General Terms: Southeast Entertainment Enterprises, Inc., a Minnesota corporation, operates entertainment facilities under the name Grand Slam Sports. These facilities offer various types of equipment for recreational use. I have had an

opportunity to inspect the facilities and equipment and I fully understand the various recreational activities offered. I understand that the equipment and activities offered by Grand Slam Sports do not constitute a public service or an essential service or program, but rather provide voluntary recreational activities of the type that can be obtained through similar programs at other facilities. My choice to use the equipment and participate in the recreational programs at Grand Slam Sports are wholly voluntary. In consideration of being allowed to use the facilities and equipment of Grand Slam

Sports, I, on behalf of myself and my minor child or legal ward, agree to the following terms:

Assumption of Risk: I and/or my minor child or legal ward will be using various equipment and engaging in various recrea-tional activities at Grand Slam Sports. I understand this equipment exposes participants to certain inherent and serious risks. I further recognize that use of the trampoline system exposes participants to additional risks including, but are not limited to, the variable effects of rebound forces that may directly injure a person or that may throw a person out of

control; the possibility that a person may land improperly or fall on, or be thrown against other persons or objects; and that their is the potential of severe injuries from flipping, running or bouncing off the walls of the trampoline system. The risks associated with the use of any of this equipment involve certain dangers that can not be completely avoided or eliminated, and may result in serious personal injury and/or potential death or disability. To the maximum extend allowed by law, I, and on behalf of my minor child or legal ward named above, ASSUME ALL RISKS ASSUME ALL RISKS ASSUME ALL RISKS ASSUME ALL RISKS for any property damage, personal injury, death or disability that may result from the use of any of this equipment including those arising from

ordinary negligence of the Releasees who are identified hereafter.

Release and Waiver of Liability: In consideration of being permitted to participate in the activities and to use the recrea-tional equipment including the trampoline system at Grand Slam Sports, I and/or my minor child or legal ward do hereby voluntarily release, waive, and discharge all claims, law suits, and demands that I or my minor child or legal ward has or may have in the future against Southeast Entertainment Enterprises, Inc., Grand Slam Sports, or any of its officers,

directors, employees, agents or representatives, and their successors and assigns, collectively referred to herein as (“Releasees”) arising out of any damage, loss, personal injury, death or disability to me or to my minor child or legal ward while using any of the equipment or participating in any of the activities offered at the Grand Slam Sports facilities. This Release is valid and effective whether the damage, loss, personal injury, death or disability is the result of any act or omission on the part of any Releasees or from any other cause. This Release and Waiver of Liability includes, but is not

limited to, any property damage, personal injury, death or disability that may occur as a result of use or misuse of the equipment or facility by anyone, use of any equipment that malfunctions or breaks, improper inspection or maintenance of the facilities, grounds, or equipment, any claimed inadequate instruction or supervision, or any slipping, tripping and/or falling while in the facility or on the surrounding premises, including all those arising from negligent acts or omission of Releasees.

In accordance with Minnesota law, nothing in this Assumption of Risk, Waiver and Release of Liability Agreement should be construed as assuming any risk, or waiving or releasing any claims I or my minor child or legal ward may have for gross negligence, willful or wanton misconduct, or reckless or intentional acts by or on behalf of Releasees.

Perpetual Effect of this Document: I agree that this Assumption of Risk, Waiver and Release of Liability Agreement ex-tends into the future and covers any and all visits for which this Agreement applies, as well as any return or repeat visits

by either myself, my minor child, or my legal ward for whom I am responsible.

Severability: On behalf of myself, my minor child and/or my legal ward, I agree that if any portion of this Assumption of Risk, Waiver and Release of Liability Agreement is found to be void or unenforceable, the remaining provisions of this document shall remain in full force and effect.

In consideration of the services of Partner Up Promotions dba Jolly Jumpz Bounce World, their agents, owners, officers, volunteers,

participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to a "PUP"),

I hereby agree to release, indemnify and discharge PUP, on behalf of myself, my spouse, my children, my parents, my heirs, assigns,

personal representative and estate as follows:

I acknowledge that my participation in inflatable devices or any other amusement device activities entails known and unanticipated risk

that could result in physical or emotional injury, paralysis, death or damage to myself, to property, or to third parties. I understand that

such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.

The risks include, among other things: The risks include, among other things: The risks include, among other things: The risks include, among other things: slips and falls; colliding with others which could cause strains, sprains, broken bones and head

injuries; cuts abrasions, and bruises; heat exhaustion, heat stroke and cardiac related events or illness; falling off of or being thrown from

the inflatable which could result in musculoskeletal injuries including head, neck and back injuries; equipment failure or operator error; the

negligence of other participants, or other persons who may be present; my own physical condition and the physical exertions associated

with this activity.

Furthermore, PUP employees have difficult jobs to perform. They seek safety, but they are not infallible. They might be

unaware of a participant's fitness or abilities. They might misjudge the weather or other environmental conditions. They may give incom-

plete warnings or instructions, and the equipment being used might malfunction.

I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely

voluntary, and I elect to participate in spite of the risks.

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless PUP from any and all claims, demands,

or causes of action which are in any way connected with my participation in the activity or my use of PUP's equipment or facilities, including any such claims which allege negligent acts or omissions of PUP.including any such claims which allege negligent acts or omissions of PUP.including any such claims which allege negligent acts or omissions of PUP.including any such claims which allege negligent acts or omissions of PUP.

