    organisation responsible for helping you

    and your family get the services you

    need where and when you need them.

    Each year we talk with hundreds of

    local people about what you want from

    your local services and plan for current

    and future health and wellbeing needs.

    This helps us have the right services in

    place to meet the requirements of our

    communities.

    Your Guide to Local Health & Wellbeing

    Services is delivered to every

    household in Knowsley keeping you up

    to date with all the services we deliver in

    your local area and how you can access

    them.

    The guide contains lots of useful

    information and contact details about

    where to get the right Health & Wellbeing

    advice and services in Knowsley; from

    leisure services to help you keep fit and

    healthy through to the health and social

    care services you need if you become

    unwell or need some support.

    Healthcare Services

    Knowsley Health & Wellbeing offers awide range of community health care

    services including GPs, dentists and

    Walk-in Centres. The map on page

    16 has details of the health services

    available near where you live.

    Social Care Services

    A range of social care services are

    available for children and young people,

    adults and older people. To find out

    what social care services are available

    in Knowsley you can call the Knowsley

    Access Team on 443 4335 who can give

    you information and advice, and put you

    in touch with relevant services.

    Support for CarersMany people in Knowsley care for a

    friend or relative who is frail, disabled

    or mentally or physically ill. Caring for

    someone can be rewarding but also

    very challenging so it is important that

    you have somewhere to turn if things

    get tough.

    The Knowsley Carers Centre is a charity

    which provides advice and support

    to carers. You can visit the centre in

    person at 143 Bewley Drive, Kirkby,

    L32 9PE or call on 0151 549 1412.

    Leisure Services

    To help you stay fit and healthy

    Knowsley has leisure centres across the

    borough. The map on page 16 shows

    your nearest leisure centre. If you would

    like more information about your local

    leisure facilities call Active Knowsley on

    0151 443 2200.

    Cultural Services

    Knowsley has a varied programme of

    cultural events taking place across

    the Borough. Keep an eye out for the

    NANCIE PRICE.WELCOMING RESOURCE

    CENTRES TO HER AREA.

    Nancie and Peggy love calling into the local Primary Care Resource

    Centre to meet old friends and check out the local artists.

    We live just a couple of minutes away

    from the Resource Centre at Huyton

    and since Peggy was born we have

    found it really handy to access the

    services we need. Regular postnatal

    check-ups for Peggy are easy she

    loves coming here, its so child friendly

    and welcoming!

    All the staff are really good, really

    helpful. Its lovely when they all

    recognise you as you walk through the

    door! A lot of the time the emphasis is

    on spotting any problems early, which I

    think is very positive.

    The facilities and services are excellent

    with GP, nurse and health worker

    access so you have everything under

    one roof. The environment is gorgeous

    and full of open spaces, bright coloursand interesting artworks. In fact I

    helped to create some artwork myself

    and Im really proud of it.

    Everybody talks to each other, so its a

    great place to catch up with neighbours

    and friends. Lots of local group

    meetings take place here so theres

    always something going on!

    I cant recommend the Resource

    Centres highly enough they are there

    for everyone to use and you might meet

    some old friends!

    MATTHEW CLARKE.CHANGE FOR THE BETTER.

    Matthew has taken control of his Diabetes and his life has changedcompletely with the help of Knowsley Health & Wellbeing Changes

    Weight Management Service.

    Around 4 years ago I went to my GP

    when I kept feeling sick and dizzy. I had

    also put on loads of weight I went up

    to 22 stone! Imagine my shock at being

    told I had Type 2 Diabetes and admitted

    straight to hospital!

    Over the next few days I had a longchat with a diabetes specialist who

    explained the consequences of this

    condition if not treated very scary!

    I was prescribed tablets and a

    therapy weight loss programme which

    unfortunately didnt work for me. I spent

    18 months struggling to lose weight,

    was reluctant to believe there was a

    problem and found it difficult to get

    motivated alone.

    Then I was referred to the ChangesWeight Management Service - group

    sessions, one hour a week for 10

    weeks. The difference was amazing,

    opening my eyes to all the issues

    surrounding food intake and healthy

    eating. The staff were all fantastic

    and really cared and understood the

    problems that diabetes raised.

