AMENDMENT 1

CITY OF COCOA

Amendment to the Scope of Work for the Well Field Design

Task Order 29 was approved by the City Council on September 26, 2012.This amendment to Task Order 29 indudes the changes to the scope of work to be

performed by CH2MHILL for the Wellfield SCADA Design. Replace entirely the originalscope of work for Task Order 29 dated 9/ 26/ 12, with the revised scope of work provided

below.

Well Field SCADA Design

Services proposed by CH2M HILL Engineers, Inc. in this Task Order will be performed inaccordance with the Agreement for Engineering Services for Water Supply, Treatment,Transmission and Distribution System Projects( AGREEMENT) approved by the City ofCocoa on November 11, 2008.

Background

The City of Cocoa has identified issues associated with the monitoring of well flows andpressures. As required by the SJRWMD Consumptive Use Permit and current FDEP rules,the City of Cocoa is required to monitor and report well flow data, from a total of 48 wellsites, on a periodic basis. In the current telemetry system, not all wells are remotelymonitored and several have communication issues preventing reliable remote monitoring.There are also several wells that do not have instrumentation installed for flow monitoring,pressure indication, or well level indication. The current telemetry system has also reachedthe end of its useful life and some of the components are not supported by vendors due tothe age and hardware/ software versions of the components.

The scope of this project is to design a new telemetry system to allow reliable remotemonitoring of all wells and to select/ specify instrumentation at the well sites to allowaccurate well flow, pressure, and level measurement. This scope includes evaluation of the

radio polling and potential use of fiber optic cable at selected well sites to obtain data fromthe well site PLC, selection of communication technology, wellfield communication systemdesign, and selection of instrumentation. Procurement, installation, testing, and constructionof the system are not included in this task order and will be included in a future task order.

The evaluation of the communications and polling required to obtain data from the wellsites may include a hybrid of radio telemetry, fiber optic communications, and/ or othertechnology as appropriate. Report generation and report automation for the wells andassociated instrumentation is not included in this scope of work.

Scope of Work

This task order will provide design of the well field spoke and hub communications system

with the hub located at the Wewahootee WTP. Sequentially polling PLCs will occur atindividual well sites to support central monitoring and control of the well field.

The following is a detailed description of each task required for the development of a set ofimplementation construction drawings and specifications for the wellfield SCADA upgrade.

A preliminary drawing list for this project is provided in Appendix A. The design basisincludes specifying equipment, model numbers, and required options.

CH2M HILL has completed a radio path survey for the City of Cocoa. The results of theradio path survey will be used as the primary basis for the communication design and radioselection for the design.

The design phase provides an evaluation of communication protocol technologies andalternate communications approaches for identified sites to provide appropriate solutions,

communications equipment specification, replacement control panels, instrumentation

specification, engineering design, installation design, integration requirements, calibrationrequirements, testing requirements, and requirements for commissioning of equipment.

Task 1 — Preliminary Design PhaseTask 1 includes 50% Design Documents for review by the City of Cocoa. The deliverablesfor this task will include preliminary Drawings and Specifications.Wellfield system design includes design for replacement of the well field control panel ateach well site, well site PLC, well site radio, well site antenna, well site antenna pole,

potential well site hardwired communications, wiring, surge suppression, well sitegrounding, Wewahootee WTP master radio system, and Wewahootee WTP master WellPLC for polling the well sites.

Task 1. 1 Well Field Communication System Design

CH2M HILL will host a project kickoff meeting at the City of Cocoa office to review thescope of services and limits of supply. A project workplan will be developed and reviewedat this meeting. Meeting attendees from the CH2M HILL team will include the projectmanager, design manager, electrical engineer lead, and I& C engineer lead.

In support of the communication system design, CH2M HILL will perform the following:Design the communications system between the well field sites and the WewahooteeWTP.

The communication design will be based on the results of the radio path surveyreport, field measurements, and evaluation of suitability of installing fiber opticlinks.

