My term as acting President of MNDAKSPAN will soon be coming to a close. On April 30th, 2016 I will join the ranks of the many past presidents and assume an advisory role in MNDAKSPAN. Deb Moengen BSN,RN,CPAN will assume leadership on May 1st, 2016 continuing on in supporting MNDAKSPAN’s mission: “to promote the education and professional growth of nurses from North Dakota, South Dakota, and Minnesota practicing in perianesthesia areas such as ambulatory surgery, preanesthesia and postanesthesia care.” (MNDAKSPAN, n.d.). I want to reflect on the past to bring us to the now. My first message was “My Journey to Presidency, perhaps it could be yours” (Novack, 2014b). Again, I would challenge you to get out of your comfort zone and become involved. MNDAKSPAN offers component members the chance to develop those hidden leadership skills as a board member through the Leadership Development Institute (LDI). Board members are sent to the LDI conference, presented by ASPAN, to assist in the training and education of component leaders. I highlighted this education in the Fall 2014 Par Excellence. (Novack, 2014a). Within the Affordable Care Act (ACA), lies the Health Information Technology for Economic and Clinical Health Act (HITECH) pushing organizations to develop health information systems (HIS) to meet meaningful use (Arlotto, 2010). MNDAKSPAN strives to meet the needs of our component members by generating an increased presence in social media and use of technology in its “going green”. A facebook site supports membership questions, announcements, and social support while constant contact and email generate additional reminders and general information coupled with Twitter and LinkedIn.

Cindy Swenson, Treasurer, initiated a MNDAKSPAN driven paypal for conference registration provided through the website. Voting has been changed to electronic ballots and conference material is conveniently located on the website. Bring your own device (BYOD) is yet another new acronym requiring new policy (“Bring your own device,” 2016).

Robert Davidson, Director of Publication through his diligent work with the webmaster created a more aesthetic and navigable website along with creating his own new look to the MNDAKSPAN newsletter “Par Excellence”. Suggestions from component members addressing website concerns continues to assist in the iterative process of technology. Keep your voice active! Along with its new look, Robert has increased membership presence in the newsletter through member research articles, conference comments, and guest editorials.

MNDAKSPAN has attained the “Shining Star” award through the diligence of Ginny Maruish, Director of Certification, and component members desire to certify. It requires a large amount of paperwork and an active component to attain and maintain such an award. She has been a role model and cheerleader to certification for several years and it is demonstrated through her passion to nursing. Thank her for the wonderful cupcakes that conference attendees have enjoyed in the recognition of certified nurses. Don’t forget the membership benefits of reimbursement for CPAN/CAPA both in the initial cost or renewal.

Our education has been bolstered by the work of Julie Oppold, Director of Education. She has offered to be a willing mentor for anyone wishing to assume her position at the end of her term. Email Julie if this is something that would interest you. MNDAKSPAN now works to secure sites in advance for announcement at the preceding conference. Save the Dates are prepared and sent well in advance, brochure timing remains reliant on ASPAN to attain approval and disperse. Vendor sponsorship continues to grow as is our Friday education day. We have received contact from the Minnesota Society of Anesthesiologists (MSA) who wish to develop an educational relationship with our component. Continue to watch for exciting events in the conferences to come.

I have touched on the development of the visions created in my first newsletter to the component. Above lists the actions that have been attained through the diligent volunteerism of our Board of Directors and the support of the MNDAKSPAN component members. Continue to watch for your opportunities to volunteer, perhaps as a member of a conference planning committee, a board position, maintaining membership and attendance at MNDAKSPAN conferences or join us for open board meetings held in the fall. Whatever your passion, make your voice heard, you will always get out of it more than you put in, you only have to be willing….

‘The Road to Perianesthesia Success, MNDAKSPAN Winter Conference” was held on Feb 26 and 27, 2016 at the Ramada Bloomington Minneapolis Airport/Mall Hotel. This was the second year that we offered a 2 day event. The conference was hosted by a group of volunteers led by Mary Briggs. Team members were Jackie Dols, Andrea Collins, Meredith Joyce and Jill Thurston. Thank you to all of them.

Writing a summary of this conference is difficult. How do I put into words the value of such an event? I observed the interaction of old friends, attendees making new friends, people sharing encouragement and wisdom, laughter and a few tears. I clearly felt powerful inspiration to be a better nurse and to believe in the valuable work we are doing. My words don’t do justice to the events of these 2 days, but I will try to give you a glimpse of each day.

Friday was designed to review perianesthesia skills. Nurses new to the specialty of perianesthesia, or seeking review, or

considering certification all enjoyed the morning. Attendance at this portion of the conference was 109.

