Agreement Regarding Participation in Work Ethic Distinction

I have attached all required documentation to support the points that I have indicated inside this folder. If the Participant is under the age of 18, I/we __________________________________ (Parent or guardian) give permission for participant to participate in the Rutherford County Work Ethic Distinction.
Participant’s decision is voluntary. Parent/Guardian is the duly authorized guardian or parent of Participant and has full and exclusive authority to make the agreements herein contained.
Online Publishing of Name Consent: In the event the student has received all required points to earn the Work Ethic Distinction, the Participant and Parent/Guardian, if applicable, give permission for Participant’s name to be included as a Work Ethic Distinction recipient on the Rutherford County Chamber of Commerce – Rutherford Works webpage at www.rutherfordworks.com.
In consideration for the right to participate in the Activity, Participant and Parent/Guardian, if applicable, willingly grant and irrevocably consent to and authorize the Rutherford County Chamber of Commerce the right and permission to display or otherwise use or reuse participant’s name as a Work Ethic Distinction recipient.
Participant and Parent/Guardian, if applicable, have read and understand this Agreement in its entirety and voluntarily enter into same, without reliance on any representations, statements or inducements, express or implied, made by a party whatsoever. We understand that we have had the opportunity to ask questions or object to any of the terms of this transaction by contacting the Rutherford County Chamber of Commerce. By entering this Agreement, we acknowledge that we fully understand and do not object to any of its terms.

You can retrieve your ACT WorkKeys National Career Readiness Certificate (ACT WorkKeys NCRC™) from myworkkeys.act.org/mwk/login.do. Use the user id and password you were provided when you initially took the assessment. Attach a copy of your industry certification certificate or award. Industry awareness events include Rutherford Works summer internship, work-based learning, job shadow, or visit to an industry (field trip). Please ask teacher and/or employer to provide signature and date of your industry awareness activity on this form.Your eligibility for the Tennessee Promise can be verified by logging into your Tennessee Promise account at www.tnpromise.gov and taking a screen shot of your community service hours and printing the screen shot.Include a copy of your acceptance letter from a post-secondary institution.Community service hours can be verified by attaching a signed letter from the agency or individual providing the community service opportunity (this can include your Tennessee Promise community service hours).ARCpoint Labs, 1826 Ward Drive Ste 204, 37129, will provide reduced cost drug screens. Students must bring a valid government ID and a copy of the Donor Pass/Collection Authorization which can be downloaded from http://bit.ly/WEDonorPass. To earn points, students must attach copy of passed documentation.

Work Ethic StandardsAll Rutherford County High School seniors are eligible to earn the Work Ethic Distinction. A student must earn a minimum of 20 from the 50 available standards points in addition to obtaining a regular high school diploma. Students who obtain this distinction are proving that they are both college and career ready. This distinction will guarantee an interview with employers as long as the candidate meets job related requirements.
Student Responsibilities:• Obtain and attach all supporting documentation and required signatures for each standard• Tally total points earned• Turn in tallied form along with all supporting documentation and signatures to counseling department no later than March 27, 2020• Optional: Purchase green Work Ethic Distinction graduation tassel
