REMARKS. Th.e average numbers of warders and prisoners in the gaol durin~ 1871 were considerably greater than during the previous

year; thiS re~ulted from the rem ova.! of the female prisoners from Pentridge Stockade to the gaol, in accordance with the recommendatiOn of the ltoyal Comtuis,ion on Penal Establishments. 'l'he number for 1870 averaged 375, whilst in 187l they averaged 483. ~l'he amount of sickness which occurred in 1871 was also greatei' thnn during the previous year; but. the per­centage on the mcrea~erl average numbers in the gaol, though an increase on the previous year, did not greatly exceerl it, whilst the percentage mortaht,y was a fraction less. 'l'he amonnt of sickness which occurred was no indication of the unhealthiness of

• the establishment,. for the greater number of the individuals who suffered from illness were ndmiHed whilst so suffering; the actu.nl number of persons who were taken ill during their sojourn in the gaol was 'comparatively trifling. ·

CLASS I.-ZYMOTIC DrsEASllS. Order I.-Miasmatic Diseases. Under this head t.he affections ofthe.throaCwere somewli:tt ntimerOlis;bufof'short duration, the disease yielding readily'

to prompt treatment. There were a con•iderable number of cases of inflnenz•, occurring principally among the female prisoners . who were a.t work in ~he sewing-rnom, the result-of good feeding and little exercise; the disease was very mild, and easily cured by approprmte treatment. 'l'he Zymotic diseases of .the alimentary cnnnl, diarrhom an"d dysentery, were numerous, but very short in duration; the cases of dysentery averaging only five: days in t.heir ewe: and tho'e of dinrrh<fla only four days. 'l'he cases of.rheumatJsrn were numerous, but not j:(enerally.import,nnt; they were all chronic cases, the acute case of rheumatic fever·

· 'o·r(ter'2.-Enthetic Diseases. : The cases of venereal disease were not numerouF, and were in most cases .very tractable to treatment; the mode of

Order 3.-Dielic Diseases. .. · 'The princl'pal disease of this order is delirium tremens, the numbers of which :idmitted to t.he gaol n.re always considerable and generally 'eve re, care being taken to separate all cases nctunlly delirions from those who are suffering from alcoholism, though .these latter often border yery closely on the gmver disease. · 'J'he- mortnlit.y from delirium tremens in t.he Melbourne Gaol is wonderfully small when compare<! with that of othor·countries, or oven with the Melbourne Hospital: This is owing; in A"reat measure, to the absence of all stimulants in the treatment, anrl to the free nse of c·o]d a!fusion over the head. 'l'his latter remedy, which 'is usually con<lemned by iA'nornnt people ·as one 'of g'reat severity,: is really, in most insl.anees, jusl,"tbe· reverse. 'l'hese cases we're in former returns placed under tho heading or Dyspepsia, C!tiss Ill., Order jl.; but it is d_ee:med b~tter to place them· along~ide those of delirium tremens, ~!i which ·they· bear a closer' hise.mblance, the causes. of both dtseases bemg the·sarne. · , .: .: , • · , . . , .

1 • Order 4.-Parasitic Diseases. . - The cases of scabies are tbe most-promine~t ones under this heading; theY. were considerab.Jy less rimherous.than during

the preceding year, and yielded very readily to treatment, their cure only averaging three days. . ' ·: • ; ~ - ! ' . . '. '.. .. ' .

'CLASS III.-I,ooAL DrsEiims. Order 1.-Disea.•es oi tke ·Ne'l'!!ous System. , . .In this class the cases were numerous. Two fat~! cases of (iisease of t.~e .b.rain occurred. One, in a ·man 40. ·years of

age, was a remarkable one.· Whilst under treatment· for anot;ber disease, in tlie hospital, he was suddenly seized with epileptic fits and symptoms of brain disease, and died eight days afterwards; the posl inortem. showed inflammatilln of. the brain. ·rrhe other <;ase was that of a young man, 21 years old, who had been under treatment for seveml diseases durin!'( the six months he was in gaol, and, immediately previous to his fa tar attack, was convalescent' from 'diarrhcea. He. was suddenly seized with stertor,ous breathin~ in,the evening, and died the next morning; the posl morlen~showirig great congestion of brain, lungs, kidneys, and intestinal canal. n'he cases of insanity were numerous. 'l'he benches of magistrates in and arounrl Melbourne. are in the habit of remanding ·persons suffering from mental aberration to the g~tol for medical enqi1iry for a week, in order to ascertain whether tbe:v are really insane or not; at the end of this' period the insanity'is either found to be t·emporary or permanent. 'l'he numbers in Lhe"H.eturn show that abrmt half of those thus sent got well in the .time SQecified; and are discharj:(ed; t.he ot.hers are sent to the asylum. Of thci two fatal cases which occtirred, one' \vas n girl,·nged 26;wh0 .showed symptoms of rliseuse rif the brain; the other was an old man, over 70 yea.r~ of age, who died of senile deinentia. A remarkable c11se of brain rli,ease.occurred, whicu terminated latully after a very short illness. A womari, 24 )'ears of n~e, was admit,t'ed to l10spit.al al. 3 p.m. on tile 29th' July, with sym!Jtoms of gast.ric derangement nnd headache; she died .at 6.30 fl.IU.; a post morlem,ex:aminati(ln show,ed her death. was caused by. a turuor in the substance of the brain about the size of a small egg. 'l'here were a considerable number of cases of epilepsy'a.nd neuralgia in exce.ss of the previous year; they were, however, regarding hotu diseases, very amenable to tre~tment;

Order 3.-:-Diseases of the Respiratory Organs.· ~

. . Order 4.-Diseases of -the Dige.•fi.ve Organs. · The cases of dysp~psia were most numerous under this h·~arlin'g, but of fittle importance 'l:ieyond their !lumbers. Three

cases of hepatitiS occurred, one of which, complicated with previous hmg disease; was fatal after four days'·illness; tile. liver was found ,in a very diseased condition, the result of reo~nt and previous infifmmation. • ,

i ; · · Order i.-Disease! of !the Locomotive System, - . .. . · ' :There was ;a gre~t increase in the numbers of acute rhe1umatism of the joints' (arthritis) this year; they were, hOwever, very amenable .to treatment. Tbe use of blisters, in nearly every case, proved very .valuable in reducing at once the pain and swelli*g attendant on this pistressing malady. · ' · 1 • ,.,

some Of the eases of ulcers 'of the lower extremities were very severe. Many of the vagrants who suffer from these diseases t.;tke no care oNhem ·when' they: are out of gaol, and the consequence is, when they are imprisoned, the whole period of their:sentence is taken up in curing them. · · ·. ' ·

CLASS IV.-DEVELOPMENTA.L DISE.I.!ms. Order 3.-Diseases of Old ·people. One case of debility, in a woman above 80 years of age, who bad been i u the gaol hospital for years, but who was .a ·proper

object for the benevolent asylum, terminated fatally, without any appa~ent disease. · " . ·. :

CLASS V. Order 1.-:A.ccidents or Negligence. The cases of wounds under this heading were numerous, but in most oases very slight i some 9f them h,aving been

There were three oases of dementia, two of w"hich were transferred to Yarr:l. Bencl; the third, though under warrant for that place, was not abte to bear removal wben sent for, and he died in hospital here. A Chinaman unsuccessfully attempted .suicide (in real earnest) by hanging himself. An English sailor unsuccessfully attempted suicide by opening his veins with pieces of a gtass bottle and by other means; he, however, was not in earnest. . . · .

An old prisoner (aged 50) suffering from gen·eral paresis depending on softening of brain, and reduced to a skeleton, clambered down off the hospital balcony; and was found lying on hard ground after a fall of twenty feet; and, strange to say, was but little injured by the fall, though from marks on his sca.lp it was evident the head had oome heavity on the ground. This man meant to kill himself. •

. There were eight deaths during the year; one of these was from phthisis, one from·,sohirrus of the rectum, one from double bronchitis, four from diseased brain, and one from epilepsy. The origin of the disease in one of the cases of diseased brain was, the centre of the os frontis being driven in many years ago by a marlinespike. Two sword-cuts, many years since, on the middle of the teft parieta.l, originated the disease, which ultimately proved fatal in a.nother of the cases of diseased brain.

The fatal epilepsy case deserves comment. The man was 53 years of age. The disease commenced a year before his death. In each fit he would be for a short time insensible, and the convulsive movements would be confined to the muscles of his face and throat; the fits were neither frequent nor severe. 'J'he man habituatly slept witb. his head ctosely ·covered by the blank!)ts, and this latter fact it was which caused his death by suffocation during a fit. In every clearly ascertained case of epilepsy, it had been invariably my custom to keep the indivi<lual at night in an associated ward, where. gas was always atightl and there was a trained attendant. In spite of strenuous objection~·on my part, ll.S recorded in my evidence before tbe ltoya Commission on penal matters, this arrangement was changed, and the epileptics put to sleep in separate cells. 'J'he death of this man, in a few months after the change, was the result. Now my former custom, of keeping epileptics in an·associated ward, is again resorted to. · ·· ·

. The number of cas.es. of sickness on board the Sacramenlo in 1871 closely approximates that of 1870, the number in 1871 be1pg 319 to 314 the previous year.

'l'he peculiarities worthy of note are the large increase in Zymotic diseases in 1871, and the great decrease or diseases of the digestive org>nis during Lhe sau.e period. In fractures and contusions there is also a marked diflerenoe, there having been in 18{1 only 28, as compared with 55 in 1870. ·

In 1871 there has been but one death; ·this occurred in the case of a Chinaman, who was of weak constitution, and who died of bronchitis. ·

At the Myers-street school we have had 187 cases of sickness with 4 deaths. At Ryrie street, 454 cases with 4 deaths. The children at the latter school are, on the whole,· inferior in physical oondH,ion to thnse in Myers street. '!'his is

because those sent to Myers ·street have been i" picked" in good healtli, whilst Ryrie street, from its first estt>blishment, has been a sort of" sanatorium" for the sickly and delicate. '

Of the 8 denths recorded, 2 were from dysentery, 2 convul8ions, 2 pneumonia, 1 from spinal disease, and 1 from debility arising from hereditary syphilis. , ·

Sickness has been of an ordinary kind, and most)Y of very _short durnt.ion. A female lunatio.was brought in wuo had a uterine polypus of very large dimensions. It was at once cut away, and

· All o~ th~ Zymotic dis.eases were of a ~ery mild form, and yielded re~dily._to treatment. :,J'he death app~aring' under the bead of perJtomt1s had or1gmally been a mild case of typhozd, ·and was discharged from hospttal; there existed, however, a small ulceration of the bowels, which• caused perforation and death in two days.

The health of the prisoners in the Sandhurst Gaol during the year 1871 was, on the whole, generally good, only two cases of death having occurred. 1'hese arose from old age, irregularity of living, and destitution.

The. three fatal oases of pneumonia were all brought into the gaol in a very low state, in fact almost moribund. . ~he oas~ of ileus WaS very rapidly fatal, a loop of small intf;lStine having ·slipped through a small foramen in the omentum,