Our first speaker was Pam Taulman, RN,BSN,MA,CPAN with “Pharmacology in Perianesthesia” Pam has worked in perianesthesia a number of years. The last 7 of those as a perianesthesia clinical learning specialist at University of Minnesota. She is a go-to resource for clinical practice questions, including pain medication management. She gave an overview of common medications used in perianesthesia. Pam brought a new learning technique to the conference. Attendees who had a smart phone, joined the question and answer review utilizing www.socrative.com We all enjoyed the technical test of knowledge.

Kim Kislia RN,BSN,MA,CCRN-K,CPAN presented the next session “ Perianesthesia Review” Kim works as a Clinical Development Specialist for University of Minnesota. She opened with the statement from ASPAN, “ PHASE I: PACU is the most important room in the hospital because it poses the greatest potential dangers to the patient so that there should be no relaxing of vigilance and there should be constant and total care provided by the nurse”. Kim encouraged us to “turn the light on, talk to your patient, use your brain. The monitors don’t have a brain, you do!”

Our Friday session concluded with Mary Briggs RN,BAN, CAPA presenting more “Perianesthesia Review” She is also working in education for the University of Minnesota. Mary began with a review of pediatric issues. “Constant vigilance is required in your work with pediatric airways.” She shared a favorite education website, http://pted.org/ from John Hopkins. It publishes education for pediatric cardiac issues. Mary offered a variety of helpful tips to work with children. “Always evaluate your H’s and T’s before a crisis.” One of her favorite quotes is from T. Berry Brazelton, MD, touchpoints: the essential reference, “While hospitalization (surgical intervention) is frightening both to children and to their parents, it can also be a positive experience. With a nurturing atmosphere…a child can grow in self-esteem and maturity by learning to conquer the fears and anxiety of a stay in the hospital. Mary also shared some insight in working with geriatric patients. “Preop assessment is critical. Concentrate on the individual risks. Encourage patients to take the patient education seriously, will be vital to the success of their surgery.” Mary concluded with certification and test taking tips.

Following the conference, we were invited to attend a vendor fair. Deb Moengen, the upcoming MNDAKSPAN president organized the event. Several vendors were present. Attendees were invited to share some social time and refreshments. Everyone in attendance enjoyed the event.

Saturday morning began with the amazing breakfast that the Ramada is known for. 219 were in attendance on Saturday. Our service project for this conference was “The Lift Garage” it is a nonprofit agency aimed to provide a vehicle to move people out of poverty and homelessness by offering low-cost car repair, free pre-purchase car inspections, and honest advice in order to support the Minneapolis community on the road to more secure lives. Donations make it possible for them to repair cars at only 33% of market rate. Their goal is getting cars safe and back on the road. We received over $400 in donations for their organization. Thank you to all who donated!

Ruth Novack, the outgoing president took a few moments for introductions and announcements. She thanked the outgoing board members and welcomed the newly elected. Thank you to the outgoing board members for your dedication to the profession of nursing and MNDAKSPAN.

Our first speaker was Chris Peltier, DNP,RN-BC,FNP-BC from the University of Minnesota pain team. She presented, “Regional Anesthesia for Post Op Pain Management.” Chris stated, “ we need to target different mechanisms to achieve pain control” She reminded us that nothing works for everyone. Nothing “always” works. She stressed the recent statement made in the Feb. 2016 issue of the Journal of Pain. Recommendation #23 is a practice guideline “that practitioners should consider a peripheral regional block.” She presented valuable information to care for the patients who are receiving blocks.

Our next speaker was Dr. Jacob Hutchins, MDA, also from the University of Minnesota. He presented “Multimodal Analgesia.” He gave startling information about the crisis our health care system is experiencing due to opioid use, both prescribed and illegal. “We have to change the way we practice. People in the U.S. expect opioids to treat pain after surgery.” He stressed using multimodal treatment of pain. “Hospitals

say they are doing it, but how many really are?” “Team members need to work together to improve this situation.” He is clearly passionate about his work and is very intent to share that passion with others.

Molly Hagen, DNP,APRN,CPNP presented our next lecture, “Pediatric Pain Management” She works at U of M Masonic Children’s Hospital with the perioperative and interventional pain service. She expressed, “I love working with pediatric patients.” She shared her passion to see improvement in pediatric pain control measures. Molly stated there is a lack of research regarding pediatrics versus adult populations, so we have to look for the evidence. She reminded us that “imagination” if a powerful influence in our care of pediatric patients. It can work for us or against us. “So be aware of the power of imagination” She also mentioned various apps for your tablet that assist in the care of children.

