Notes on Completion of ABRSM Theory Exams Entry Form 2019 General information

The entry form should be completed in BLOCK capitals with a pen or ball pen. One letter for each box and leave a blank box between words. Do not exceed the boxes provided. Blind or partially sighted candidates who require Braille or enlarged question papers should submit a written request and doctor’s certificate together with the completed entry form. Additional charges may be levied on candidates requiring special arrangements, if approved. Personal data of applicants/candidates are used by the HKEAA and the Examination Board for delivery of examination and assessment services. Whether you provide the requested personal data is voluntary. However, if you fail to provide all the data, or if any of the data are inaccurate or incomplete, the HKEAA may not be able to accept your entry or provide all or part of the examination and assessment services. The personal data submitted may also be used for:

(a) assisting tertiary institutions and other government/public organisations in their admission processes; (b) assisting tertiary institutions and other government/public organisations in respect of their requests for information in granting scholarships; (c) assisting tertiary institutions and other government/public organisations to confirm the candidates’ eligibility for financial assistance or other

forms of subsidization, in which case, the necessary personal data of the candidates may have to be disclosed to the concerned organisations for verification;

(d) certifying candidates’ examination results in response to legitimate requests; (e) processing any refund or payment in relation to the examination; (f) conducting educational research and analysis in an anonymous format in which the identities of candidates are not traceable; and (g) marketing the services and products of the Examination Board (including examination services, courses, events, publications, and other

examination materials or resources) subject to the consent of the applicants/candidates.† The HKEAA may also transfer the personal data of applicants/candidates to third parties for use for the above purposes or other directly related purposes, including government/public organisations, schools and educational institutions, banks (for processing refund or payment), and service providers providing various administrative or technical services to facilitate the delivery of the examination and assessment services including but not limited to data punching, registration process, despatch of examination documents, and the capture, disposal or other processing of data. In compliance with the Personal Data (Privacy) Ordinance, access to all personal data will be made available, on payment of a fee, to anyone who can establish his/her right to be informed of such data as are retained by the HKEAA. Please also note that applicants’/candidates’ personal data/correspondence/other information received by the HKEAA may be transferred to the Examination Board concerned for delivery of examination and assessment services and for any of the abovementioned purposes or other directly related purposes. In general, within a reasonable period after completion of an examination, the HKEAA would have transferred all such data to the Examination Board who would then assume full control of such data. Applicants/candidates should approach the Examination Board direct for their personal data/information handling policies if necessary. † If you have given consent but wish to withdraw your consent for your personal data to be used for this purpose, please send a letter of request to the Registration Centre (Ref to Section 7).

Registration by Post The following should be attached to an entry form sent by post: i. a photocopy of the official and valid candidate’s HKID card/Passport/Birth Certificate (for verification purposes only) which will be destroyed after the

registration period; ii. a stamped, self-addressed envelope (size : 9” x 4”) for the Authority to return a demand note for settlement of examination fees at a later stage. Applicants must note that their entries will not be accepted if any of the above-mentioned attachments have not been enclosed. Cash or cheques should NOT be enclosed. Entries sent by post should reach the International and Professional Examinations Division of the Authority’s San Po Kong Office on or before the postal registration deadline for the specified examination date. Please ensure that sufficient postage is paid to avoid unsuccessful delivery. The sender will bear any consequences arising from not paying sufficient postage. The Authority cannot accept the responsibility for any delay/loss in mail service. If a demand note is not received before the in-person registration starts, the applicant should submit another application in person/by a representative/on-line on or before the respective registration closing date/time. Registration in Person/by a Representative Applicants may submit an entry form in person or have the completed entry form handed in by a representative during the specified period. A photocopy of the official and valid candidate’s HKID card/Passport/Birth Certificate (for verification purposes only) should also be submitted. Supporting documents produced by applicants at the time of registration in person/by a representative should either be the originals or hard copies. Soft copies of supporting document are NOT accepted. On-line Registration On-line registration will be available at online.hkeaa.edu.hk during the specified period. Before submitting an on-line registration, applicants should scan and save the image file of the official and valid HKID card/Passport/Birth Certificate for each of the candidate(s) (for verification purposes only which will be destroyed after the registration period). Before registration, applicants are required to create their own accounts with a valid email address. Applicants who registered the examination by online registration may login their accounts at the Registration Home Page to view their examination entries, download the admission forms and view the examination results. Applicants will be asked to make payment online as part of the registration process. The Authority cannot accept any responsibilities for unsuccessful registration because of technical/network failure. On-line registration is not available to candidates using re-entry vouchers. Admission Forms Admission forms will be sent to the applicants by post. Applicants who have not received their candidates’ admission forms 7 days before the examination date should contact ABRSM Team at 3628 8721 or 3628 8787. Release of Examination Results and Despatch of Certificates Examination results will be sent by post to the applicants approximately two months after the date of examination. Certificates of successful candidates will be sent by registered post/local courier to the applicants about three months after the date of examination. Applicants who have not received their candidates’ certificates should contact ABRSM Team within five months after the issue of certificates. Otherwise, the certificates will be regarded as successfully received by the applicants. Uncollected certificates will be destroyed five months after the issue of certificates. After that, requests for uncollected certificate will be regarded as applications for replacement of certificates and supplementary fees will be levied. The examination data including entry records, question papers, answer scripts and results, if any, will normally be destroyed and erased from the computer system six months after the examination date. Therefore, all examination documents including admission forms and mark forms, if any, cannot be re-issued after destroy and erase of data.

