How to Register
1. Create a Single Sign On (SSO) account from; https://clinicalaudit.hscic.gov.uk2. Select ‘Sign In’ Then select ‘Register’
Please note: If you have already registered, your GMC code will need to be added to your account in order to access the Consultant Check Report - Please contact [email protected] if you need to have your GMC code linked to a previous registration.

• Complete your details online.• Then complete your details on
the electronic registration form (available from www.hscic.gov.uk/og)

How to Register
• Email the completed form to your Caldicott Guardian. Your Caldicott Guardian will need to approve your access to the NOGCA data collection system by completing their details on the form then emailing it to [email protected] (HSCIC Contact Centre).
• The Contact Centre will process the form and grant your SSO account access to the OG Clinical Audit Platform. They will email you to confirm once this has been processed.
• You can now log into the Clinical Audit Platform from; https://clinicalaudit.hscic.gov.uk/og and enter your SSO details to log in.

• From the home screen select ‘Enter data’
• Enter the NHS number and Date of Birth

• You will be directed to the Patient Record page – enter data into each cell then select ‘Update’

• You can now create either a HGD or OG Tumour record for the patient by selecting ‘Add HGD Record’ or ‘Add OG Tumour Record’

• The Record Tree will now expand and allow you to add surgery, pathology, CRT, and ERPT records
• Select the record type to manually complete each cell until all data is complete

File Upload
• From the home screen select ‘File Submission Dashboard’
• Select ‘Browse’ to find the file on your computer
• Select ‘Upload’
once all files have
been selected

• Files are processed in a couple of minutes• A message will appear once they are
processed – select the ‘click here’ text to see if the upload was successful or if there are any warnings/errors.

• Remember:• CSVs must use the following names;
o Patient_.csvo HGD_.csvo Tumour_.csvo Surgery_.csvo Pathology_.csvo Oncology_.csvo ERPT_.csv
• You can add any identifying text after the underscore ‘_’o e.g. Patient_HSCIC_Jan2015.csv
• Sample patient’s tumour record in CSV file format;o 1111111111,28/02/1964,ABC123,03,1,01/01/2012,01/01/2012,WALES,01,01,0,

Check Success of Previously Uploaded Files
• You can also use the ‘File Submission Dashboard’ to check if previous files were uploaded successfully
• Click on the arrow to the right of the file you wish to investigate

Check Success of Previously Uploaded Files
• The record will expand to show file name, type, status and number of records
• Click ‘View details’ to see details of the errors/warnings generated

• This will reveal how many records were imported successfully, how many were imported with warnings, and how many were unsuccessful
• Please note: o warnings ( ) are designed to highlight data quality issues, a record which has
generated a warning has been uploaded, but does require further attentiono Errors ( ) mean that particular record has not been uploaded, the record needs to be
uploaded again or entered manually

How to View Reports
• Select ‘Reporting’ from
the homepage• Choose the report you
would like to view• The reports are
extracted into a csv file so that you can use locally

Deletion Log
• This report lists all records that have been uploaded by your Trust and subsequently deleted

Trust Extract
• View all the data submitted by your Trust• Once extracted this report can then be
filtered on diagnosis date, submission date, date of surgery, Consultant GMC code etc.

First Diagnosing Trust Participation Report
• Choose the date period and select ‘Export’• This will export a count of all the patients
(and associated records) diagnosed at your Trust along with the date the last record was created
• For example;

Once you’re registered, sign in to
the Clinical Audit Platform and follow
these steps:
1. Select ‘Reporting’ from
the homepage
2. Select either:Consultant Check This report allows consultants to verify their data before the data is extracted for the Consultant Outcomes Publication. The data is linked to individual GMC codes. The report is only available to consultants and will show the surgical cases assigned to your GMC code.
OR
Data Manager Check This report allows data managers to verify the data for the Trust before it is extracted. The report will show the surgical cases assigned to your Trust.

3. Select the Year of Diagnosis for the report you want to view and click ‘Submit’
4. You will be given the option to Open or Save the file

5. The report will be extracted into a CSV file so you can use locally.
The Consultant Check report shows all surgical cases submitted to the
audit against your GMC code.Please note that this report can only be accessed by Consultants and is verified
by GMC code.
The Data Manager Check report shows all surgical cases submitted
to the audit for your Trust.
Both reports contain the following data items;
Treating Hospital; Surgeon Responsible for original operation; Additional Surgeon
1 for Original Operation; NHS no; First Name; Surname; DOB; Sex; Diagnosis Date of Cancer; Date of Surgery; Main
Procedure; Patient Fitness; Surgical Complications; Death in Hospital; Post
