Service ModelPathways NW offers in-depth support, recovery and rehabilitation. We are committed to helping individuals plan for the future and gain more independence.

We recognise the importance of gender-based differences of women i.e. the effects of trauma, violence, and abuse and intergenerational issues is worked upon.

We offer placements which are highly structured and consistent in a small family oriented environment. Each home is managed by a highly experienced mental health professional such as a nurse or psychologist. This, coupled with high staffing ratios, is key to services user success.

We have 20 places available within the scheme. Our Oswaldtwistle care home is a gender sensitive female home. Our gender sensitive services address the particular needs of women is a safe, non threatening environment.

A key component of the service is the understanding of the critical role of gender in the risks, course, and treatment of women’s mental health.

We are able to support young adults aged 18 to 65. We specialise in working with people who have complex; multifaceted needs.

This may include mental health needs, complicated by personality issues, mild learning difficulties and alcohol and substances misuse for example.

The scheme endeavours to meet the requirements of residents with challenging mental health and behavioural needs. The management of risk is a key element of work in recovery services.

Prospective Service Users are assessed on a individual basis – therefore we do not have a inclusion or exclusion criteria as we feel it is our responsibility to alter our service to meet people’s needs effectively.

Our Care Homes

Accrington Care Home

A six bedded unit situated a short walk from the town centre. Whalley Road is a spacious six bedroom property, with considerable communal living space. The property is homely and bright with a great outdoor space, perfect for summer BBQs. Whalley Road accommodates both male and female service users aged 18 to 65.

Oswaldtwistle Care Home

A tremendous individual property, Oswald Sreet is both spacious and welcoming. A short walk from the pleasant town of Oswaldtwistle the house benefits from being close to local amenities whilst being in a pleasant residential area. Oswald St has a fabulous forecourt garden perfect for outside projects and summer BBQs. Oswald Street is a seven bedroom property, with each room having an en-suite bathroom.

Great Harwood Care Home

Located on the outskirts of Great Harwood, the property is a ten minute walk from the quiet market town of Great Harwood. The property is ideally suited for access to Accrington and Blackburn Town Centres. A warm family style seven bedroom property with two en-suites, ideal for service users, both male and female people, aged 18 to 65.