2019 年度 ABRSM 樂理考試 - 填寫報名表指引一般事項

申請人/考生個人資料為讓考評局及考試機構提供考試及評核服務所使用。申請人/考生可自行決定是否提供所需的個人資料。但倘若未能提供全部所需的資料或提供了不準確或不完整的資料，考評局可因而拒絕接受有關申請或拒絕提供全部或部分考試及評核的服務。已提交的個人資料，亦可能作以下用途：

(甲) 協助高等院校、政府或公營機構處理入學申請； (乙) 應高等院校、政府或公營機構要求，提供資料協助處理獎學金申請； (丙) 應高等院校、政府或公營機構要求，提供資料協助核實考生於津貼申請的資格，在此情況下，本局將向有關機構披露所需的個

根據個人資料(私隱)條例，任何人士若能向考評局證明其資料當事人之身分，在繳付有關費用後，可向考評局提出查閱其個人資料的申請。惟請注意由考評局所收集有關申請人/考生的個人資料/聯絡方法/其他資料，可能會轉交相關考試機構用作提供考試及評核服務及對上述各項作任何用途或其他直接有關的用途。一般情況下，在考試或考試季度及所有相關的考試服務完結後，考評局會將所有相關資料轉交擁有該等資料的完全處理權的考試機構處理。如有需要，申請人/考生屆時可直接向所屬考試機構申請查閱其個人資料或有關資料處理的政策。

1 申請人(年滿 18 歲或以上)資料

請注意，經老師／樂室／學校／其他為申請人保送的考生，在繳付有關費用後，可直接向本局要求更改申請人 (即把申請人改為考生本人／家長／監護人)。

填寫在報名表上的「申請人」須負以下職責： (甲) 要閱讀並承諾遵守考試規則，以確保關注與考試相關的所有事項； (乙) 保送考生參加考試，並確保所提供的資料正確無誤，包括姓名，級別和考試科目，及任何特定需要； (丙) 向考生闡明:

i. 考生可選擇自行報名參加考試，或經由家長∕監護人、學校或音樂老師代報； ii. 考生如獲頒發合格證書，申請人的姓名亦會列印於該證書上。

(丁) 通知考生有關考試日期，時間和地點； (戊) 接收所有與報名事宜相關的通訊(包括考試成績/證書)，並盡速將之轉達各考生。

郵遞報名

下列文件須連同報名表格一併郵寄至本局：

(1) 考生的有效香港身分證/護照/出生證明書副本（副本只供審核資料用）。申請手續辦妥後，本局會將之銷毀； (2) 一個已貼郵票的 9 吋 X 4 吋回郵信封，以便本局於核對表格後，將繳費單寄予申請人。若上述附件未有連同報名表一併遞交，申請恕不接納。切勿郵附現金或支票。報名表格必須於郵遞報名截止日期或以前送抵本局新蒲崗辦事處。請確保已支付足夠郵資，以免郵件未能送達考評局。寄件人須自行承擔因未有支付足夠郵資而引致的任何後果。任何郵遞的延誤及遺失本局恕不負責。倘申請人在該次考試的親身或委託他人報名日期開始時，仍未收到本局所發出的繳費單，必須在截止報名日期之前，親身或委託他人重新遞交報名表或透過網上辦理報名手續。親身或委託他人報名申請人填妥報名表格後，可於指定日期內親自遞交報名表格或委託他人辦理報名手續，考生的有效香港身分證/護照/出生證明書副本（副本只供審核資料用）必須於報名時一併遞交。申請人於親身或委託他人遞交報名表時所遞交之有關文件必須是正本或影印本，任何軟複本恕不接納。網上報名申請人可於指定時間內，透過網頁 (online.hkeaa.edu.hk) 在網上辦理報考手續。在使用網上報名前，申請人須先為個別考生掃描考生的有效香港身分證/護照/出生證明書（供審核資料用）。申請手續辦妥後，本局會將之銷毀。在報名前，申請人須先以有效的電子郵箱開設有關帳戶。經網上報考的申請人可在考試報名主頁，登入帳戶查看已報名的考試、下載准考證及查閱考試成績。申請人於輸入其本人及考生的資料後，須同時於網上繳交考試費。任何因電腦技術/網絡上失誤而未能於指定報名期內成功報名者，恕不負責。考試代用券不適用於網上報名。准考證本局於考試前，將准考證郵寄予申請人。倘申請人於考試前七日仍未收到其考生的准考證，應立即與本局聯絡，安排重印。發布考試成績及寄出證書考試成績會在考試後約兩個月寄給申請人。合格考生的證書將於考試後約三個月以掛號信/本地郵政速遞形式寄給申請人。倘申請人未能於上述時間收到其考生的證書，申請人必須於五個月內 (由發出證書日起計)與本局聯絡。否則，有關證書將視作為成功派遞給申請人。未領取之證書將於五個月後 (由發出證書日起計)銷毀。倘於日後要求領取有關證書，將作「申請補發證書」處理，考生須另繳附加費用。所有涉及考試的文件及檔案（包括報名記錄、試題、答卷和考試成績，如適用），在一般情況下，將於考試日期後六個月全部銷毀及從電腦檔案內删除。因此，考試的有關文件（包括准考證和考試成績單，如適用）在資料銷毀及删除後均不會獲得重發。
