In first-line CML, Glivec® remains standardfor large majority of patients
Glivec®: established, proven standard treatment for Ph+ CML1
• 200 000 patient years of treatment• 89% patients survive at 5 years2
• 5 years of data – strong efficacy, well tolerated, easy to manage
First cancer therapy to show declining rate of disease progression over time
1 Philadelphia chromosome-positive chronic myelogenous leukemia2 Survival at 5 years is 95% when deaths unrelated to CML are excluded

CML treatment recommendations emphasize importance of optimizing Glivec® therapy first
Glivec® preferred as initial treatment, with standard dose 400 mg/dIf sub-optimal response or failure:• First choice: dose-escalate
Glivec® to 600 or 800 mg/d• Rule out highly-resistant mutation
(in case of treatment failure)• Consider stem cell transplant or
investigational agent

Recent Nature Medicine article reported 10 cases of severe congestive heart failure (CHF) in patients who had been treated with Glivec®
• Most of these patients had pre-existing conditions including hypertension, diabetes and coronary artery disease
Novartis has further evaluated all available data from clinical trials and spontaneous reporting. Findings:• Cardiac events are uncommon
• CHF and left ventricular dysfunction (LVD) occasionally reported
• In a Phase III study, severe CHF and LVD was observed in 0.7% of patients taking Glivec® and 0.9% of patients taking IFN + Ara-C
Follow-up actions have included:• Revised US Gleevec® ‘precautions’ labeling
• Issued letter to HCPs with text of new label
• Recommended monitoring of patients with known cardiac disease or risk factors

Phase I studies about to begin, targeting patients with advanced breast cancer, glioma and prostate cancer
Genetic mutations leading to PI3K pathway activation are very common in human malignanciesSuch mutations can include:• Loss-of-function mutations
affecting negative regulators of PI3K, such as PTEN
• Gain-of-function mutations affecting activators of PI3K
PI3K inhibition and cancer suppression
