POLICIES ! Meeting rooms are available Monday through Friday 9:00 am to 4:45 pm for a minimum of 60 minutes and additional time is billed in 15 minute increments. ! Scheduling is on a first come, first served basis with 24 hours advance notice. ! Payment on-line is immediately available to secure the reservation via PayPal. If you select to pay by a valid credit card (Visa, MasterCard, American Express) receipt of this form along with payment is required to reserve the reservation and a pre-authorization for the entire amount of the time scheduled will be processed. ! Only PayPal, credit cards and/or cash are accepted for payment of services. ! Estimate does not guarantee the final charge. RATES/CAPACITY ! Rooms are billed at the following hourly rates: $30 for Huddle Space and $75 for Board Room. ! Minimum reservation is sixty minutes. Billing thereafter is in 15-minute increments. ! Reservations outside of our normal business hours are available at a rate of $75/hour, with a minimum of three hours. Users assume full responsibility for the security of the suite during after hours along with damage or theft incurred during usage. User agrees to pay to restore property to original condition. ! Board Room/Meeting Space - Max. Capacity: 10 - 25 people – various configurations available. Additional set-up charge if not used in Board room configuration of $50 per use. ! Huddle Space - Max. Capacity: 6 people around conference table. ADDITIONAL SERVICES ! Current price sheets for additional services are provided with this form for your records. ! Beverage and Catering Service may be requested 48 hours in advance. (Charge will be calculated at cost and delivery fee + 20%) ! Equipment other than access to wireless internet, the TV, telephone or whiteboard must be requested 48 hours in advance to reserve. Equipment charges vary. Please inquire. CANCELLATIONS ! <1 business day (24 hours) = 100% of charge + all catering/equipment charges. ! 1-2 Business Days = 50% of charge + all catering/equipment charges. ! >2 Business Days = No Cancellation Charge. !
