These are your MyRewardPoints™ Card (“MRP”) terms and conditions. Please read them care¬fully and retain them for your records. By accepting and using the MRP, you agree to be bound by these terms and conditions. In these terms and condi-tions, “you” and “your” means the person who has received the MRP and “we,” “us,” and “our” means FT Rewards, LLC d/b/a Frosch Rewards and Incentives (“FRI”), our successors, affiliates and assigns. These terms and conditions are subject to change without notice and supersede any and all previous terms and conditions. Redemption Instructions: The attached MRP is only redeemable through FRI for FRI re-demption options which may include, but are not limited to, merchant gift cards, prepaid cards, merchandise and gift products (“Re-demption Options”). You can redeem your MRP by following these simple instructions: • Go to http://holiday.incentel.com and view our up-to-date list of Redemption Options. • Follow the instructions provided on our website to make your redemption selec-tion. Your redemption will be processed within two (2) business days. • Most redemption selections are shipped within 72 hours of placing your order and received within ten (10) to fourteen (14) business days. If you select eGift Card

products as your Redemption Option, you will not receive a physical plastic card in the mail. eGift Card products are sent by email and delivered to your email inbox typically within minutes of placing your order. If you do not receive your selection within the time allotted, please contact The @YourService™ Customer Care Center toll-free at 866-388-3959 or awardhq@incentel.com. Commercial Transaction: FRI sells the MRP exclusively to businesses (referred to as “Client(s))” for the purpose of re-warding and /or incenting employees, customers and patrons. The value of each reward/ incentive is shown on the MRP in points and one point equals one U.S. Dollar in purchasing power. FRI does not sell the MRP to individual consumers. You acknowledge that you have received this MRP in connection with a promotional program and that you have not paid FRI value or compensated FRI in any way for the MRP you have received. Nature of the MRP: The MRP is not legal ten-der and cannot be used as a method of pay-ment. The MRP is not a contract, agreement or offer for a contract or agreement. The MRP is not a gift card or a gift certificate. The MRP can only be exchanged for Redemp-tion Options equal to the value of the points shown on the MRP, subject to these terms

and conditions. The points on the MRP can-not be converted to cash or cash equivalents at any time. A MRP is not refundable. Forfeiture: Each MRP is registered on the date it is sold to Client and on that regis-tration date an expiration date is also es-tablished. YOU MUST USE YOUR MRP PRIOR TO THE EXPIRATION DATE SHOWN ON THE MRP OR YOUR POINTS WILL BE FORFEIT¬ED. The ex-piration date cannot be extended. Redemption Options: Redemption Op-tions may include merchant gift cards, prepaid cards (stored value cards), catalog selections, vacations or movie tickets or other such options as are arranged through third-party merchants from time to time. Redemption Options are subject to change without notice, are only available while in supply and may be delivered to you subject to additional, specified terms and condi-tions as required by the third-party mer-chants. You should become familiar with all terms and conditions that apply to each Redemption Option. You understand that all Redemption Options are provided by third-party merchants, FRI disclaims any and all warranties whatsoever including, but not limited to, FITNESS FOR A PARTICU-LAR PURPOSE or WARRANTY OF MER-CHANTBILITY with regard to such goods

and services and that any disputes regarding the goods and services shall be directed to the manufacturer or third-party merchant provider. If a Redemption Option is discon-tinued, reasonable efforts will be made to offer a substitute of equivalent quality. Limitations: FRI is not responsible or liable for a MRP that is lost, damaged or stolen. FRI is under no obligation to re-issue a MRP that has been lost, damaged or stolen. Re-issuance of any MRP will be subject to terms and conditions established by FRI and will occur only upon payment of a MRP replacement fee of $5.00. General: All interest earned on the value of any MRP received from a Client prior to redemption shall be the sole and exclusive property of FRI. These terms and conditions will be governed by the laws of the State of Texas, without regard to conflict of law prin-ciples. Whenever possible, each provision shall be interpreted in such a manner as to be effective and valid under applicable law, but if any provision is held to be invalid, il-legal or unenforceable under any applicable law or rule, the validity, legality and enforce-ability of the other provisions herein will not be affected or impaired thereby. These terms and conditions, or any portion thereof, shall be subject to applicable law and VOID WHERE PROHIBITED BY LAW.