Evaluate the use of fiber optic links between well sites 15 through 19 and will

determine the cost for fiber optic installation versus installation of new 60' towers at

these well sites. Based on the analysis, the City will select the communication optionfor these well sites.

Develop a draft communication block diagram showing the communications systemarchitecture including PLC hardware, radio equipment, and other communicationsequipment.

Conduct one meeting with City staff to review the initial recommendations andcommunication system architecture.

Task 1. 2 Radio Equipment and Well Site Antenna and Pilot Testing

The radio selected during the radio path analysis project was the GE MDS SD2, operating inthe 217- 220MHz band.

In support of the radio equipment selection, well site antenna selection, and pilot testing,CH2MHILL will perform the following:

Evaluate and select a radio bandwidth frequency of 12.5KHz or 25KHz and thecommunication protocol type.

Procure three radios, one set of redundant master polling radios, attenuators, andoffice test antennas to simulate field conditions, and power supplies for in-house

testing purposes. The equipment will be utilised as part of the SCADA whole plantdevelopment system test environment. Equipment will be transferred to the

Wewahootee WTP as active units to be installed in the field during the constructionphase of work. Redundant master and three radios will be set up in the officeenvironment, anticipated signal levels produced and the polling scheme will beimplemented and tested.

Evaluate the use of and select either serial communication or Ethernet

communication protocols. Verify that expected poll cycle times are acceptable and

that communication failure algorithms function correctly.

Prepare a specification data sheet to be used for procurement, includingmanufacturer, model, size, type, and any specific options required. Prepareinstallation specifications and details to be used for installation.

Select antennas based on the radio path survey and field measurements for eachradio. Select antenna height by well location based on the radio path survey andfield measurements. Master antenna located at the Wewahootee WTP will be

designed and procured under a separate scope of work.

Prepare an antenna specification to be used for procurement, includingmanufacturer, model, size, type, and any specific options required. Prepareinstallation specifications and details to be used for installation.

Include the design for radios to be used for communications at each well site, the

master radio system at Wewahootee WTP, individual well site antennas, specific

mounting hardware, and cabling.

Task 1. 3 Control Cabinet and PLC

In support of the control system cabinet design and PLC system level design, CH2M HILL

will perform the following:

Prepare a specification to be used for procurement and installation includingmanufacturer, model, size, type, and specific options required. Prepare installation

specifications and details to be used for installation.

Develop a typical standard control panel layout to be used by all well sitesregardless of the differences in installed equipment. Specify up to two separate

cabinet type designs to accommodate one for emergency generators and one without

emergency generators.

Develop a typical panel wiring detail to be used by a panel manufacturer tocomplete the detailed wiring diagrams and build the panels. Scope includes up to 2different typical well site templates based on installed equipment.

Design and specify Surge suppression and UPS equipment. Surge suppression andUPS will be selected to meet the requirements of each installation location, will be

standardized throughout the wellfield, and all attempts will be made to match

equipment currently used in the Dyal WTP, Wewahootee WTP, and well field. Surgesuppression includes antenna cable, power signals entering control cabinet, and

analog signal wiring at both ends of the cable.Design will be based on City of Cocoa-selected PLCs. The PLC selected by the Cityof Cocoa for the well sites is the Siemens S1200 PLC. The PLC selected for the

Wewahootee WTP is the Siemens S7300 series. Coordination of specific model and

features will need to follow the requirements defined in the City of Cocoa's PLCstandards document.

Prepare a PLC specification data sheet to be used for procurement, including

manufacturer, model, size, type, and any specific options required. Prepareinstallation specifications and details to be used for installation by the panel builder.Include in the design the PLC processor, PLC racks Input/ Output( I/ O) modules,

power supplies, communication modules if required, and mounting hardware. PLC

I/ O card layout and wiring will be based on developing a standard design. Developtypical PLC and I/ O module rack layout drawing for use by the panel builder increating final detailed drawings.Specify interface cables and any other modules needed to communicate between thenew Wewahootee WTP master polling PLC and the Wewahootee WTP SCADA

system. Determine if the new polling master PLC will share the same cabinet as themain plant PLC or reside in a separate cabinet.

