1. My CV Introduction Important note: This User Guide provides details for navigating, completing and submitting your Faculty Annual
Report (FAR) and generating the Questrom Multiyear Report (QMR) in My CV. There are additional User Guides for
other functionality, such as navigating and modifying your profile, publications, teaching activities and professional
activities (service). These can be found on the Training and Support page of the My CV Project Website.
Overview
My CV is a software system that is used to help Boston University collect, analyze and showcase the scholarly output and
professional activities of the University's academic community. The information collected in My CV includes
appointments, education and degrees, publications, teaching activities, grants, and professional activities.
Wherever possible data is prepopulated from both internal source systems, like SAP, and external source systems to
save time and reduce data entry.
Beginning in calendar year 2020, My CV will be used to complete the Faculty Annual Report and to generate the
Questrom Multiyear Report (QMR) - previously called the Supplemental Annual Report.
2. Accessing My CV You can access My CV at mycv.bu.edu where you will login with your BU login name and Kerberos password.

3. Review Your Profile Before beginning your FAR exercise in My CV, review your profile and update or add any missing data. The FAR is
generated using data collected during the review exercise as you add items from your publications, grants, teaching
activities, and professional activities modules or complete fields within the exercise. The QMR is generated using data
aggregated in the exercise as well as from data pulled directly from your My CV profile and modules. The first step for
completing both is to evaluate your profile and update existing records or add missing ones.

4. Accessing the FAR Exercise All of the data used to generate the Faculty Annual Report used by the Office of the Provost is collected in the FAR
exercise. The QMR will be generated from parts of the FAR exercise and additional data from the user profile and
modules. This guide will go over completing the exercise and illustrate the differences in the two reports.
4.1 Navigation You can access the FAR exercise in My CV in two ways:
(1) Menu Tab: Navigate to Menu > Build > Review Processes and select the exercise.
(2) My Actions: Navigate to Home and select Start from the My Actions carousel.

4.2 Start Page You will be brought to the FAR Start Page that contains basic instruction. To begin your FAR, click Start.
4.3 FAR Exercise Structure The FAR is organized in a hierarchy. The exercise as a whole is broken into different lists/sections. Information is collected for each list/section by selecting items from your profile or completing fields in the exercise. Supporting information is also collected and can pertain to individual items, lists/sections as a whole, or at the exercise level.

You will be asked to add Supporting Information at the exercise level for your tenure status.

Some sections require supporting information at
the list and/or item level. Please refer to the
instructions provided in the exercise for additional
guidance.
4.3.3 Items
Within a list/section, you may need to select items
from your profile, such as a publication or a course
taught. In order to associate an item to the list, it
must first be added to your profile.
In some lists/sections, such as Teaching Activities
and Works in Progress – Publication Pipeline, you
will be asked to provide item-level supporting
information.

4.4 Summary Panel The Summary Panel on the right of the exercise will provide status information
about each list.
A hard warning indicates key information is missing and must be provided.
A soft warning, represented by the symbol indicates that either supporting
information for the list or items associated with that list have not been
added or that fields have not been completed. The system will allow you to
proceed with this soft warning. It is only meant to provide a reminder that more
information can be added.

5. Complete your FAR
5.1 Tenure Information You’ll begin your FAR by indicating your tenure status.
You will indicate your tenure status, if tenured, provide your tenure year, and if tenure track, provide your tenure review
year.
If you cannot recall your exact Tenure date, you can simply provide the year.
After you have completed these field, you have three options to continue:
1. Cancel will erase any changes you have made to the page
2. Save and continue will save your changes and keep you on the current page
3. Save and exit will save your changes and bring you back to the FAR Welcome page
After you are satisfied with the information you have provided, you can continue the FAR by clicking Save and exit.
This action brings you back to the Welcome page.
After the FAR level Supporting Information has been added, you can move onto the next list, Leave. You can access any
list by clicking on the list’s name.

5.2 Leave(s) In the Leave list you will either be indicating that you did not take a leave or providing information about your leave(s)
over the past year.
If you were not on leave over the past calendar year, click (1) Add supporting information and check (2) Not Applicable.