    Alongside this therapy I was introduced

    to Activity for Life a rolling 12 weekgroup fitness programme of gym, aqua

    aerobics, Tai Chi and other exercises.

    The experience, for me, was part

    physical and part emotional. I have

    made loads of great friends.

    After 4 years of treatment, therapy,exercise and diet control I amnow down to 14 stone and feelingfantastic! I feel I have regained

    control of my life again.

    My decision not to let Diabetes beat me

    but to fight it with the help of Knowsley

    Health & Wellbeing services is the

    reason I want to tell others not to give

    in. The best help and advice is available

    right here on your doorstep.

    REBECCA LABON.HOW HER YOUNG FAMILY

    HELPED HER QUIT.

    Rebecca has quit smoking with the help of her local Pharmacy and

    now she has plans for a family holiday to Disneyland.

    My children had seen ads on tv about

    stopping smoking and they kept

    pestering me to quit. When I called into

    the Pharmacy at North Huyton Primary

    Care Resource Centre I saw all the Fag

    Ends posters and realised help was out

    there.

    I started the 12 week course a few

    weeks ago where I met my stop

    smoking buddy - she is really helpful.

    You get one to one consultations and

    advice. She works at the Pharmacy so if

    I need any help or advice I can just pop

    in to see her.

    When I first stopped, I tried patches,

    but they made my skin itch so now I

    use gum and its working great. I quit

    straight away and although its tough I

    have not touched a cigarette since.

    Now I dont get out of breath when

    Im chasing the kids up stairs. And the

    money I have saved we now put away

    each day. This goes straight to the bank

    ready to pay for our dream holiday at

    Disneyland, hopefully at Christmas.

    One of the side effects of quitting is

    that you need to have something to

    do with your hands - so I have taken a

    greetings card course where I spend a

    couple of hours at home in the morning

    designing cards and writing poetry.

    With a range of help and support

    available including pharmacies and

    Fag Ends you can really quit and also

    seeing the benefits of a holiday for the

    kids keeps me going!

    CHOOSE WELLGP SURGERYOf course, sometimes we all needto go to the doctors. Your localGP surgery provides a range of

    services including:

    general medical advice and treatment

    prescriptions

    referral to a specialist or a hospital

    immunisations / tests

    In recent years, many more nurses have

    been trained to give advice, take tests,

    and diagnose and treat illness thatused only to be dealt with by doctors.

    These nurses are very highly skilled

    health professionals. They are often the

    best person to see at the practice, and

    you can usually see them quicker than

    seeing a GP.

    Out of Hours

    Out of normal surgery hours, all GPs

    have an emergency service. This

    service is only for urgent medicalproblems that cannot wait to be treated

    until normal surgery hours. To access

    this service call your normal GPs

    surgery number.

    Emergency Dental Treatment

    If you need emergency dental treatment

    out of hours, call your own dentist

    and you will be directed to an on-call

    service. If you dont have a dentist, call

    NHS Direct who will be able to direct

    you to a dentist who can help.

    PERFECT FOR ME!

    Janine had been suffering from earache and felt she needed

    to see someone quickly!

    I had been suffering from earache for a

    couple of days and it wasnt getting any

    better. I knew if I made an appointment

    with my GP, I would have to take time

    off work and might have to wait until the

    next day so I decided I would pop into

    the local Walk-in Centre at Nutgrove

    Villa in Huyton Village centre.

    I was seen within 20 minutes, assessed

    in Triage by a nurse who checked

    through my medical history and was

    able to examine my ears. Once she had

    a good look around she diagnosed an

    infection and was able to prescribe a

    7 day course of antibiotics. I just walked

    round the corner to the local Pharmacy

    and within a few days the problem had

    cleared up.

    All the staff at the Centre werelovely and its ideal to have all the

    professionals, doctors, nurses and

    the like, under one roof with quick,

    easy access!

