Accidents Don’t Just Happen!

We have all heard that before and unfortunately, sometimes it’s true.

So you can make a difference by keeping a positive safety attitude by practicing safe work habits for yourself, your fellow employees and your employer.

Oil & Gas Training

Accident Investigation

Many accidents may seem to happen for obvious reasons. To better understand all factors involved we do a accident investigation.

This investigation has two main goals:

To determine the cause of the accident

To use this information to prevent similar accidents in the future

Remember, an accident investigation’s goal is not to assign blame. It is to prevent anyone else, including you, from being injured.

Page 17: Oil & Gas Training-Drilling VersionOil & Gas Training ... · PDF fileOil & Gas Training-Drilling VersionOil & Gas Training-Drilling Version ... TM Oil & Gas Training ... regular intervals

17

Oil & Gas Training

Accident Investigation

When an accident happens we need to take the following steps:

First: Take care of the injured person

Second: Report to the supervisor

Third: Secure the accident area

Oil & Gas Training

Accident Investigation

The supervisor will note all injuries on the IADC daily drilling record book and write “YES” in the injury column of the time record.

All accident report forms must be completed and witness statements will be taken.

All forms must be completed even if no medical attention is required and the supervisor must report the injury in a timely manner to his assigned office. NOT REPORTINGNOT REPORTING accidents in a timely fashion can lead to disciplinary action.

Oil & Gas Training

Accident Investigation

If you are interviewed, understand that the investigator needs your help. Be honest and provide as much information as you can.

You may be interviewed even if you did not witness the accident. If you normally work in the area, or have previously performed the job, your experience might be valuable.

Page 18: Oil & Gas Training-Drilling VersionOil & Gas Training ... · PDF fileOil & Gas Training-Drilling VersionOil & Gas Training-Drilling Version ... TM Oil & Gas Training ... regular intervals

18

Oil & Gas Training

Accident Investigation

This information is gathered to help determine the exact cause of the accident, not to place blame on anyone.

Offer as many details as you can about the workers involved, thecondition of the equipment involved, and the area where the accident happened.

The facts found in an investigation are used to get at the cause of the accident, not for any other reason.

Oil & Gas Training

Accident Investigation

Sometimes a “near miss” incident happens which does not result in an accident but a “close call”.

Report any incidence of this nature to your supervisor because this may warn us about a problem before an accident actually occurs.

Lessons learned from “near misses” are far “less expensive”than accidents.

An accident investigation can create a safer, more productive workplace for everyone.

So do what you can to help. The Life You Save Might be Your OWN!

Land Transportation

We may not all be professional drivers, but most of us drive everyday.

Most Accidents are caused by one of FIVE factors:

Distraction

Frustration

Fatigue

Poor Vehicle Condition

Alcohol and/or Drug Use

Oil & Gas Training

Land Transportation

When we are driving, distraction comes in many forms.

Eating, reading a map and talking on the phone are a few.

Pull over to a safe area to accomplish these tasks.

Oil & Gas Training

Land Transportation

Staying calm and alert is critical when driving.

STRESS, FRUSTRATION and ANGER can cause you to lose focus. If you feel out of control, take a pause and count to ten. Pull over if you can, step safely out of the vehicle, and regainyour composure.

Check tires for proper inflation and have adequate thread.

Check headlights and flashers, replace any burned out bulbs.

On large vehicles, follow the DOT checklist.

Inspect your vehicle before you start out.

Oil & Gas Training

Land Transportation

Make sure the vehicle has all basic emergency gear such fire extinguishers, spare tire, tire iron, jumper cables, flashlight, and warning markers.

A first-aid kit and motor oil could come in handy.

Inspect your vehicle before you start out.

Since people come in different sizes and shapes, you need to make the vehicle suit your body.

Adjust items such as seats and mirrors to suit your needs.

Oil & Gas Training

Land Transportation

Become familiar with the vehicle you will be driving.

Go for a test drive to understand turning radius and stopping distance before making your trip

Oil & Gas Training

Land Transportation

The use of alcohol is a factor in over 50% of all fatal accidents.

It is OUR COMPANY POLICY to have a drug and alcohol-free workplace. Alcohol and drug use while operating a vehicle, or while on the job, is unacceptable.

Always inform your supervisor if you are taking a prescription drug given by your doctor which may impair your ability to operate a vehicle or do your job.

22

Oil & Gas Training

Land Transportation

Wearing a seat belt is one of the most important factors when driving a vehicle.

Other indicators which make driving less dangerous are:

Obey traffic signs & speed limits (public roads or drilling lease)

Signal early for lane changes & turns

Watch for trouble on the road

Don’t tailgate

Anticipate possible accidents

Oil & Gas Training

Land Transportation

Exercise good judgment when weather conditions make the road more hazardous. Allow extra time to reach your destination.Be aware that alternate routes may be necessary.

Oil & Gas Training

Land Transportation

Dusk and dawn are the most dangerous times to drive.

Turn on headlights to be sure others can see you.

Use even more caution if road conditions are poor due to weather or other factors, when driving at these times.

Oil & Gas Training

Land Transportation

Traffic accidents happen to the safest drivers. In case of an accident, stay calm and do not move the vehicles unless someone is in immediate danger. Turn off the engine and check for injuries. Do not move anyone until qualified medical help arrives.

If an accident occurs, call the police immediately or ask passers-by to do so. Contact your supervisor as soon as possible.

DO NOT LEAVE the accident scene. Exchange information with the other accident participants and witnesses. Acquire a copy of the accident report.

Oil & Gas Training

Let’s review the important points of Land Transportation:

Vehicle Accidents are the #1 Cause of Death on the job

Driving Safety is not just for Professionals

Stay Focused, Alert and Sober when Driving
