What is the Delta Center?

The Delta Center for a Thriving Safety Net provides technical assistance to state primary care associations

and behavioral health state associations to build a stronger safety net, particularly in ambulatory care

settings.

The Delta Center aims to inspire innovation and change in value-based care and payment, through

both policy and practice.

Who is behind the Delta Center? • Funding by the Robert Wood Johnson Foundation • Led by John Snow Research & Training, Inc. (JSI) in partnership

with the MacColl Center for Health Care Innovation at Kaiser Permanente and the Center for Care Innovations (CCI). This partnership combines strengths in policy and value-based payment in the safety net, delivery system transformation, and sustainable change in organizations.

• The Delta Center is partnering closely with the National Council

for Behavioral Health and the National Association of Community Health Centers marking their first large-scale collaboration in this area.

What is meant by significant value-based payment opportunities?

States that have plans or efforts underway that are moving safety-net providers towards either Category 3 or Category 4 in HCP-LAN Framework.

Example opportunities include: – Medicaid ACOs, – Certified Community Behavioral Health Clinics, – State Innovation Model Test awards (not just planning grants), – DSRIP or DSRIP-like Medicaid waivers, – FQHC APM demonstrations, – 2703 Health Homes State Plan Amendments, – Medicaid MCO-sponsored programs that involve HCP LAN

Categories 3 and 4 payment reforms – Or equivalent expertise, models, or initiatives.

Delta Center Technical Assistance Modalities 7 In-person convenings Enable peer sharing and best practice exchange; faculty will include

national and regional peer experts; each convening will last 1-1.5 days.

Capability-building webinars

Foster networking and deeper knowledge transfer on specific topics that emerge as priorities at the state association level.

Coaching Help identify needs; troubleshoot/overcome challenges; assist state associations in developing an impactful collaborative for providers and effective policy work related to payment.

Expert consultants Provide in-depth and on-site consultations on specific topics or challenges identified by state associations in the network.

Online resource center and toolkit

Includes tools and resources on evidence-based care delivery transformation changes in primary care and behavioral health integration and understanding value-based payment at the provider level.

Capability-building workshops

Sample content includes: • Organizing & conducting engaging technical assistance • Working with payers, including Medicaid • Understanding value-based payment and care • Creating a quality improvement/change strategy with data for the

Proposed Content Payment reform and policy transformation: understanding and articulating options for value based pay; working with payers including Medicaid agencies and health plans; using data to communicate provider value; documenting policy options; synthesizing best practices in payment and policy change Care delivery transformation: team based care, behavioral health integration, data culture for data driven decisions and clinical decision support, panel management, population health management – outreach and in-reach, assessing for social needs, managing complex care patients, care coordination, patient engagement Cultivating practices of a learning organization: learning about co-designing TA offerings with members; applying adult learning theory principles and strategies; leverage top experts in the field; design effective evaluation tools to assess knowledge and skills transfer; using data for storytelling; creating a quality improvement/change strategy that emphasizes a culture around testing, continuous measurement and feedback. Building partnerships: creating shared vision and action plans in care transformation and payment change with partners. Partners could include other providers, other sectors (behavioral health and primary care), hospitals, payers, etc.

Benefits of Participation The Delta Center will provide:

- A network of 10 state organizations to share best practices and problem solve challenges to transform care and move to value-based payment

- Opportunity for behavioral health and primary care state associations to work together to advance partnerships.

- A cadre of well vetted national experts, coaches, and capability building workshops to assist state associations in providing high quality learning opportunities for their providers to provide value-based care in alternative payment models

- 2-year $240,000 grant to each association

What We’re Looking For • 5 State Primary Care Associations and 5 Behavioral Health State

Associations as lead organizations to advance innovation, knowledge and spread of changes in value-based care and payment

• Organizations with experience to share and a desire to enhance current efforts and/or explore new areas within care delivery and payment reform

• Trusted partners for technical assistance (TA) delivery and leadership

• Active and engaged participation in collaborative and evaluation activities

• Open to advancing partnerships between Behavioral Health and Primary Care Associations

• Commitment to building capacity within organization to support providers transition to value-based care and payment

Program Expectations/Deliverables • Attend 7 in-person convenings

• 5 opportunities for shared learning across primary care and behavioral health plus 2 sector-specific convenings adjacent to NCBH and NACHC annual conferences

• Attend 6 virtual learning events • Participate in coaching and check-in calls with Delta Center staff and

faculty (estimated 3-5 per year) • Actively engage with providers • Share successes and challenges with Delta Center to be written into

policy insight briefs • Convene with stakeholders, policy makers, and payers to move payment

and policy forward • Participate in evaluation activities lead by Mathematica • Participate in readiness assessments and submit work plans • Submit quarterly progress reports
