Making a support plan

Do you need to make a support plan? It can help you to work out how

much and what kind of support you may need.

The plan you make should be person-centred. A good, person-centred

support plan will:

� say what is important to you and for you

� say what things are like for you now and how you want your

life to be in the future

�be made with your circle of support and include family,

carers, and friends

� say what support you need to make your plan work

� have an action plan

Take time to think about what is important to you. Finding a home

is about you. Do not just move somewhere because there is

a space - look for somewhere that is right for you.

Talk to other people who live in the same type of housing that you

are looking at. And talk to people who have similar support needs.

How long will you live there? Just for a short while or for the rest of

your life? There are some types of housing that you may only be able

to live in for a short time.

What talents do you have? Think about what you can do

for yourself. What would you like to learn?

How much responsibility do you want to have? Do you want to be in

control of paying bills and looking after your home, or do you want

someone else to do this? This can be very different depending on the

type of housing you live in.

Perhaps you do not feel ready right now to live in the type of home

you want. You might think about living somewhere else first, so that

you can learn skills for when you are ready to move on to the home

you really want.

Things to remember:

� There may be a waiting list for some types of housing.

� Some types of housing are more difficult to get than others.

� Wherever you live:

○ Adult Social Care may pay for some support. This depends

on the amount of support you need. But, even if they say

they will help with support, they might still ask you to pay

towards the cost, if they think you can afford to.

○ You may also pay for private support.

○ You may get help for free from your family and friends.

Assistivetechnology

Equipment that helps you with things like communication or keeping safe. For example, a computer or emergency telephone alarm.

Outreach orfloating support

Support you get only when you need it, not all the time.

Housing relatedsupport

Support that comes with your housing to help with your daily life. It helps you to do things for yourself, rather than have them done for you.

Types of support

Personalassistant

Someone who works for you as your helper. You might pay for them using a Direct Payment or Personal Health Budget. This is money that Adult Social Care or the NHS give to people to spend themselves on their support.

Domicilliarysupport

Someone who visits your home and helps with daily living. For example, help to do cooking, cleaning, shopping or with personal care.

Family

Living with a family that supports you. This could be your own family or another family that you live with through the Shared Lives scheme.

Residential care Staff at the care home where you live support you day and night.

Live in support The person who supports you lives in your home with you.
