Active Duty

Upon receipt of orders, drop off a copy to the Family Practice Clinic Front Desk

You will need to place your final-out date at the top

Your record will be sent to the MPF 1 week prior to your Final Out

EXCEPTION: PRP must hand carry records.

You will be cleared from vMPF once orders are received and your record is forwarded to the MPF

Family Members IAW AFI 41-210, Patient Administration Functions, original

outpatient medical records will be mailed to the gaining MTF. Which means, family members WILL NOT be allowed to hand-carry their medical record to the gaining base.

Records will be sent to gaining base once we receive an indorsed DD Form 877, Request for Medical/Dental Dental Records or Information from your gaining base.

DD Forms 877 can be picked up at the Family Practice clinic Front Desk. This is to verify that you have arrived at the location listed on your sponsor’s orders.

Family Members May request a copy of your medical record.

Request forms may be completed at the Family Practice Clinic front desk, with a 30 day notice.

Once copies have been made, you will be notified for pick up.

If 30 days notice is not given, you will be asked to provide a mailing address to have the copies forwarded to upon completion.

Records are sent through certified mail so they will be tracked.
