A. ABOUT THE SERVICE

The birth record is a permanent record of the person. However, the law somehow allows

certain modifications as when there is a legitimate event that causes the modification of the

person’s record, as in: a court order to effect a change or correction; or in conformity with certain

administrative procedures; or as a consequence of an approved quasi-judicial function by this

office. Marginal annotation is therefore inevitable.

B. WHO CAN AVAIL OF THE SERVICE

A person whose birth or marriage is registered at the MCRO of La Trinidad, Benguet

affected by the aforesaid legal modification is entitled to the issuance of annotated certificate of

birth, marriage. In case of death certificate, the nearest kin or surviving relative may avail of such

service

C. REQUIREMENTS

For civil registry records affected by court order, RA 9048, RA 10172, RA 9255, Legitimation

under Article 177 of the Family Code and RA 9858, Supplemental Report.

A. ABOUT THE SERVICE

Any person has the right to request for a copy of his/her registered document. As such, upon

the payment of the required fees and for as long as there is still a copy of the document at the archives

of the MCRO, the same can be issued. Otherwise, a certification to the contrary may be validly

issued. The office observes the confidentiality rule on registered Certificates of Live Birth .

B. WHO CAN AVAIL OF THE SERVICE

Following the legal maxim, "The place of occurrence is the place of registration", A person

born, wed and died within the jurisdiction of La Trinidad, Benguetmust be registered in this office.

Hence, he/she is entitled for the available copy issuance of his/her birth, marriage or death record once

requested.

C. REQUIREMENTS

1. The document owner, or any person authorized by him.

2. If the child is a minor, his parents, guardian, or the institution legally in-charge of him, as the

case may be, shall issue the authorization required.

3. The duly signed valid ID of the one giving the authorization and the grantee shall be presented

presented for verification.

A. ABOUT THE SERVICE

The best evidence to prove the legal existence and filiations of a person is one's

Certificate of Live Birth (COLB). The registration of a person's birth vests in the child his/her

right to a name, nationality, identity and family. In the same manner the registration of the

Certificate of Marriage (COM) is a prima facie document to prove the married status of a

person. The registration of marriage establishes on records the existence of the legal union

between husband and wife. The Certificate of Death (COD) proves, on the other hand, the

cessation of man's earthly existence, and its registration presupposes the official termination

of such existence.

B. WHO CAN AVAIL OF THE SERVICE

Following the legal maxim, "The place of occurrence is the place of registration",

all persons born, married and died within the territorial jurisdiction of La Trinidad, Benguet

should and must be registered before the Civil Registrar's Office of La Trinidad.

C. REQUIREMENTS

1. Four copies of the Certificate of Live / Certificate of Marriage / Certificate of Death duly

accomplished and signed by the proper parties.

2. For COLB of an illegitimate child to be registered under the surname of the father, the

father should appear personally before the MCR with his Cedula and valid I.D. In

addition, the mother of such child must give her consent.

3. Timely registration of birth and deathis within 30 days from its occurrence, while 15 days

for marriage.

A. ABOUT THE SERVICE

The office recognizes the right of the person to register their document though outside their

prescriptive period so as to provide a credible data of facts necessary for governmental usage. A vital event

is considered for late registration if the prescriptive period for its reporting has already lapsed. In cases of

birth: within 30 days from its occurrence; while 15 days is required for marriage; and 30 days for death.

Outside of these prescribed days are considered delayed registration which will require sets of needed

documentary pieces of evidence.

B. WHO CAN AVAIL OF THE SERVICE

Any person during his/her lifetime may seek for the registration of his/her COLB/COM/ or the COD of

his/her deceased relative. Following the legal maxim, "The place of occurrence is the place of

registration", any person born, married and died within the territorial jurisdiction of La Trinidad, Benguet

should be registered before the Civil Registrar's Office of La Trinidad.

C. REQUIREMENTS

1. FOR BIRTH

a. Four copies of the Certificate of Live Birth, accomplished and signed by

the proper parties.

b. Duly accomplished affidavit for delayed registration at the back of the

COLB.

c. NSO Certificate of No Records (Negative Records)

d. ANY TWO of the following documents (whenever applicable) to prove

the BIRTHDAY AND BIRTHPLACE.

Baptismal Certificate;

School Records (Form 137,Transcript Of Records);

Medical Records if born in a hospital or medical clinic;

Voter's Registration Record;

SSS Membership Record;

e. Affidavit of two-disinterested persons attesting to the facts of birth.

f. Marriage certificate or marriage certificate of parents

2. FOR MARRIAGE

a. Four copies of the Certificate of Marriage duly accomplished and signed by the

A. ABOUT THE SERVICE

RA 9048 was signed into law on March 22, 2001. It gives the Local Civil Registrar or the Consul General the authority to correct clerical or typographical error and likewise allows the change of first name or nick name in a civil register without need of judicial order. The law was amended by RA 10172 on August 15, 2012 and added the sex and day or month of birth to be included in those that can be corrected without need of judicial order.
