The Memorandum of Acting Executive Director Sinocruz reads:"On 26 February 201"3, the Commission issued Minute Resolution No" 13-

0298 entitled 'ln the Matter of the Procurement of Voters lnformation Sheet(VlS)', renamed the term 'Voter's lnformation Sheet (VlS)' into Voter's lnformotionond lnstruction Sheet (VI|S), and resolved, among others, to direct the ActingExecutive Director to:

a) Prepare the necessary template/ format of the Voter's lnforntationand lnstructron Sheet (VllS) toking into consideration the following:L. To indicate the following Voter's lnformation:1.L. Name of voter;1.2. Assigned precinct number; and1. 3. Assigne d pol li n g ce nte r.

2. To provide simpliJied instructions to the voter on how to votecorrectly;3. Page2 M R No.13-0298Per review of the previous price ceiling (V/S used in 2010) andconsiclering the urgency in procuring said information sheet as mandated byRepubtic Act No. 7904, it is our humble submlssion to adopt the price ceilirtgof Php3 00 per sheet inclusive of delivery cosf esfab/lshed and authorized bythe Honorable Commission En Banc in Minute Resolution No. 10-0451 , pltts

ten percent (10%) thereof, or for a unit cost of Php3.30 per sheet. The VllS shall be printed atthe regional levetat a cost not to exceed one peso and fifty centavos (P1.50) per VllS,inclusive of delivery cost to the Election Officers (EOs);to direct the EOs to cause the addressing and delivery of said VllSto the voler together with the Voter's lD, if available, at a cost not toexceed one peso (P1.00) per VllS; andthat in the delivery of the Voler's lnformation and lnstruction Sheettogether with the Voter's lD, to require the receiver (voter or personreceiving in his/her behalf)to acknowledge receipt thereof.

Let the Acting Executive Director implement this resolution.
