A Nurse Liaison is available to assist you with the admission process, at your facility or hospital, around the clock, 24 hours a day, 7 days a week.
Once a referral phone call is made, our Nurse Liaison will provide the following:
• On-siteassessment
• Paperwork/Documentation copying or faxing
• Phonecallstofamilyand/or power of attorney
• Arrangetransportation
• Postdischargefollow-up
The goal of each Nurse Liaison is to streamline the process from admission to discharge.
