KERTH: I would like to thank Dr. Roe for his kind comments and con- gratulate him on the successful management of a very difficult case. I n response to Dr. Bartley’s comments, I would like to reiterate that in the majority of instances of aortic insufficiency following or resulting from a dissec- tion starting just above the aortic valve, the valve itself is remarkably normal. The insufficiency results from a loss of support of the aortic annulus with pro- lapse usually of the noncoronary leaflet. Reconstruction of the aortic root pro- duces a completely competent valve. We have always been quite conservative in our approach to the replacement of a valve which can be restored to normal func- tion by a reconstructive technique. NOTICE FROM THE EDITOR When plans were made originally for bimonthly publication of The Annals of Thoracic Surgery it was anticipated that it would be a 96-page journal. How- ever, every issue in the first two years of publication has far exceeded or will exceed this page limitation. The increased size of issues has been subsidized principally by The Society of Thoracic Surgeons, although the Southern Thoracic Surgical Association has also contributed generously. Due to the wealth of material available this year from the meeting programs of The Society of Thoracic Surgeons and the Southern Thoracic Surgical Associ- ation, it has been necessary to suspend temporarily the publication of collective reviews, and acceptance of submitted manuscripts has been limited. Starting with the January, 1967, issue, the Annals will be published monthly. As might be expected, this will necessitate an increase in the subscription rate. It is anticipated that publication of collective reviews will be resumed with monthly publication and that space will be available for publication of selected articles, notes, and case reports in addition to meeting papers.
