What our patients say…
Sarah’s story *
Sarah was a 30-year old married
woman with an existing diagnosis of
Bipolar Disorder who was admitted to
the Mother and Baby Unit at 7 days
postnatal. Her son was her first baby; he
was planned and very much wanted.
Sarah remained well antenatally,
however despite a robust plan of care
around delivery and the early postnatal
period Sarah became significantly
unwell 3-4 days postnatally.
Sarah experienced paranoid delusional
ideation, she was suspicious of her
family's behaviour, wouldn't allow her
midwife into her home and experienced
thoughts that her son was going to die.
She believed the water in her home
was contaminated and therefore she
couldn't make bottles to feed her son.
Sarah did not accept admission initially

and was placed on a Section 3 of the
Mental Health Act.
During admission Sarah's original
dose of Quetiapine was increased and
Fluoxetine added, but as she failed to
respond to antidepressants, was given a
course of ECT to which she made a good
response.
Whilst Sarah had a warm bond with
her son, she was unable to provide
consistent and safe care for him. She was
supported by nursery nursing staff until
her care improved.
Sarah was discharged home 8 weeks
after admission and on discharge she
described feeling overwhelmed with
joy that she was returning home with
her baby, that she was well and a good
mum.

Joanne’s story *
Joanne was a 20-year old single mum
who was 37 weeks pregnant with her
first child, which was unplanned, when
she was admitted to the Mother and
Baby Unit following concerns from her
GP and Mental Health Assessment by
the Crisis Team.
She presented with significant and
disabling low mood. Joanne had
reduced functioning, was retarded in
presentation, she expressed suicidal
ideation with some intent. Joanne had
a history of suicidal behaviour. She
had experienced severe depression
historically requiring admission on
two previous occasions and had
taken an overdose of her prescribed
antidepressant, 2 days prior to
admission.
Joanne explained ongoing relationship
difficulties, some financial worries
and inadequate housing. She felt
unable to address these issues feeling
overwhelmed, hopeless and helpless.
Now on the unit Joanne’s obstetric care
was transferred to the local obstetric
team; Joanne was seen regularly by the
Unit Midwife.
One week following admission,
supported by her mum and named

nurse, Joanne went into spontaneous
labour and delivered at the local
maternity department without any
problems.
The following day Joanne returned to
the Unit. She required considerable
support in caring for herself and her
baby but responded well to the changes
in medication and gradually, with
supervision, took responsibility for her
baby’s care.
Joanne required counselling from the
Unit Psychologist to address issues
with self esteem, confidence and
relationships, but on discharge described
a warm and loving relationship with her
daughter