If you were on leave at any point during the past calendar year, please add an item for each leave by clicking (1) Add
item, then (2) indicate the type of leave, (3) the semester(s), and (4) dates, then clicking (5) Save and exit.
To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to return
to the Welcome page or (2) Go to Teaching Activities to go directly to the
next list.

5.3 Teaching Activities In this section, you will select teaching activities (courses taught, courses developed, and additional teaching activities)
from the past calendar year to associate with your FAR. Courses taught over the past three years where you are listed as
the lead instructor have already been prepopulated to your profile. All other teaching activities, including courses
developed, must first be added to your profile before you can select them on your FAR.
To begin, click Select items.

All of your eligible teaching activities will display. You can either select individual teaching activities to associate with
your FAR by clicking (1) Select for Teaching or you can associate all of your eligible teaching activities by clicking (2)
Select All on the results page.
If you cannot find a particular teaching activity within your list of eligible items, it either doesn't have a reporting
date within the past calendar year or is not currently in your My CV profile. You will need to add teaching activities
to your profile before they will be available to add to your Faculty Annual Report.
If you wish to associate all of your teaching activities by clicking Select All, be sure that all items are displayed in
the results. To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.

For each course taught, you will need to add item-level supporting information regarding (1) percent responsibility, (2)
the number of TFs, (3) TAs, and (4) graders. To provide this information, you can click the soft warning symbol at the
bottom of each course taught. Once you have completed these fields, click (5) Save and exit.

If you do not have any teaching activities to report for the past calendar year, select (1) Add supporting information, (2)
check Not Applicable, and click (3) Save and Exit to continue.
To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to return
to the Welcome page or (2) Go to Advising to navigate to the next list.

5.4 Advising In this section, you will record your advising activity from the past calendar year. To begin, click Add item.
Complete the fields that are applicable to your advising activity from the past calendar year.
The text you provide in Summarize your special contributions to student advising will appear in the one page
summary report reviewed by the Provost.
Once you have completed all applicable fields, click Save and exit.
If you do not have any advising activities to report for the past calendar year, select (1) Add supporting information,
check (2) Not Applicable, and (3) Save and Exit to continue.

To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to return
to the Welcome page or (2) Go to Publications to go directly to the next list.
5.5 Publications In this section, you will select publications from the past calendar year to associate with your report. To begin, click
Select items.
You can either select individual publications to associate with your FAR by clicking (1) Select for Publications or you can
associate all of your eligible publications by clicking (2) Select All on the results page.

If you cannot find a particular publication within your list of eligible items, it either doesn't have a reporting date
within the past calendar year or is not currently in your My CV profile. You will need to add publications to your
profile before they will be available to add to your Faculty Annual Report.
Items that display with the data <No eligibility date> will also appear in the list of eligible items. This is mean to
prompt you to add dates to items where applicable. If these items are works in progress, they may be included in
your Works in Progress – Publication Pipeline section. If you have items displaying with <No eligibility date> you
should not use the Select All function.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.
Scholarly articles authored by BU faculty after February 11, 2015 are subject to the University's Open Access
Policy. To help you comply with the policy, My CV flags publications within your profile that fall under the policy
and faciliates the required deposit/waiver processes. You may contact the BU Library with any questions about
the policy and compliance.
If you do not have any publications to report for the past calendar year, select (1) Add supporting information, (2) check
Not Applicable, and (3) Save and Exit to continue.

To continue to the next section, you can navigate to the List Summary >
Where Next? and click (1) Back up to FAR - Questrom School of Business to
return to the Welcome page or (2) Works in Progress – Publication Pipeline
to go directly to the next list.
5.6 Works in Progress – Publication Pipeline
In this section, you will add works in progress to be included in your FAR. Items will come from your publications
module. To begin, click Select items.
For each work in progress you may (1) add supporting information at the item level. For scholarly articles, the (2) round
of review must be added. For any type of work in progress, (3) you may include a description. Once you have completed
entering this information, (4) click Save and exit.

After you have selected all of the items you wish to associate with your FAR, click “Done” in the Select Items ribbon
above.
If you do not have any works in progress to report for the past calendar year, select (1) Add supporting information,
check (2) Not Applicable, and (3) Save and Exit to continue.
To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to return
to the Welcome page or (2) Go to Distinctions and Awards to go directly to
the next list.