Design and specify systems to support the standard I/ O listed below. Add contactsto enable remote monitoring, power transfer, and starting/ stopping of generatorsystems.

o Typical well site I/ O will include the following:Digital Input( DI): Well pump in remote, Well pump run status,Check valve not closed (if available), UPS Alarm( if UPS exists),

Utility power fail, Well house intrusion, Control Panel Intrusion, ATS

Task 1. 4 Site Work and Demolition

In support of the site work and demolition, CH2M HILL will perform the following:Prepare demolition drawings and take photographs depicting panel demolition,equipment modifications, wiring to be removed, and instrumentation changes.

Develop new site plans to show new instruments.If fiber optic communications are provided between well sites 15-19, include site

plans showing cable routing, excavation, and backfill requirements.

Task 1. 5 Antenna Pole Design

In support of the antenna pole design, CH2M HILL will perform the following:The SCADA system antennas are expected to be supported by concrete poles withreinforced concrete foundations. The poles will be specified, and will be designed

and installed by a pole manufacturer for the specified loads and code requirements.The foundation system for the supporting poles will consist of cast-in-place concrete

foundations below grade designed by CH2M HILL.Perform a site visit to evaluate the suitability of a standard size concrete foundation.Standard soil conditions will be assumed for each location and existing soilinformation from any previous bores may be used to determine requirements forpole and foundation installations. Any deviations in soil conditions will be requiredto be re-evaluated by the geotechnical engineer and are not part of this scope ofwork.

Prepare a specification to be used for procurement including manufacturer, model,size, type, and any specific options required. Include specification for the concreteused for the pole foundations. Prepare installation specifications and details to be

used for installation including structural, electrical, and civil engineeringrequirements. Design scope of work does not include Geotech site investigations and

design will be based on typical soil characteristics in that area.

Include construction phase requirements for individual well site antenna installation

and alignment based on adjusting each antenna to optimize radio received signalstrength. Include the requirement to document the final installation direction and

radio signal strength which can be used as a baseline for future performance

assessments.

Demolition requirements indude demolition of the existing well site towers with thetower foundation abandoned in place.

Task 1. 6 Electrical Design

In support of the electrical design, CH2M HILL will perform the following:Prepare a specification to be used for procurement including manufacturer, model,size, type, and any specific options required. Prepare installation specifications anddetails to be used for installation.

Existing signal and control wiring from equipment, valves, and instruments will bereplaced with new wiring utilizing new conduits. Prepare electrical riser diagrams

for new instrumentation and existing control wiring modifications. Prepareinstallation specifications and details to be used for installation. Provide typical site

plans to show electrical connection points, instrument installations, and groundingrequirements.

Provide specification and drawings for grounding of the control panel, powersupply to the panel, surge suppression, instruments, PLC, antenna, and tower.

Review the current Emergency Generator wiring diagrams and provide wiringdesign to allow remote monitoring and remote start/ stop features.

Task 1. 7 Instrumentation Design

In support of the instrumentation design, CH2M HILL will perform the following:Standardize the instrumentation required at each well site to include flow, pressure,

level, and conductivity. Evaluate and specify instrumentation for well site locationsthat are missing the standard instrumentation. Specify new instruments only forcases where there currently is no instrument or where the City of Cocoa hasdetermined that the existing instrument needs to be replaced.

Prepare a specification to be used for procurement including manufacturer, model,size, type, and any specific options required. Include field mounted surgesuppression for transmitters.

Specify control panel and well house intrusion switches for sites that do not alreadyhave these devices. Include limit switches on existing check valves where retrofit orreplacement is possible.

Prepare installation criteria and documentation including requirements formounting, calibration, and testing.

