Vista Compatibility - 23587 All of the NextGen applications and utilities, version 5.4.29 and higher, support Microsoft Windows Vista operating system.

Important: You MUST disable User Account Protection (UAC) for Vista (Group policy and/or individual PC) prior to upgrading to NextGen 5.4.29.

To Disable UAC for Vista for large networks:

1 Click Start. 2 Select All Programs > Run. You are prompted with the Run dialog.

3 Enter gpedit.msc and click OK. You are prompted with the Permissions dialog.

4 Click Continue. You are prompted with the Group Policy Object Editor window.

5 Browse to Computer Configuration > Windows Settings > Security Settings > Local Policies > Security Options.

6 In the window on the right of the Group Policy Object Editor dialog box, locate the User Access Control policies and disable all of the highlighted policies, as shown in the following example.

To Disable UAP for Vista for individual workstations:

7 Click Start. 8 Select All Programs > Run. You are prompted with the Run dialog.

9 Enter msconfig and click OK. You are prompted with the Permissions dialog.

10 Click Continue. You are prompted with the System Configuration window.

11 Select the Tools tab.

12 Locate and select Disable UAP (UAP might change to UAC in next Vista beta builds and in the RTM version).

Note: You can re-enable UAP by selecting the Enable UAP line and then clicking the Launch button.

13 Click the Launch button.

A CMD window is prompted. Once the command is done, you can close the window.

14 Close the System Configuration window.

15 You MUST restart your workstation in order for the changes to apply.

Use CLP02 Element to Determine COB - 20558 When the primary and secondary payers on an encounter have the same ERA Payer Number, the ERA Import function now uses the CLP02 element to determine which COB to use.

Ability to Use People Lookup with Person Merge - 20820 You can now perform a people lookup search with the Person Merge feature for situations when the pre-populated search scenarios do not apply. Clicking the Exact People button initiates People Lookup for the merge.

Search Ahead Limited to Open Slots - 21064 When the application is configured to permit only one appointment per slot, the Appointment Search Ahead feature no longer returns appointment slots that have an appointment booked. Previously, the Appointment Search Ahead returned all the slots that met the search criteria. The Appointment Search Ahead limits the slots it returns to open slots regardless of whether the appointment event is allowed in the slot category or not.

Enhanced Event Chain Interval Settings - 21458 There are now no limits to the number of minutes or hours you can enter when you configure event chain intervals. Previously, the number of minutes you could enter was limited to 60 and the number of hours you could enter was limited to 24.

Enhanced Encounter Note Email Function - 22205 When you email an encounter note, the application now appends the encounter number to the end of the email subject.

Description Fields Added to Payment Ledger - 22265 The Inv Des 1 and Inv Desc2 charge description fields have been added to the Payment Entry dialog box. These fields display when you select Invoice in the Source field.

Alert for Pending EMR Charges - 22941 When the Enterprise Preference, "EMR Manual Charge Processing," is activated and you access a patient chart for a patient who has pending charges, the application now displays an alert to indicate that there charges are waiting to be accepted. Previously, the application displayed an alert about pending EMR charges only when you accessed the Charge Posting dialog box for a specific patient encounter.

Location-Specific Logos for Statements and Fee Tickets - 23087 You can now import logos for locations in the Image Explorer tool of the EPM application. You can apply and print location-specific logos for fee tickets and statements. When printing fee tickets, the application determines if a location-specific log exists and if it does, it prints that logo instead of the practice logo.

The application prints location-specific logos for statements when one of the following conditions is true:

The statement options are set to Print One For Each Location.

The statement options are not set to Print One For Each Location, but all the encounters on the statement are for the same location.

Reference: See the EPM User Guide for instructions on importing logos for locations.

Override Policy Number on Fee Tickets - 23749 The application now displays the override policy number on fee tickets when the patient is not the subscriber.

Setting a Maximum Amount for Charge Entry - 24186 The Maximum extended charge amount option has been added to Practice Preferences to enable you to prevent the entry of incorrect prices during charge entry. You can now enter a maximum amount permitted in the Maximum extended charge amount field, which is located on the Charge Entry tab of Practice Preferences. When the option is set and a user manually types a line item charge amount in EPM Charge Posting that is higher than the Maximum extended charge amount value, the EPM application displays a warning and does not save the charge amount.

The Maximum extended charge amount setting does not affect SIM groups, event SIMs, or any charge entered from the EMR application, the Optik application, or any other external application that submits charges. The purpose of the feature is to prevent keying errors in the EPM Charge Posting dialog box.

Significant Events Created for Person Searches - 24848 The application was updated so that when you search for a person/patient, the application records the search criteria through significant events - if the event is enabled. For example, if you search for John Doe with a birth date of 01/01/1950, the application stores a significant event of (Last): Doe, (First): John, (DOB): 01/01/1950 in the sig events table. You can view the results by running the General > Sig Events report.

Patient Balance Includes Encounters Without Charges - 25191 The application has been updated so that the Patient Balance dialog box now shows encounters that had no charges against them. Previously, the Patient Balance dialog box displayed only encounters with charges.

