At NUH, we recoginise the fundamental role general pracitioners and family physicians play in the healthcare continuum in Singapore. We are therefore committed to engage GP partners in the community actively and building stronger partnerships - to achieve seamless, quality care for our mutual patients. GP Liaison Centre (GPLC) is thus set up with the aim to facilitate rapport and communication between physicians in private practice and NUH’s doctors. It acts as a central coordinating point between GPs and the hospital. Contact us, if you require assistance in any of the following areas:

- Patients’ diagnosis reports after discharge from Specialist Outpatient Clinics (SOCs) and/or Accident and Emergency (A&E) Department

- Contact with Specialists - for advice on patients’ conditions and treatment- GP Continuing Medical Education (CME) Programmes- NUH Services and Events- Participation in Shared Care Programmes

NUHSharedCareProgrammesAt NUH, we believe that the vision of providing excellent clinical care if achieved by doing what is right for the patient based on best evidence and quality, as well as getting the overall processes of care right. This means that patients are treated at the appropriate setting for their medical needs by the right providers, with the most efficient use of resources. For chronic illness patients especially, the largest proportion should be cared for, ideally, in the community and primary care settings, where our GP partners play a vital role in offering continuing and comprehensive health care for each patient and their families.

Supporting our medical specialties in various shared care initiatives, GPLC hopes to build closer intergration and cooperation between GPs and NUH, and establish a coordinated healthcare continuum from the hospital to the community, and vice versa.
