Hopeless lower molar 80 year old female, taking oral bisphosphonates


Grafting with synthetic bone substitute


Considerable resorbtion of alveolar ridge after healing. Limited mouth opening.


CBCT, as there is insufficient ridge for bone sounding to be adequately diagnostic


Selection of slice 5 mm distal of rootsurface.


Soft tissue readings and distance to mandibular canal


Transfer of readings to cast and proposed bucco-lingual implant axis


BLP locks in Bucco-lingual position and top of the implant.


Buccal wing positioned while maintaining bucco-lingual and top of the implant


Lingual and buccal wings positioned


Wings connected to vacuform carrier with Ortho Acrylic


Radiographic Implant Replica’s overlap, image is diagnostic.

No need for rotational correction, so use 0 degree ( green) rotation block


8.5 x 4.3 mm Nobel Active, placed as planned, flapless procedure