NPI Column Added to Reports - 25543 In File Maintenance, the following reports were modified to include columns for NPI and DME NPI fields where appropriate:

Enhanced Chiropractic Billing - 2780 For chiropractic billing, you have the option to populate the 837 file with the initial treatment date. When you enter a date in the Initial Treatment Date field on the Encounter Maintenance dialog box and generate an electronic claim, the application automatically populates the DTP segment in the 2300 loop as follows:

DPT01 with a qualifier of 454

DTP02 with the initial treatment date

Support for National Drug Codes - 3997 The application now has electronic and paper support for National Drug Code (NDC) information (837 CTP and LIN segments). You can now add NDC information to a SIM and enable the NDC for claims. The following changes have been made:

The Narrative tab on the Service Item Library Maintenance dialog box is now the Other tab. Fields added to the Other tab are: National Drug Code, Basis of Measure, and Drug Unit Count.

To enable the NDC for electronic claims, the new Enable NDC coding for electronic claims check box was added to the Payer Defauts-2 tab.

To accommodate 1500 paper claims, the new Claim Printing Library selection of NDC Code, else Narrative was added to the option [19] Reserved for Local Use.

If the NDC information is entered in the SIM library and either the new payer option or the new claim print option is enabled, the NDC information is included on the claim for those SIMs.

New UB04 Claim Form - 14429 A new UB04 claim form has been added to the application. The old UB92 form is still available for use as we make the transition from the old to the new form. You can submit institutional paper claims on the form required by the payer. Medicare requires the new form by May 23, 2007.

The following changes have been made to the application to accommodate the new UB04 claim form:

Changed the references for UB92 Claims in the application to UB Claims to accommodate both form types. The changed areas include:

Payer master, Practice Parameters tab, UB92 tab – renamed to UB Claims Changed the Payer master, Practice Parameters tab, UB Claims sub-tab to remove FL

labels and use only descriptions. This change applies to fields where the form locator numbers are different between the two forms. These fields include:

Added a new provider field, Operating Provider, to the Payer master, Practice Parameters tab, UB Claims sub-tab. This field is only functional for UB04 paper claims and electronic claims.

Changed the Claim Printing Library to accommodate both forms. A new UB Claims box, added to the top of the Claim Printing dialog box, enables you to select either UB92 or UB04. Options that display in the library depend on the form selected. Only options associated with that form display. Options for UB92 claims have not changed.

Added options to the Claim Print Library for the UB04 form.

Added new paper claims qualifier settings to the Payer master file, System tab to accommodate qualifiers on the new form. The additions include:

Enhanced the Claim Request Lookup dialog box to provide the capability to differentiate between the old form and the new form when looking up paper claims. When the Media Type field is set to paper, options for UB04 and CMS 1500 are added to the Claim Form drop-down list.

NDC Options Added to CMS 1500 – 14430 The following options have been added to box 24 Supplemental charge-related data to display above line:

Support for Eight Diagnoses on Claims - 15772 New functionality has been added to support eight diagnoses on all claims except 1500 claims. The following areas in claims have changed:

Increased the number of diagnoses on a claim to eight diagnoses without a claim break. To enable this, select the Allow eight diagnoses and the Disable diagnosis claim break check boxes on the Claims tab in Practice Preferences.

Added a warning that displays on the Claim Production Status report when:

An electronic claim has more than four diagnoses and the Allow eight diagnoses check box on the Claims tab in Practice Preferences is not checked

Creating a HCFA or CMS 1500 claim with more than four diagnoses

Enabling the NPI at Enterprise and System Levels - 20355 Previously, you enabled the NPI only at the global level via the submitter profile. Now, if you submit claims to a clearinghouse and some payers accept the NPI while others do not, you can override the submitter profile option in the Payer master file by selecting an option in the Enable NPI on electronic claims field at either the system or practice level.

The effect of override depends on where you select it in the Payer master file:

System level - The payer master file setting on the System Parameters tab overrides the submitter profile setting for enabling the NPI.

Practice level - The payer master file setting on the Practice Parameters > Other tab overrides both the submitter profile setting and the system level setting.

New Options for Enterprise Chart - 17900 NextGen EMR has been enhanced to include optional Enterprise Chart functionality. The Enterprise Chart option facilities a multi-chart view of patient clinical data from multiple practices at the same time.

Important: The Enterprise Chart is a permanent option. Once you decide to enable the Enterprise Chart you cannot turn it off.

When you enable the Enterprise Chart functionality, you have an additional option to enroll all existing patients, as well as default the enrollment of all new patients, into the Enterprise Chart.

Removed Search Dialogs from Faxing - 22185 The NextGen faxing functionality has been enhanced by removing all Search dialogs that preceded the Send Fax window. The Send Fax window has been changed to default the Destination field based on the last selection made by you for the type of the item being faxed. Charge Entry and Risk Adjusted Diagnoses - 15772 In Practice Preferences, the Charge Entry Options section that was on the General tab is now on the new Charge Entry tab.