Deb Drew, ACNS-BC, RN-BC, AP-PMN recently retired after 31 years as a clinical nurse specialist at U of M. She was responsible for the standard of care for patients with pain. In December of 2013 she introduced a new pain assessment tool to replace the numeric scale with the goal of improving pain assessment and patient satisfaction with pain management. At the national ASPAN conference in 2015, Chris Pasero referenced her work. Deb gave a summary of why she implemented a new pain assessment tool and how she implemented it in their facility. The CAPA pain scale is dependent on conversation with the patient. One patient comment stands out, “Made me feel like the nurses care more about my pain.” She shared some of the fears and concerns of the new tool. ”Fear of making an “assessment”: some nurses are more comfortable with patient’s statement of a number than trying to interpret interaction. MDs fear that they won’t know how to respond when nurse calls with CAPA information.” She closed with this quote from Edmund Burke, “Nobody makes a greater mistake than he who did nothing because he could do only a little.”

After lunch, Dr. Eitan Podgaetz MD,MPH presented “Enhanced Recovery After Surgery.” He is a thoracic surgeon at the University of Minnesota. He is a pioneer for thoracic surgery Enhanced Recovery After Surgery Pathways. He began with “surgery” is a “team sport” not an individual sport. “It doesn’t start with a patient wearing a gown in preop, it starts in the clinic.” Perioperative Surgery Home protocols, also known as ‘fast-track surgery’ or ‘multimodal optimization’, are a combination of evidence based perioperative strategies which work synergistically to expedite recovery after surgery”. His goal is

to provide consistent, efficient, quality care through multidisciplinary collaboration. He said he is not afraid to challenge old practice, “Just because you have done it that way for 30 years, doesn’t make it right!”

The next presentation was led by Deb Massey RN,BSN,MA,CPAN,HNB-BC and Cathy Standish RN,BSN,MA,CPAN,CAPA. Deb is working at the VA Medical Center in Minneapolis in outpatient surgery. She also works at Mayo Clinic Rochester Methodist in PACU. In her free time she serves in the Minnesota Air National Guard. Cathy works at Fairview U of M in pre and post op. She is level I and II Healing Touch and Reiki level I and II. Alternative medicine in the perianesthesia setting is one of the most requested topics we see on the post conference evaluations. They began their presentation with an introduction to a few essential oils they had placed on each table. They also had a table with various information, available for attendees to visit throughout the day. Deb and Cathy helped to define current therapies in integrative care. Holistic health is the care of the whole person. They presented the Department of Defense model of holistic care which summarized treatment. We all gained a better understanding of alternative care measures and the importance of recognizing the individual needs of our patients.

Erin Murphy RN, BSN, MA, FAAN presented, “RN and Legislator: Serving the Public”. She has served in the Minnesota legislature since 2006. After working as an OR nurse, she worked for the Minnesota Nurses Association as a lobbyist and organizer. She shared that she always heard her hard working, middle class family members discuss politics. When her mother experienced health care hardships, Erin felt called to serve. “As nurses you need to use your voice, the public puts their trust in you!” Her campaign was built on “listen, diagnosis and act” just like nursing. She challenged everyone to speak up and let their representatives know about the issues in health care. Erin expressed sincere appreciation to all the nurses in the room and encouraged them to feel pride in what they do for others on a daily basis.

Mark Tanning, SPHR,CEBS,CLTMC closed our conference with “Personal Growth in Stressful Work Environments” He recently retired from the position of senior leader in human resources at U of M physicians. He previously worked in corporate America at several large companies. He returned to health care administration and has taught many versions of this material to health care professionals. He has been actively involved in community service projects including building a hospital and girl’s orphanage in rural Honduras and co-authoring anti-racism curriculum used in the St. Paul School district. Mark definitely inspired us. “Research proves that negative thoughts are harmful, our brain needs positive thoughts to be healthy” “80% of adult waking hours are spent at work and only 49% of people have positive thoughts about their workplace” He challenged us to use mindfulness to reframe our thoughts. “Choose to be powerful, don’t point at others to change, Change yourself!” “When you look in

the mirror in the morning, you face the greatest obstacle of the day. Self-talk positive, not negative.”

The deadline for this newsletter did not allow time to review all of the post conference evaluations. I will take the time to review them and listen to what you are saying. We plan future topics based on your requests. We try to present the most requested topics. The next conference is in Bemidji on October 1st. The planning chair, Sarah Bowles is already in the process of conference planning. So make sure to mark your calendars for that day. This will be my final newsletter article after serving as Director of Certification with MNDAKSPAN for six years. It has been a privilege to have met and talked with so many of you throughout the years. It continues to amaze me how similar the

challenges are that we all face throughout the component and how much one can learn from those you meet at conferences.

