Overview of Governance Committee Terms of Service & Relevant PPS Bylaws
Members commit to 12 month terms, incl. Co-Leads.
Members can serve up to 3 terms (we’re entering Term 3).
Members who miss 3 or more consecutive meetings at any point during their term
of service (without prior notice) are automatically rotated off.
Members who wish to not renew for an additional term of service are welcome to
do so.

Seeking Endorsement for Measurement Year 3 Performance (July 1, 2016 – June 30, 2017)
P4P Performance Payment Model Overview
Partners are eligible to receive P4P performance payments if at least 1 member is
attributed to the partner for a given metric:
– The payment for a given metric is divided proportionally to the attribution;
– E.g. One patient attributed out of 100 total patients = 1% of the attribution;
– E.g. 1% attribution on a metric worth $100,000 = $1,000 performance payment.
PPS overall must achieve annual metric performance target in order for dollars to
be available for funds flow.
Payments will be made to NYP PPS partner organizations once NYS has
communicated our MY3 final performance (~ April 2018).

Key Elements of the CBO Innovation Fund
Focus areas
Improve access to and engagement in primary or ambulatory care for medically
underserved populations;
Improve access to and engagement in community-based, licensed behavioral
health services; or
Improve education around the appropriate use of emergency departments.
Target service locations:
– Northern and lower Manhattan, the Bronx and Queens).
Who is eligible?
– CBOs: Agency/organization should derive the majority of its revenue from non-
Medicaid billing activities.
