Class I:

An invasive electrophysiological procedure is

indicated when the initial evaluation suggests an

arrhythmic cause of syncope (in patients with

abnormal electrocardiography and/or structural

heart disease or syncope associated with

palpitations or family history of sudden death).

Indications

Clinical and ECG features Suggesting Cardiac Syncope

• Syncope during exertion or supine

• Palpitations at the time of syncope

• Suspected VT (e.g. heart failure or NSVT)

• BBB

• Mobitz 1 second degree AVB

• Sinus bradycardia <50 bpm

• WPW

• Long QT

• ARVD or Brugada S.

• Sustained by a macro-reentrant circuit

• Circuit is confined to the right atrium

• Episodes can last from seconds to years

• Chronic atrial flutter usually progresses to atrial fibrillation
