PROGRAM GUIDELINES

• Must be a residential natural gas heatingcustomer of Northern Utilities (Rate CodesR5 and R10 for New Hampshirecustomers; R2 for Maine customers)

• Account must be in customer’s name

• Rebate amount not to exceed actualpurchase price or $25, whichever is less

• Limit one rebate per customer accountper year 

• Valid for qualifying measures purchasedbetween May 1, 2008 and April 30, 2009

• Entire form must be completed for rebateprocessing and submitted with datedreceipt(s) and original UPC code frompackage(s) within 45 days of purchase
