2) Payment must accompany the registration form . Make checks payable to Village of Mount Horeb . Credit cards are accepted at the Recreation Department .
3) Send to or drop off at the Mount Horeb Recreation Department, 222 E . Front St ., Mount Horeb, WI 53572 . A drop box is located outside our front door for your convenience .
aMeRiCans wiTH DisabiliTies aCTWe welcome all individuals and families from our community to use our parks and recreation programs regardless of race, religion, sex, national origin, age, or disability . Those individuals with special needs are encouraged to notify us at least two weeks in advance to help us accommodate your request .
On-siTe RegisTRaTiOnsInstructors will NOT be allowed to accept registrations or payments at class locations . All registration transactions must be completed at the Recreation Department office or online prior to class start date .
ResiDenT/nOnResiDenTA resident is anyone who resides within the Mount Horeb Village limits (we have a listing of addresses in the Village if you are unsure of where you reside .) All others are considered non-residents, and will be charged an additional $10 .00 to the resident rate . This charge is assessed per person per program . This additional non-resident fee helps with staffing costs and equipment expenses for our programs .
weaTHeR CanCelaTiOnsIf you are wondering if a class or program is canceled due to weather, you can call our cancelation line at 437-3400 x .9 .
RegisTRaTiOn DeaDlines/laTe feesA $5 .00 late fee will be added to your payment if postmarked after the registration deadline date listed . Please plan ahead .
RefUnD POliCyA refund will be issued when a program is cancelled by the Rec . Dept . Cancellations made seven working days or more prior to the scheduled start date of an activity will have a $2 .00 surcharge . Refunds or credits will not be issued if you cancel out of an activity less than seven working days from the program start date . A credit (minus a $2 .00 surcharge) will be issued in the case of illness or injury . Credits can be used for any recreation program in the future . Please note that online convenience fees are non-refundable .

No receipt or confirmation will be sent without a self-addressed stamped envelope enclosed in your registration .
eMPlOyMenT infORMaTiOnThe Mount Horeb Recreation Department is taking applications now for winter positions including rink supervisors, youth basketball supervisor, basketball scorekeeper and basketball referees . Persons are asked to contact the Mount Horeb Recreation Department for an application . Applicants must be at least 15 years of age .

Your registration will not be processed and will be returned if no payment is sent . However, if partial payment is sent in, we will send back notice of monies owed, and you will then need to resubmit payment to the Recreation Department . If you have registered for only one program, and that program is FULL, you will then be sent back your payment and confirmation that you have been placed on the waiting list . If you have more than one program you are registering for, and a class is FULL, the money for the FULL class will be put as a credit on your account and you will be sent a confirmation that you have been placed on the waiting list for that program . The other transactions on the registration form will be processed .
nOn-sUffiCienT fUnDs POliCyA $15 .00 service charge will be assessed on all checks returned due to NSF . Once an NSF check occurs, all future registrations must be paid in cash or money order only .
PUbliCiTyThe Mount Horeb Recreation Department reserves the right to utilize photos and names of participants for publicity purposes . Participants not wanting their names and/or photo used must notify the Recreation Department in writing at the time of registration .
PROgRaM infORMaTiOnPrograms may be cancelled if minimum registrations are not met or facility issues occur . You will be notified before the start date if an activity is cancelled .
waiVeR Of liabiliTyThe Village of Mount Horeb is not responsible for any injuries sustained as a result of participation in any department sponsored activities, or while using departmental facilities . The Mount Horeb Recreation Department recommends that anyone enrolled in a program or using facilities make provisions for adequate insurance coverage .
