Complete a new 2017 Application Part 1 – Fill & Sign Application Form Part 2 – Sign GNWT Birth Certificate Release Form Part 3 – Sign Church Records Release Form 2. Submit signed Application and Forms to NWTMN Copy of forms can be obtained at your local Métis Council Office or found online at www.nwtmetisnation.ca Every successful Applicant must have a Birthright Ancestor defined as a person who is; • Born on or before December 31, 1921 • A person of Cree, Chip or Slavey ancestry • Resided, Used or Occupied the Agreement Area (basically South Slave Region of Northwest Territories) Identification of a Birthright Ancestor is done by our Genealogist and is supported through collection of vital statistic/historic records Genealogist must collect an evidence chain connecting you to your Birthright Ancestor, the Métis Nation pays for collection • An Evidence Chain is documents such as Birth Certificates and Baptismal Certificates with names of parents • Documents are collected for every person directly connecting you to your Birthright Ancestor 1. Must have a Genealogical Research Report issued recommending Approval and signed by the Genealogist 2. Must be 16 years old 3. Must complete a Membership Card Application Form 4.