There have been many changes with regards to the certification process during my tenure. One of the biggest changes I have seen in the last few years is the advancement of technology enabling CPAN/CAPA testing to become more computerized. This has allowed applicants the option of testing twice a year over the course of several weeks and the option of taking the test in their own area! WOW!!! I am proud to say that I have continually seen the number of certified nurses grow in our own component and the number of dually certified has doubled.

Many employers embrace and support certification. Numerous institutions offer professional nurse advancement projects, some of which are only available to certified nurses. The financial reward for doing these projects is often significant.

Background Information: Documentation around pain and sedation is an important aspect of care in the PACU and Phase II. In nursing we know that if “it’s not documented it’s not done”. The implementation of Electronic Health Record Systems (EHR) has helped in some areas such as date/time stamp, legibility, but inconsistencies remain in pain documentation (Samuels and Kritter, 2011). A focus on pain management in the perianesthesia area was undertaken as a project for a hospital professional nurse advancement program to improve documentation, update staff around current policy, and examine patient satisfaction. Objectives of Project: To improve documentation of pain assessment, reassessment and the pain goal consistent with an updated policy. Educate staff and patients/families around the pain goal and increase number of patients meeting the pain goal at discharge to the floor. Improve understanding and documentation of sedation side effects of analgesia. Process of Implementation: Data was collected via audits prior to implementation and throughout the project. Staff were updated monthly on unit’s success and opportunities for improvement. Education and updated policy information was conducted through email, staff meetings, and nurse reviews. Unit

involvement was also supported by management with unit goals focused around the Press Ganey question “degree pain was controlled” in the perioperative setting and staff performed self-audits for their yearly nurse reviews. Statement of Successful Practice: Improvements in areas of documentation and meeting the pain goal were noted after implementation of interventions, but varied throughout the project. An increase in Press Ganey scores were also noted but varied. It is difficult to determine if improved document relates to an increase in patient satisfaction and may likely be a result of the overall unit focus and project on pain management.

It is quite an honor to be asked by one of your graduate students to write an editorial for her professional organization’s newsletter. I eagerly accepted the invitation. My plan was to provide a scholarly treatise about the impact of informatics as a nursing specialty. Google scholar, WorldCat, and CINAHL quickly became my best friends as I scoured the literature to find the perfect works chronicling the history of nursing informatics. I used key words such as computers, health information technology, nursing informatics, clinical informatics, data, technology, and clinical decision support to uncover the preeminent definition of nursing informatics.

I discovered that the American Nurses Association defines Nursing Informatics(NI) as: “The specialty that integrates nursing science with multiple information management and analytical sciences to identify, define, manage, and communicate data, information, knowledge, and wisdom in nursing practice. NI supports nurses, consumers, patients, the interprofessional healthcare team, and other stakeholders in their decision-making in all roles and settings to achieve desired outcomes. This support is accomplished through the use of information structures, information processes, and information technology” (ANA, 2015).

In addition, the American Medical Informatics Association(AMIA) crafted its own description and list the following as core areas of work:

• Concept representation and standards to support evidence-based practice, research, and education • Data and communication standards to build an interoperable national data infrastructure • Research methodologies to disseminate new knowledge into practice • Information presentation and retrieval approaches to support safe patient centered care • Information and communication technologies to address inter-professional work flow needs across all

care venues • Vision and management for the development, design, and implementation of communication and

information technology • Definition of healthcare policy to advance the public’s health (AMIA, 2015)

The Health Information Management Systems Society (HIMSS, 2015) even created an infographic based upon its 2015 Impact of the Informatics Nurse Survey.

As both a nursing informatics faculty member and a Chief Information Officer I was pleased and proud to find the literature so replete with definitions and the world so full of organizations devoted to the specialty. There was however, something missing. I remained unsettled. Why was I not able to close this editorial with a pithy and erudite conclusion?

During my reflection, I had a moment of clarity. In spite of, or maybe because of the descriptions and associations, I wondered why is nursing informatics actually a specialty. Why is informatics, unlike nursing theory, pharmacology, pathophysiology, human anatomy and physiology, and care planning, a foundation of basic nursing practice? Is it not critical for all nurses, regardless of practice level, to understand how to collect data, synthesize information, communicate knowledge, and share wisdom about patients and populations?

Now I am eager to explore ways to incorporate the art and science of informatics into the building blocks of the art and science of nursing. Thank you Ruth for the inspiration!