You can now increase the number of diagnoses from four to eight on the Charge Entry window in EPM by selecting the Allow eight diagnoses check box on the Claims tab in Practice Preferences.

In File Maintenance, you can indicate that an ICD9CM code is a risk adjustment diagnosis code. Then, when a risk adjusted code is used in EPM, the letter "R" displays next to the code. To set the indicator, you must enable the indicator by selecting the Differentiate Risk Adjusted diagnoses check box on the Charge Entry tab of Practice Preferences. Then you set the Risk Adjust Indicator for ICD9CM codes in the ICD9CM master file.

Importing Practice-level Information - 21413 You can import information from one practice to another to avoid repeatedly entering the same data in multiple practices. You import the information on the new Import tab on the Add/Modify Practice Information dialog box in File Maintenance. Importing information is useful if you create practices and want some master file data to be the same among them. You can import the following master file data, which appends to existing data:

Practice-specific sections of all Payer master files.

Practice Preferences, including Letters and Dunning Messages (location-specific settings do not import).

Contract Edits do not import because they pertain to security. Library information does not import.

Deleting Active Stations - 22714 Under General Options, you can set the new Universal Preference Delete all active stations records greater than 24 hours old so that the nightly job automatically deletes records from the active_station table (which controls single sign-on and patient sync) for active_station records that were orphaned due to abnormally terminating the application.

Deactivating Users Replaces Deleting Users from All Groups - 25056 According to CCHIT requirements, the features for deleting and deactivating users are combined into one "Deactivate" feature, which allows users to be manually and automatically deactivated from all assigned groups. In previous versions, a user could be manually deleted by other users but only automatically deactivated according to settings in the System Administrator application.

Machine Name Added to Remote Scanning Services List - 19974 The Location column has been added to the Remote Scanning Services List dialog box in the File Maintenance application. The new column displays the name of the machine where the remote scanning service is installed.

Image Full Screen Option Removed - 21810 Because it did not function as designed, the F11 option to resize the ICS Viewer image to full screen has been removed for 5.4.29. This feature could return in a future version of ICS after further development work is completed. For now, you can continue to make the ICS Viewer full screen by maximizing the Viewer window. You can increase the size of the image in the Viewer by using the Viewer's zoom controls.

Document Type List Expanded to 1000 Items - 23903 The Document Type drop-down list that displays when you file a document now displays up to 1000 items. Previously, the list was restricted to only 100 document types.

Option to Synchronize ICS and EMR Across Practices - 23979 You can now configure an enterprise to permit the ICS and EMR applications to synchronize across practices. When you activate the Enterprise Chart option in Enterprise Preferences, you can view an image in EMR that is stored for the ICS application for any of the practices within the enterprise. Previously, there was not a way to view images for a patient in EMR that were maintained by a separate practice within the enterprise.

Note: After you activate the Enterprise Chart option, you cannot turn it off.

Ability to Print Each Page of a Document Individually - 24303 A new option, Print each page individually, has been added to the User Preferences for ICS. When this option is activated and you send a large volume of pages to your printer, the ICS application now breaks each page into a separate print job. Previously, the ICS application created only a single print job for all the pages and this sometimes overwhelmed the available system resources.

Despeckle Feature Returned to ICS Viewer - 24644 The Despeckle annotation option has been returned to the ICS application. This feature was removed previously. You can access the Despeckle feature from the Annotation toolbar.

Maximum Number of Remote Scanning Services - 25260 When you enable Remote Scanning for ICS in Enterprise Preferences, you can now indicate the maximum number of Remote Scanning services that can be created by setting the Max Scanning Services field. This field appears after you activate the Enable Remote Scanning option. Previously, you could not set the maximum number of services allowed.

Three new buttons display on the Optik order forms when VisionWeb is licensed for your practice.

Ability to Charge for Eyeglass Balance Lenses - 20354 The ability to charge for eyeglass balance lenses has been added to the Optik application. Previously, you could not charge a different amount for a balance lens in an eyeglass order. The Balance Lens tab has been added to the Lens Maintenance master file. On the Balance Lens tab, you can specify whether or not you want to charge the same price as the prescription lens for the balance lens. If you do not want to charge the same price, you can specify the price that you want to use for balance lenses.

Implemented a new routing rules engine for routing appointments, medication renewals, communications messages and completed templates. Previously, all communication types were routed directly to the provider. Now, the practice defines complex rules that determine who will receive incoming patient notification messages.

Implemented an Instant Medical History questionnaire engine in the NextMD Web site

Implemented a new templates engine that greatly expands the usability and capability of online forms that have been posted online

Extended and improved the secure communications functionality. Improvements include:

Added Auto Create Encounter User Preference The NextGen EMR User Preferences has been enhanced to include an option that allows you to choose whether you wish an Encounter to be automatically created upon selection of a patient.

CCHIT Requirements In order to comply with the requirements of the Certification Commission for Healthcare Information Technology (CCHIT), the following features were added to the NextGen EMR:

If you do not, you might receive a sync warning message when you attempt to work with letters in EPM. You can sync all default letters or all letters of a specific type, such as Budget Letters.