The City of Cocoa will procure, supply, and install the level instrumentation at each wellsite. The level probes will be installed by a direct subcontractor to the City of Cocoa with theprobe cable complete and terminated in the control panel by the City of Cocoa' ssubcontractor.

Task 1. 8 Control System Integration

In support of the control system integration requirements, CH2M HILL will perform the

following:

Develop a switchover plan which includes provisions for both radio communicationsystems to be active at the same time. The switchover plan will include sequencingof new PLC and communications installation and integration of data with graphic

HMI interface.

Define integration requirements for current existing SCADA system. Defineintegration requirements for new SCADA system.

Refine operational control narratives for wells on the new SCADA system. Include

well selection based on water quality and well capacity. Define the data aggregationtechnique for well reporting data and location of the logic performing the dataaggregation. Define requirements for well flow totalization performed at the well

PLC, plant PLC, HMI, and Historian. Define failure modes for loss of

communication including response of the well PLC. Define failure modes forinstrument or signal failure and appropriate alarms. Define requirements for

variable types and variable manipulation for totalization and max/ min/ average

values required for monthly DEP report data.

Develop test plan and documentation standards for control system testing.Review automated reporting scheme to determine if there are additional variablesneeded to be added for the well locations.

Task 1. 9 Well Documentation

In support of the well documentation requirements, CH2M HILL will perform the

following:Create a new typical well P& ID that reflects the current system and include the new

instrumentation that is being added.Define O& M requirements and construction phase submittals.

Define as-built drawing requirements.Define project sequencing requirements to organize and coordinate work betweenthis project and other concurrent projects at the WTP.

Task 1. 10 50% Design Review Meeting

A 50% design phase review meeting will be held with the City of Cocoa. CH2M HILL willprovide the meeting agenda, meeting summary and action items resulting from thismeeting. The design requirements and criteria will be finalized after receipt of the City ofCocoa' s comments at the 50% review meeting. The design will be considered frozen for theremainder of the design phase and any additional City of Cocoa changes received beyondthis meeting will be considered for mutual review as a change to the scope and fee.Task 1 Deliverable:

Preliminary design documentation( 50% design level) package includingspecifications for material and installation and drawings. Six (6) sets of specification

documents and 11" X 17" drawings.

Task 2 — 90% Design

CH2M HILL will provide 90% design phase Drawings and Specifications suitable for

implementation by the designer for City of Cocoa review. This set of documents willinclude the incorporation of the comments from the 50% Review Meeting.A 90% design phase review meeting will be held with the City of Cocoa. CH2M HILL willprovide the meeting agenda, meeting summary and action items resulting from thismeeting. Design documents will not be suitable for use as contract documents for biddingand construction contracting.Task 2 Deliverable:

90% design documentation package including specifications or data sheets formaterial and installation and contract drawings and installation details. Six (6) sets of

specification documents and 11" X 17" drawings

Task 3 Final Well Field SCADA System Design

CH2M HILL will provide Final Construction Drawings and Specifications suitable for

implementation by the designer for the City of Cocoa review. This set of documents willincorporate the comments from the 90% Review Meeting as well as any constructabilitycomments.

Task 3 Deliverable:

Final construction package including specifications or data sheets for material andinstallation and drawings. Six( 6) sets of specification documents, three( 3) sets of 11"

X 17" drawings, three (3) sets of full size drawings (22" X 34"), and one electronic

version of these documents.

Task 4— Probable Construction Cost

Based on the information developed as part of this design, CH2MHILL will prepare a Raw

Order-of-Magnitude (ROM) estimate of probable construction cost. This estimate will beconsidered accurate to within plus 30-percent to minus 15-percent of the actual project cost.

General Project Assumptions

General

1. The design will be based on the federal, state, and local codes and standards in effect at

the start of the project. Any changes in these codes may necessitate a change in scope.2. The drawings will follow CH2M HILL CAE/ CAD standards. CH2M HILL master

specifications for implementation services will be used as the basis for all specifications.3. With the exception of the final review, the project team will not stop during formal

reviews of submittals.

