    Yakama is the only healthy heart project in the NWIt uses a unique approach the pharmacist work with the patient on the intervention

    DPP16 week lifestyle classes48 participants per yearFocus on nutrition, physical activity and weight loss (7% of body weight)Southern Oregon project has been a model project for consortiumWarm Springs DPP model*In November 2012 we held our Second Annual Diabetes Awareness Day which was in collaboration with IHS Model Diabetes Program, Diabetes Prevention Program and Confederated Tribes of Warm Springs SDPI Community Directed grant programs. We had a number of guest speakers and presentations from topic experts throughout Central Oregon. This was a day-long seminar which was attended by 111 community members. The event included healthy breakfasts and lunches and a number of fitness activities. Target audience was adults(see attached video clip)In February we held our 20th Annual Heart Smart Dinner and Health Fair. There were 255 community members who attended. The health fair included approximately 30 different education tables including tribal departments, IHS, and other community groups from throughout Jefferson County. A local childrens dance group provided entertainment during a healthy dinner of local salmon. Early Childhood Education classes provided decorations with an emphasis on healthy living and heart health. Focus was the entire Warm Springs community with collaboration for the event between Model Diabetes Program, IHS Diabetes Prevention, and Tribal SDPI grant. IHS Diabetes Prevention Program participates in the Holiday Parade and the Pi-ume-she Parade.IHS Diabetes Prevention Program staff continues to provide monthly diabetes fun runs/walks for community participants in collaboration with the tribal diabetes prevention staff members. Pictures are from the Bunny Run in late March. Tribal SDPI staff members continue to do monthly fun runs with the students of the Early Childhood Education Program. They also sponsor a community garden for all who want to participate.

    *The Nooksack Diabetes Program along with the Health Clinic coordinated a Diabetes Health Fair "Treatment and Prevention" on 5/22/2013 from 10am to 2 pm - Our target audience was Nooksack Community tribal members with diabetes and community members who may be at risk (everyone welcome!). We had 63 who signed in along with clinic staff who staffed booths and outside vendors with booths. We had 17 booths, including foot exams, diabetes shoe fitting, traditional plants,nutrition, A1C, blood sugar and BP testing, upcoming "Water Challenge" promotion with information on sugar in beverages, eye care, behavioral health and more. We provided a healthy lunch and had a speaker during lunch who spoke on "Taking Control" The speaker was sponsored by the Sanofi A1C Champion Program. We provided a number of tools for diabetes management such as logbooks, Diabetes ID bracelets, DVD "Living with Diabetes", glucose tablets, lotion, nail files, hand sanitizer, and information on Low Blood Sugar treatment.     We had hosted our annual Klamath Walk; which had taken place on May 19th, 2013 in Klamath Falls, Oregon at Moore Park by Klamath Lake. This event is for Prevention and Awareness purposes. It is an event that is open to the community and had hosted 117 individuals this year. This event was planned and organized by the Diabetes Management Program and there was also a Nurses Station for glucose and blood pressure readings for those curious or for those we wanted to track their levels before and after physical activity. The Walk route was no more than 1.5 miles in length and the entire event lasted about 2 hours in length. Jessie

Reflecting on the way our ancestors lived during a time in which seeking sustenance was physically taxing and our current generation living in a time of disease, alcohol and substance abuse-all causing the terrible health of our community. Participants made the best choice they could have by getting up early, facing the chilly breeze and taking that next step toward health. History often repeats itself. Choose Health. *Monthly Diabetes Clinic The second Wednesday of each month all clinic staff focus on the care of patients with diabetes. In addition to routine clinical providers we have a contracted Podiatrist join us for additional diabetic patient care.Monthly Diabetes Support Group meetings The first Thursday of each month we have a patient centered Diabetes Support Group Meeting. This meeting is co-hosted by a lead diabetic patient and the Tribal RD/CDE. This class is mainly for individuals who have received previous DSME and are in need of review of information or motivation. Topics covered vary from month to month and include such things as kidney care, exercise, medications, meal planning, foot care and avoiding complications.Diabetes Self-Management Education (DSME) Patients receive DSME in a group setting or one on one with the Tribal CDE/RD. DSME classes are scheduled to begin the first week of each month. There are 4 classes to a set and each class is 2 hours long. Individual patient education needs vary and are addressed appropriately on an as needed basis.I Strengthen My Nation youth conference was held in March of 2013 Education was provided on Diabetes with a focus on diabetes prevention/nutrition/physical activity.Transformation Caf (TC) This is a popular activity that was started last year. The focus is on weight loss. TC ties in with the Diabetes/Diabetes Prevention Programs in that overweight and obesity are usually the common thread in lifestyle diseases such as diabetes.

    *Community OutingsYellowhawk snow-shoe outing on Feb 2 with about 15 participants; target audience was CTUIR community membersHeadstart gardening for 2 classes of children ages 3-5yo; 20 in each group for a total of 40; planted marigolds in community gardenIn July last yr and this year, we will repeat our Headstart walks to the garden for plant identification, tasting and then back to our cooking room for cooking what we picked. Last year, we had 15 students each day. This year will be twice per week.