    If you feel you need a quickassessment of a medical problem

    the Walk-in Centres aroundKnowsley are ideal. All the facilitiesare modern and the staff are reallyfriendly and helpful and you can justWalk-in, good eh!

    As the name suggests, youshould only go to A&E or call999 if immediate emergency

    care is needed.

    But A&E departments tell us that

    around 4 in every 10 people who go to

    A&E dont need to wasting valuable

    time and resources needed for real

    emergencies.

    It is often very obvious if someone

    is seriously ill and needs immediate

    emergency care.

    An emergency is a critical or

    life-threatening situation and

    includes things like:

    Unconsciousness

    Losing a lot of blood

    Difficulty in breathing

    Severe chest pain

    Suspected broken bones

    We have probably the best emergency

    Charlie Brownes tip for staying healthy is to always have something

    to look forward to.

    My tip for staying healthy is to always

    have something to look forward to. I

    really believe a positive attitude is one

    of the reasons I continue to be fit and

    well. Another reason is the help I have

    had from Knowsleys Activity for Life

    programme.

    When I was in my forties (Im 72 now) Isuffered two heart attacks and had to

    take early retirement from work. Then

    six years ago I had a quadruple heart

    bypass after suffering ill health for a

    number of years.

    At the time of the heart attacks I

    thought I would be lucky to hit 50. But,

    now I feel so lucky to be still alive and

    enjoying life and I think its important

    to have something to look forward to

    every day, whether it is a visit from adaughter, going out somewhere or just

    getting up in the morning!

    At the time of my heart attack I was

    smoking like a chimney and had so

    much stress at work. And while I wasnt

    overweight, I was eating all the wrong

    type of foods. After my operation I

    was referred by my GP to the Activity

    for Life programme run by Knowsley

    Health & Wellbeing and Ive not looked

    back since. It changed my life; it helped

    me to come out of a sad and dark time

    after losing my wife to breast cancer at

    the same time as the heart bypass op. I

    was put on a free 12-week programme

    of activities including gym, tai chi andaerobics. I enjoyed it so much and felt

    so much happier as well as healthier.

    After the course I started to pay

    gym membership to continue all the

    sessions!

    I am now a Champion for the Knowsley

    at Heart programme, where I help to

    encourage other residents to have a

    free health check at any one of the

    community venues in the borough.Ive also joined an Activity for Life

    committee where we organise day trips

    for fellow participants. All in all life is

    great in Knowsley for me!

    Activity for Life is a GP referral scheme which forges a stronglink between health and council services in the Borough. It is thislink which has earned NHS Knowsley and Knowsley Council anational award the Reducing Health Inequalities Achievement

    of the Year at the annual MJ Awards for local authorities. Judgessaid they were impressed by the highly effective partnershipbetween the local NHS and the Council.

    Options provides all the services you

    expect for a GP including:

    Practice nurses

    Chronic disease management

    Minor Surgery

    Maternity Medical Services

    If you are a Knowsley resident or have

    a Knowsley GP you can access the

    Options service:

    at a GP-led health centre and 3

    at weekends and evenings

    You do not need to register with theservice to book an appointment with

    Pat has found Rachel and the IKAN team invaluable at solving some of

    her health and wellbeing problems

    Since I first met Rachel Ive found

    things are a little easier to get done.

    My garden had become a playground

    for the local kids and I was worried

    someone was going to get hurt. She

    arranged for the broken flags to be

    sorted and for someone to fit security

    lights at the back of the house.

    Since my husband died I have been

    struggling to get things done. Ive had a

    couple of bad falls, once where I broke

    my shoulder and another time broke my

    hip. But now I feel far more confident

    knowing Rachel and the IKAN team are

    just a call away!

    I have a walk-in shower which has been

    getting blocked leaving water up to

    my ankles. When I mentioned this to

    Rachel she had a professional round tofix it quickly, saying she didnt want me

    falling again.

    Everyone should know about the IKAN

    team, they are great for us oldies and

    no job is too big or small. They also set

    up social events to get folk out of their

    homes and making new friends.

    I dont know what I would do if I didnt

    have Rachel to call upon - she is a real

    life saver!