5.7 Distinctions and Awards In this section, you will select distinctions and awards from the past calendar year to associate with your report. To
begin, click Select items.
You can either select individual distinctions and awards to associate with your FAR by clicking (1) Select for Awards you
can associate all of your eligible distinctions and awards by clicking (2) Select All on the results page.
If you cannot find a particular distinction or award within your list of eligible items, it either doesn't have a
reporting date within the past calendar year or is not currently in your My CV profile. You will need to add
distinctions and awards to your profile (as distinctions/awards under professional activities) before they will be
available to add to your Faculty Annual Report.

If you wish to associate all of your awards by clicking “Select All”, be sure that all items are displayed in the results.
To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.
Only the top six distinctions and awards will appear on the one page report reviewed by the Provost. To rank your
distinctions and awards, navigate to the List Summary > What Next? and click (1) Order Items. Use the (2)
controls at the bottom right hand corner of each item to adjust the ranking. Once you are satisfied with the
rankings, click (3) Done in the Order Items ribbon.

If you do not have any distinctions or awards to report for the past calendar year, select (1) Add supporting information,
check (2) Not Applicable, and (3) Save and Exit to continue.
To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to
return you to the Welcome page or (2) Go to Internal Service (BU) to go
directly to the next list.

5.8 Internal Service (BU) In this section, you will select internal service activities from the past calendar year to associate with your report. To
begin, click Select items.
You can either select individual internal service activities to associate with your FAR by clicking (1) Select for BU Service,
or you can associate all of your internal service activities by clicking (2) Select All on the results page.

If you cannot find a particular service activity within your list of eligible items, that is because it doesn’t have a
reporting date within the past calendar year, it is not currently in your My CV profile, or it is already included in
your list of external service activities. Activities may not be listed on both internal and external service lists.
If you wish to associate all of your internal service activities by clicking Select All, be sure that all items are
displayed in the results. To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.
Only the top five internal service activities will appear on the one page report reviewed by the Provost. To rank
your internal service activities, navigate to the List Summary > What Next? and click (1) Order Items. Use the (2)
controls at the bottom right hand corner of each item to adjust the ranking. Once you are satisfied with the
rankings, click (3) Done in the Order Items ribbon.

If you do not have any internal service activities to report for the past calendar year, select (1) Add supporting
information, check (2) Not Applicable, and (3) Save and exit to continue.
To continue to the next section, navigate to the List Summary > Where Next?
and click (1) Back up to FAR - Questrom School of Business to return to the
Welcome page or (2) Go to External Service (non-BU) to go directly to the
next list.

5.9 External Service (non-BU) In this section, you will select external service activities from the past calendar year to associate with your report. To
begin, click Select items.
You can either select individual external service activities to associate with your FAR by clicking (1) Select for External
Servic or you can associate all of your eligible external service activities by clicking (2) Select All on the results page.

If you cannot find a particular service activity within your list of eligible items, that is because it doesn’t have a
reporting date within the past calendar year, it is not currently in your My CV profile, or it is already included in
your list of internal service activities. Activities may not be listed on both internal and external service lists.
If you wish to associate all of your awards by clicking Select All, be sure that all items are displayed in the results.
To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.
Only the top five external service activities will appear on the one page report reviewed by the Provost. To rank
your external service activities, navigate to the List Summary > What Next? and click (1) Order Items. Use the (2)
controls at the bottom right hand corner of each item to adjust the ranking. Once you are satisfied with the
rankings, (3) click Done in the Order Items ribbon.

next list.

5.10 Continuing Grants In this section, you will list existing grant funding that continued into or through the past calendar year. To begin, click
Select items.
You can either select individual grants to associate with your FAR by clicking (1) Select for Cont’ing Grants or you can
associate all of your eligible grants by clicking (2) Select All on the results page.