Should PUP or anyone acting on their behalf, be required to incur attorney's fee and cots to enforce this agreement, I agree to

indemnify and hold them harmless for all such fees and costs.

I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the cost of such injury or damage to myself. I further certify that I am wiling to assume the risk of any medical or

physical condition I may have.

In the event that I file a lawsuit against PUP, I agree to do so solely in the state of Minnesota, and I further agree that the substan-

tive law of that state shall apply in that action without regard to the conflict of law rules of that state. I agree that if any

portion of this agreement is found to be void or unenforceable, the remain document shall remain in full force and effect.

Class time is 3:15 to 6:15 pm for all 10 sessions to satisfy the 30 hours re-

Upon completion students will be eligible to take the test to receive their instruc-

tional permit. Students must be at least 14 when class starts and 15 years old

to take the permit test.

Behind the wheel lessons will be arranged later once the student has been driv-

ing a few months. Three lessons, each 2 hours, is required before taking the

road test at/after age 16.

Early Childhood Screening is a free check of a child’s growth and devel-opment provided by Pine Island Public Schools. Screening in early childhood supports readiness for kindergarten and promotes positive child health and development. If any areas of concern are identified, children can get the help they need before starting school. Screening at age three is ideal, as it provides the earliest time to offer support and resources to children and families.... The screening tools are adjusted for a child’s age, so waiting for a child to learn more skills before screening is not recommended and provides no benefit. It can also delay care for any areas that may be of concern.

The screening includes height and weight, vision and hearing, and a developmental screening, which looks at the child’s thinking, language and motor skills. Parents will also have a health and immunization review with a nurse, followed by an exit interview. Early childhood screening includes a vision screening that helps detect potential eye problems, but is not a substitute for a comprehensive eye exam.

Call the Pine Island School at 356-8785 or Community Education at 356-8876 to schedule your appointment. Appointment times range from 7:30 AM until 3:00 PM and take about an hour and a half. Early Childhood Screening is a free check of a child’s growth and devel-opment provided by Pine Island Public Schools. Screening in early childhood supports readiness for kindergarten and promotes positive child health and development. If any areas of concern are identified, children can get the help they need before starting school. Screening at age three is ideal, as it provides the earliest time to offer support and resources to children and families.... The screening tools are adjusted for a child’s age, so waiting for a child to learn more skills before screening is not recommended and provides no benefit. It can also delay care for any areas that may be of concern.

The screening includes height and weight, vision and hearing, and a developmental screening, which looks at the child’s thinking, language and motor skills. Parents will also have a health and immunization review with a nurse, followed by an exit interview. Early childhood screening includes a vision screening that helps detect potential eye problems, but is not a substitute for a comprehensive eye exam.

Call the Pine Island School at 356-8785 or Community Education at 356-8876 to schedule your appointment. Appointment times range from 7:30 AM until 3:00 PM and take about an hour and a half. It can, by releasing accumulated tension, restore a natural vitality to body and mind. This class can be used as a primer to prepare for other hatha yoga classes, or on it’s own for the benefits it provides. The series is rather gentle and should be within the ability of nearly everyone. A blanket and/or a mat for the class is recommended.

I believe that for the success of our program, coaches at all levels need to be on the same page by working together, believing in the same philosophy and teaching concepts that will be beneficial to our players as they move throughout various levels of our program. At PI we have a system and progression that begins preparing our players for our varsity system when they begin learning team basketball in 5th and 6th grade.

roads to Red Wing, MN. Our first stop will be the Pottery Museum of

Red Wing. The Museum is dedicated to preserving and celebrating

the rich and colorful history of Red Wing’s clay industry. More than

6,000 vintage pieces of artisan-crafted stoneware, pottery, dinner-

ware and folk art bring the story of historic Red Wing to life in dozens

of dynamic exhibits coving 13,000 square feet. Come and explore

the amazing legacy of the potteries born of earth and fire on the

banks of the mighty Mississippi. We will have a 45 minute tour and

some time for shopping at the gift shop.

Next stop, Treasure Island Casino to tempt our taste buds at the

Tradewinds Lunch Buffet. After lunch, we will have several hours to

explore and have fun at the casino. Patrons will receive a FREE $15

player’s card to play the slots.

We will take a self-guided tour of the famous Bachman’s Ideas House.

While touring this historic home decorated you’ll find a mix of vintage and

new, which will inspire you with decorating ideas perfect for the Holidays.

Please note that because the Bachman’s Ideas House is an historic prop-

erty, it is not fully handicapped accessible. The main floor of the house is

accessible via an outdoor wheelchair ramp; however the second floor is

only accessible by stairs. We will enjoy a buffet lunch at Patrick’s Bakery

Café, located in a greenhouse at the Bachman’s Garden Center. There

will be plenty of time to shop and look around in the gift center and the

greenhouses.

Next, we’ll make a stop at Home Essentials Boutique in Lake Elmo, a

seasonal boutique that has a changing array of everyday and seasonal

items. With over 185 juried vendors, bringing their very best, you will al-

ways find fabulous furniture, original floral arrangements, exceptional

gourmet foods, and unbelievable one-of-a kind treasures and gifts at

Our final stop will be at Gerten’s Garden Center for more holiday inspira-