NEWS FROM ASPAN: REGION 3

It’s been a rather quiet time frame for ASPAN, though on individual fronts, we’ve been accomplishing so much! Currently, we are really gearing up for National Conference in Philadelphia. If you have not yet registered: I’d strongly recommend that you take advantage of the fact that the “Early bird” registration deadline has been extended. If you’ve not had the opportunity to travel to the “City of Brotherly Love,” I’d strongly encourage you to take advantage of this splendid opportunity! The committee has put together a very diverse program of educational activities.

I have been very busy (more on a personal level) with ASPAN activities these last couple of months. Celebrate Successful Practices welcomed over 120 abstracts for review of posters. I believe that we have chosen wisely in selecting the ten topics that will be presenting oral presentations during this upcoming National Conference. I truly enjoy this activity because it allows me to see what so many of our Perianesthesia colleagues are accomplishing that improves practices.

The Regional Directors are planning on a fun presence during Component Night. Come for the fun and stay for the entertainment: can you figure out which historical character each of us represents?

Soon, you will have the opportunity to purchase a new Certification Review Book. Many of your Perianesthesia colleagues have had some input into this upcoming resource. It will be of great benefit if you accept the challenge to study for and sit for the certification exam. I believe that it will also be a great resource for everyday practice as well! Keep your eye on this reference and see how many people you know had a part in this publication!

I hope that if you do attend the National Conference, that you’ll introduce yourself to me. This past year has been one of the most fun of my long, ILSPAN/ASPAN career: being able to get out and meet my peri-anesthesia brothers and sisters.

You all make a difference each and every day in the lives of the patients for whom you care; you have also made a difference each and every day in my life! Thank you!

Minnesota Society of Anesthesiologists (MSA) partnership:

MSA has contacted MNDAKSPAN and has proposed an educational partnership. They have approved a $1000.00 donation to MNDAKSPAN education and are at our disposal to assist with speakers as needed with MNDAKSPAN. MNDAKSPAN retains the right to the selection of speakers and topics they will assist as needed. MNDAKSPAN recognizes this as a wonderful opportunity of collaboration in promoting our mission. Thank you and acceptance of this collaborative agreement will be sent out.

Treasurer Report:

Cindy reported that for 2014 we operated at a small loss for the year. In 2015 we operated at a slightly higher overall loss for the year due to the increased costs associated with the Website redesign and other expenses. For 2016 our net expenses to net expected revenue will again be an overall loss based on our budget. The Board held a lively discussion about how to either increase revenue, decrease expenses, or both. The entire Board agreed that the Summer Board Meeting should be about Education,Conferences, Scholarships, and expenses associated with these topics.

Director of Governmental Affairs: No report, the new Director of Governmental Affairs is Traci Gujer.

Director of Publication: New website design is complete, no additional information.

Director of Education: We need to choose a site for the Spring 2017 conference. The expenses from our current venue are very high (Ramanda in Bloomingon). Group discussion about changing location but staying in the cities. Food is our biggest expense. Deb Moengen is going to investigate other location in the cities that may give us a better rate on room rentals and food. Julie reported that the Fall of 2017 site has still not been choose. There is a group of nurses in Rochester that is willing to be on the planning committee for the conference.

Other conference discussions: Cindy reported that “PayPal” charges us $5.00 per transaction and recommends our refund policy include deducting this amount from refunds if paid via paypal.

Motion Motion made by Deb Moengen to change our cancellation fee to be a 20% administration fee if you cancel 1week prior to the conference and no refund if you cancel less than 1 week prior to the conference. Cindy Swenson 2nd the motion, Motion passed There was a discussion about the $5.00 Friday. This has been well received and members have had good comments. With the cost of PayPal and Coffee, we are losing about $5.00 per attendee. Issue tabled until the summer Board Meeting, no motion or vote.

Contributions and Feedback to PAR Excellence

The key aspect of better representing MNDAKSPAN activities throughout the year is through member submissions to the newsletter. Even if it is just a short paragraph, an entire article, or a single photo, we warmly welcome all contributed submissions to the newsletter.

Ideas to consider:

• submitting an article or editorial • sharing photos from conferences or other member activities • debuting an excerpt from your research or evidence based practice project • sharing a successful program, strategy, or initiative

Please feel free to contact the Director of Publication, at [email protected], with submissions, questions, comments or

suggestions. Submission deadlines are the month preceding the earliest publication date.

At times, PAR Excellence will feature themes and submissions may be held for publication in those issues. MNDAKSPAN does not guarantee that articles submitted will be used in the publication. Authors will be contacted regarding whether the article will or will not be used. Thank you!
