Mona Dental L ab, L L C, co-owner Ben Kwasney at tended a seminar at L MT?s L ab Day West on May 20. The topic, presented by Preat Corp. president Chr is Bormes, was: Removable Opt ions for the Edentulous Arch. More detailed informat ion can be given in discussion for case planning. The following includes some highlights from the lecture:

By 2020, there will be an est imated 38 million edentulous arches in the U.S. Because implants preserve bone, edentulous pat ients with the abilit y to have implant surgery should make it a pr ior ity.

Before a pat ient has implant surgery, an end goal should be determined so the

Technology Meets Art ist ry was the theme of our presentat ion at L MT Magazine?s L ab Day West on May 20 in Garden Grove.

The event was a chance for us to show off samples of CAD removable part ial denture frameworks to fellow technicians, lab owners, dental technology students and dent ists.

When sending a new part ial case, please inspect the model for distort ions. Make sure rest seats are adequately prepped pr ior to taking a final impression and indicate any specific design inst ruct ions you may have.

With CAD part ials, there is great f lexibili t y in tailor ing a part ial to your specificat ions. Clasp size can easily be adjusted, along with features such as the connectors and mesh pat tern for acrylic retent ion. We use the highest quality alloy by Nobilium in our cast ings.

Zest Dental Solut ion?s L ocator at tachment is the most widely used in the lab today. Authent ic L ocators are the most super ior stud at tachment because of their pivot ing abilit y. There are many copycat at tachments out there, but none achieve the pivot ing abilit y of Zest?s innovat ion. To ensure the best funct ion, purchase genuine L ocators.

The pivot ing accommodates natural movements dur ing occlusion and prevents the at tachment from being locked in a stat ic hold. Non-pivot ing at tachments can easily dislodge the denture.

Chris Bormes, president of Preat Corp., advises using extended-range male inserts when there are three or more at tachments in an arch. He says L ocator impression copings should not be used when there are more than two implants or if the implants are divergent .

The new L ocator R-Tx abutment has hit the market with some mixed reviews. Some pat ients complain the denture feels too loose. One indicat ion for using the R-Tx would be for an arch with two implants that have more than 60 degrees divergency/convergency.

NEW PRODUCT ALERT TIP OF THE MONTH

Do you have a pat ient with a history of full denture fractures? Or a pat ient allergic to acrylic?

In either case, a Karadent full denture is a worthwhile considerat ion. We started using this mater ial recent ly for full dentures, as it was designed for full and part ial restorat ions. The final prostheses have been received well and we are hoping they will catch on as one of our more popular offer ings.

Not only is the mater ial pleasing to the pat ient?s eye, the injectable microcrystalline polymer is more durable than acrylic. While not guaranteed unbreakable like its f lexible cousin (TCS), Karadent will not crack like t radit ional acrylic. The semi-r igid resin has been tested for relines and repairs with acrylic and TCS Inc., which manufactures the product , advises acrylic will bond to Karadent . We have not at tempted relining or repair ing with acrylic. We st i ll

recommend inject ing with the or iginal resin to ensure a st rong bond and maintain a uniform appearance.

Our Karadent full denture is pr iced slight ly higher than all-acrylic opt ions. Please call or e-mail for details.

Sleek alt ernat ive t o acrylic

This full denture was recently made for a client of Mona Dental Lab, using Karadent resin by TCS. Karadent is an alternative to acrylic.

r ight type of appliance (fixed or removable, bar or no bar) can be chosen.

Fixed hybr id opt ions (aka the all-on- four t reatment ) are fine for some pat ients, but there are drawbacks. The three main reasons a removable prosthesis should be prescr ibed are:

- High smile line (need flange) - L ip support- Dexter ity (can easily remove)

Once you have decided to go the removable route, the next quest ion is bar or no bar?

I f the pat ient has a flat at rophied r idge, the choice is simple: bar.

For pat ients with healthy bone,

the choice can be made by looking at the advantages and disadvantages. The graphic to the left by Preat Corp. can give you some of the factors to consider. After making a bar decision, the at tachment type will be considered.

With several at tachment opt ions and combinat ions available, we can help you choose on a case-by-case basis.

Some common at tachment systems include:

- L ocator (most popular)- Hader clips - Preci-Sagix (aka Bredent

Var io-Stud) - Preci-Clix - Ceka- Universal plunger - O-r ings - Magnets

Buccal escape vent s bet t er t han l ingual

Those of you opt ing to process at tachments chairside were probably told to cut holes in the lingual side of the denture when picking up the at tachment with acrylic.

I f you are not already doing so, t ry cut t ing buccal escape vents. I t is a simple fix that will not leave the pat ient feeling the patched up holes with their tongue. I f f i lled proper ly, they will not be visible when wearing the denture.

Lingual escape vents, l ike the ones pictured in this lower denture, may cause the patient to feel the repaired surface with their tongue. Try making buccal escape vents for your next chairside attachment pickup.
