MEMORANDUM

RE: MENINGITIS CLEARANCE FOR HOUSING STUDENTS

Dear Student,

A requirement for living in student housing at CSULB is to show proof of being vaccinated against meningo-coccal disease. Student Health Services, in conjunction with housing administration, is dedicated to ensuringan efficient and smooth transition for your move into housing. Therefore, we have several options for you toget your vaccination record to us for clearance.

It is highly encouraged, if you are not already vaccinated, to receive your meningococcal vaccine prior to thestart of the Fall 2011 semester. The vaccine is referred to by a few different names, such as ―MCV4‖,―Meningococcal Conjugate‖ and most popular is ―Menactra‖. It helps protect against the most common strains

of the meningitis bacteria (A, C, Y, and W-135) in the United States. If you received the meningococcal vac-cine before the age of 16 years old, you will need to have a booster vaccine.

The Student Health Center does offer the Menactra for $114.00. Beginning August 22, 2011, we will offer thevaccine on Monday and Tuesday mornings, 9:00am-12:00pm, and Friday afternoons, 1:30pm — 3:30pm, dur-ing our immunization clinic. Immunization clinic is by appointment only. If you have health insurance youshould consider seeing your primary care physician prior to the start of school because the vaccine is usuallycovered by insurance. SHS does not accept or bill any insurance.

Here are the different options for you to get your vaccination records to us:

Fax your vaccination record to (562) 985-1644

Drop off a copy of your record at the Student Health Center

Bring a copy of your vaccination record on your move in day and leave it with housing personnel

Bring a copy of your vaccination record to the Parent Orientation Program

** Please write your name, student ID #, and cell phone # on the copy of your records.

Please do not hesitate to call or contact us with any questions at (562) 985-4771. We are happy to assist you inany way.

Sincerely,

Michael Carbuto, DODirector and Chief of Medical Staff

THINGS TO THINK ABOUT

RENTER’S INSURANCE:

Insurance coverage for your personal property and the items that you bring to school are your responsibilityand CSU Long Beach is not responsible or liable for your property. We suggest students purchase renters‘ in-

surance to ensure that all possessions are not lost due to theft, fire or accidental damage. Buying a renters‘ in-surance policy is a small investment that may pay huge dividends in the future.

CSU- Long Beach has a relationship with GradGuard.com, a national insurance company focused on creating―right-fit‖ insurance products for college students. Their renters‘ insurance policy will protect your personal belongings if they‘re lost or stolen and provide liability coverage if there is damage done to your property or

someone is hurt on the premises. These protections are very affordable at around $12 month and offer moreconvenient options for protection vs. a traditional homeowner‘s policy. Please visit www.gradguard.com/renters or call (800) 625-1568 for more information and to receive a quote.

REMINDER All students need to make sure they print and bring their pre-registration passport.

Students will need to sign in at www.housing.csulb.edu prior to move in day.

FIRST PAYMENT DUE DATE:

Don’t forget that Thursday September 1, 2011 is when your Housing

Payment is due. You can pay online by e-check or credit card on the

student’s mycsulb or at the housing office by check or cash.

FRIDGE, MICROWAVE, SAFE:

We have partnered with Collegiate Concepts, Inc (CCI) to exclusively provide residents with theMicroFridge and the SafeConcepts electronic safe for use in the halls. For more information on the products, please visit http://www.collegefridge.com.