If you cannot find a particular grant within your list of eligible items, that is because it doesn’t have an eligible
reporting date range, it is not currently in your My CV profile, or it is already associated with one of the other
grants lists in your annual report. Each grant may only be associated with on grant section (new, continuing, or
proposal) of your report.
Sort by date (oldest to newest) to more easily locate and
select existing grant funding that continued into or through
the past calendar year.
If you wish to associate all of your grants by clicking Select All, be sure that all items are displayed in the results.
To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.

If you do not have any continuing grants to report for the past calendar year, select (1) Add supporting information,
check (2) Not Applicable, and (3) Save and Exit to continue.
To continue to the next section, navigate to the List Summary > Where

to the Welcome page or (2) Go to New Grant Funding to go directly to the
next list.

5.11 New Grant Funding In this section, you will select new grant funding from the past calendar year to associate with your report. To begin,
click Select items.
You can either select individual grants to associate with your FAR by clicking (1) Select for New Grants or you can
associate all of your eligible grants by clicking (2) Select All on the results page.
If you cannot find a particular grant within your list of eligible items, that is because it doesn’t have an eligible
reporting date range, it is not currently in your My CV profile, or it is already associated with one of the other
grants lists in your annual report. Each grant may only be associated with on grant section (new, continuing, or
proposal) of your report.

5.12 New Grant Proposals In this section, you will select new grant proposals from the past calendar year to associate with your report. To begin,
click Select items.
You can either select individual proposals to associate with your FAR by (1) clicking “Select for New Proposals”, or
choose to associate all of your eligible proposals by clicking (2) Select All on the results page.
If you cannot find a particular proposal within your list of eligible items, that is because it doesn’t have an eligible
reporting date range or it is not currently in your My CV profile. You will need to add proposals to your profile
before they will be available to add to your Faculty Annual Report.

If you wish to associate all of your grants by clicking Select All, be sure that all items are displayed in the results.
To help ensure all items are displayed, increase the number of results per page.
After you have selected all of the items you wish to associate with your FAR, click Done in the Select Items ribbon.
These items are now associated with you FAR.

If you do not have any new grant funding to report for the past calendar year, select (1) Add supporting information,
check (2) Not Applicable, and (3) Save and Exit to continue.
To continue to the next section, navigate to the List Summary > Where
Next? and click (1) Back up to FAR - Questrom School of Business to return
to the Welcome page or (2) Go to Attendance at Questrom and BU Events
to go directly to the next list.

5.13 Attendance at Questrom and BU Events In this section, you will add information about Questrom and BU Events you attended in the past calendar year. These
items will not come from your profile and must be created in the exercise. To begin click “Add item”.
For each item select (1) the event type and add (2) a description if necessary. Select (3) Save and exit to continue.
You can then click (1) Add item or (2) Back up to FAR - Questrom School of
Business to return you to the Welcome page.

6. Review your FAR To review your FAR, navigate to the Welcome Page > Summary Panel >
What Next? and click Export to Word or PDF.
You have the ability to export two versions of the FAR:
(1) QST Faculty Annual Report (Full Version): This report will display all information you’ve entered for your FAR in My
CV.
Review this report to ensure that your FAR is complete.
(2) QST Faculty Annual Report (1 page): This summary report will be viewed by the Provost as part of your annual merit
review.
Review this report to ensure you have properly ranked all items.

7. Complete your QMR By completing the FAR exercise, you have also completed sections that will be used to generate the Questrom Multiyear
Report. Some parts of the QMR are generated directly from the My CV user profile and modules so updating that data is
an important first step. Ideally any updates would have been completed prior to the FAR exercise.

8. Review your QMR To review your QMR, navigate to the Welcome Page > Summary Panel >
What Next? and click Export to Word or PDF.

Select the report style Questrom Multiyear Report
and click Download as PDF.
This report will display all information used to
generate your QMR from My CV.
Review this report to ensure that your QMR is
complete.
9. Submit your reports Once you have reviewed your reports and you are ready to mark them as
complete, navigate to the Welcome Page > Summary Panel > What Next?
and click Mark as done.
This will bring you to a confirmation page. Scroll to the bottom and click
Mark as done.
Your Department Administrator and Faculty Actions Manager will be
notified that you have submitted your FAR. They will then move your exercise to the next stage in the review process