4. The City of Cocoa will provide drawings and any other available information in theOWNER' s possession; however, such drawings and other documents may not be reliedupon to be accurate for design purposes.

5. City of Cocoa will provide written review comments for draft deliverables within twocalendar weeks or deliverable will be considered acceptable as submitted.

6. Application Software development, Services During Construction( SDC), procurement,

installation, and testing services are not included in this scope of work.7. Master radio tower, master antenna, and tower concrete foundation design are included

under Wewahootee WTP upgrade scope of work.

8. Network security and segmentation design are not in the scope of this project.

Permitting1. No permitting services are included in this SOW. Permitting services, if required, will be

included in the procurement/ construction phase of work.

2. The radio antennas will be provided via a performance specification with the antenna

installer obtaining the applicable building permit(s).3. The City of Cocoa will be responsible for payment of all permit application fees

Radio Licensing1. Radio licensing application services for the radio system will be provided in the

construction phase contract.

Demolition

1. No salvaging or relocation of materials and equipment is required. The City of Cocoawill provide available drawings of these facilities for use as reference drawings in the

construction documents. Annotated photographs will be used in the drawings to

convey demolition details.Electrical and Instrumentation& Controls

1. SCADA programming will be provided under a separate task order(s)

Schedule

This proposal will cover services and activities to be delivered through May 31, 2013. Adetailed project work plan will be developed in conjunction with the City of Cocoa' sscheduling requirements. CH2M HILL will commence work within 1 week of receiving awritten notice to proceed for the project.

The anticipated schedule shown below is based on the number of weeks after the formal

issuance of the notice to proceed by the City of Cocoa:Project Kick-Off Meeting and Site Visit 2 weeks

Compensation

This task order will be executed as firm fixed price for labor and expenses. Subcontracts and

material procurement will be contracted at actual cost, plus applicable taxes, plus 10%markup.The firm fixed price for this task order for the labor and expenses portion of the scope will

be$ 391,609 as shown in Compensation Table below. The allocation for material costs for this .

task order will be$ 15,400. The total fee for this task order is$ 407,009.

CH2M HILL will provide the City of Cocoa a monthly invoice based upon the schedule ofvalues as detailed in the Compensation Table below, along with a summary of activitiesperformed under this contract. An estimated earned value( percent complete) for each

project subtask will be the basis for progress payments.

Material costs will be invoiced based on actual costs plus markup as defined above.This proposal is effective for 90 days from the date submitted.

V. CONSENT AGENDA:

1. To approve participation in a cooperative purchase agreement for the purchase

of new SWAT ballistic vests through Special Products Group ( a subcontractor of

Safariland, LLC) under the Florida State Contract " Body Armor Protective Vest" — contract# 680-850- 11- 1. ( Chief of Police)

2. To approve amendment 1 to Task Order 2012- 29 with CH2M Hill Engineers, Inc.,

for the Wel[ field SCADA Design Project effective October 1, 2012, through

September 30, 2013. ( Utilities Director) p

Approved. Task Order Request Form and Amendment 1 to be executed and a

certified copy provided to CH2M Hill Engineers, Inc.

3. To approve the proposal by Connect Consulting, Inc., ( CCI), High Springs, Florida,

for hydrogeological services for well rehabilitation (project no. WS1106), utilizing

the City of Palm Coast' s RFQ-J- 102, services contract for professional

hydrogeological services, which is effective April 15, 2010, through April 14,

2013. ( Utilities Director)

Approved.

4. Approve participation in a cooperative purchase agreement for the purchase of

vehicles and associated equipment utilizing the Florida Sheriffs Associationvehicle/ equipment purchasing contract # 12- 20-0905, which is in effect until

September 30, 2013. The Ford vehicles will be purchased through Duval Ford of

Jacksonville, Florida; the Mack dump truck will be purchased through NextranTruck Center of Orlando, the awarded dealerships for the contract. The police

equipment will be through EVS of Pompano Beach, Florida, the low quote.
