Is Insurance Coverage Required?
All students with a minimum of part time enrollment
status, and who live within a 60 mile radius of Berrien
Springs, must be insured by a policy that covers Injury
and Sickness.
Coverage of any dependents is optional for U.S. citizens
and permanent residents.
All international students (F1 and J1), and dependents (up
to the age of 26) must be insured by a policy that covers
injury and sickness.
Waiving Student Insurance
The student insurance may be waived by completing the
waiver during registration central and uploading proof
of comparable insurance.

Comparable standards of coverage must be met.
• Minimum coverage of $100,000 USD for both injury
and sickness.
• If you take a policy with a deductible, this must be less
than $500.
• Hospitalization must cover a per day minimum of
$1,100 USD and ICU charges of $1,500 USD per day.
• The policy should cover physician office visits.
• For international students (F and J visa holders), the
policy should include benefits of $25,000 for
repatriation & $50,000 for medical evacuation. All
dependents must be covered.
How do I know if it is Comparable?
Deadline to submit proof of alternate insurance or to
“waive” Andrews Student Insurance is 15 days after
registering.
Full premium will be charged to your account if the
waiver is not completed.
Deadlines for Waiving Insurance
Michigan Medicaid
• Granted on a month by month basis.
• Required to bring your current Medicaid card in to
our office at the beginning of each term.
• Failure to provide this information in any term will
result in a charge for insurance from that term
through the remainder of the school year and is not
refundable!
How much is my premium?
Per Year (August 15, 2014 – August 14, 2015)
$1,415 for single student
$3,590 for student + 1 dependent
$5,389 for student + 2 dependents

Signing up and Confirmation of Coverage
You may sign up for student insurance as you complete
your registration, if you chose the Student Insurance
Policy.
Available to view when you visit Registration Central.
The annual policy requires you to do nothing further and
provides coverage for the entire school year.
What does my insurance cover?
Covers injury & sickness
Immunizations – immunizations recommended by the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention are covered.
Routine Physical – NGL covers routine physicals for women only
Pre-existing Conditions – Covered (not excluded)
Vision & Dental Care – Pediatric services, including oral and vision care are covered. Otherwise, vision and dental care is not covered, unless resulting from a covered accidental injury.
Expenses in Home Country – Excluded for International Students
The deductible is a portion of the bill that must be paid out of the member's pocket before any insurance coverage applies.

The co-pay is the portion of the bill you are required to pay when you visit a physician or medical facility.

ER vs. Ugent Care
• Persistent chest pain
• Persistent shortness of breath
• Severe pain (abdomen or back)
• Loss of balance or fainting
• Difficulty speaking or vision
• Severe heart palpitations
• Newborn baby with a fever
• Head and eye injuries
• Broken bones or dislocated joints
• Deep cuts requiring stitches
• Seizures without a previous diagnosis of epilepsy
• Fever without rash
• Minor trauma such as a common sprain
• Painful urination
• Persistent diarrhea
• Severe sore throat
• Vomiting
Prescription medication: 50% of actual charge is
reimbursed.
Submit a completed claim form to the ASRM
• Pharmacy printout that includes the rate paid.
• Copy of the receipt must indicate:
• Name of the prescription
• Cost
What do I do when I need to see a doctor?
1. Find an Preferred Provider before you seek treatment.
– Lakeland Care Inc.
– University Medical Services
– Physician Hospital Organization (PHO)
2. Services Outside the local area.
– PHCS Network
– Nationwide network , “MultiPlan”
Claim Payments
If the doctor billed your insurance company –
• You will receive an EOB (Explanation of
Benefits) from ASRM, LLC, (claims
administrator).
• If you did not pay the co-pay and co-insurance
yet, you will need to make that payment to your
provider at that time.
How do I claim for bills that I paid?
If the doctor did not accept your insurance card and you
paid the bill at the doctor’s office –

• Send the completed claim form, with an itemized bill, from
the provider, within 30 days to:
