•การศกษาทมมาก และมขอมลทอาจไดผล ไดแก•โรค MS (ปลอกประสาทเสอมแขง) ใชในเรองของอาการเกรงหรอปวดจากตวโรค แตไมไดใชในการรกษาตวโรคโดยตรง•โรคลมชกบางชนด ทดอตอยามาตรฐาน•อาการปวดจากระบบประสาทสวนกลางทดอตอยามาตรฐาน

Epilepsy
•Not all epilepsy cases are suitable for cannabis
•The strongest evidence of medical cannabis in epilepsy is approved in•Dravet syndrome
•Lennox-Gastaut syndrome
•Refractory epilepsy needs more data
Drug- resistant epilepsy
•50% of patients with new onset epilepsy are controlled with first AEDs
•If first two AEDs are fail, low chance for achieving seizure control recommend refer to epilepsy specialist (VDO-EEG monitoring & determine candidacy for surgical treatment)

Pseudo-intractability
•Resistance to Rx due to clinical errors:• Incorrect diagnosis
• Inappropriate choice of epilepsy
•Overtreatment (increase side effects)
•Poor medical compliance
•Failure to identify potential precipitating factors (sleep deprivation, stress, flashing lights, menstruation, alcohol consumption)
Cannabidiol & seizure
•Preclinical studies suggest that cannabidiol has anticonvulsive effect
•THC also shows anticonvulsive effect, but also produce psychoactive effect (unlike CBD which is not)
•CBD is widely studied for epilepsy treatment especially in refractory cases

•Severe myoclonic epilepsy

•Severe childhood epilepsy
•Triads:•Multiple seizure types (GTC, atonic and absence)
•EEG: slow (< 3 Hz) spike-and-wave discharge
• Impaired cognitive function
% Seizure reduction from adjunctive Rx placebo controlled from 3 articles

•มขอบงชและหลกฐานเชงประจกษในโรคลมชก Dravet และ Lennox-Gastaut syndrome
•ส าหรบ refractory epilepsy (ใชยากนชกทเหมาะสมแลวอยางนอย 2 ตวขนไป ไมสามารถคมอาการชกได และไดรบการประเมนจากผเชยวชาญประสาทแพทยแลววาไมสามารถใชการรกษาอนได เชน การผาตดโรคลมชก)
•ตองสามารถประเมนและตรวจตดตามไดอยางตอเนอง
•ใชยาในสตรทม CBD:THC อยางนอย 20:1 ขนไป
•ระวงเรอง drug interaction: CBD จะไปเพมระดบยากนชก เชน clobazam phenytoin และการใชรวมกบ valproate จะท าใหการท างานของตบผดปกตได

Medical cannabis in MS
•Active substance:•Tetrahydrocannabinol (THC)
•Cannabidiol (CBD)
•Medication form:•Oral cannabis extract (OCE)
•Synthetic THC
•Nabiximols (spray)
Systemic review medical cannabis in MS
•Symptoms that have some evidences in MS•Patient’s reported spasticity: THC:CBD 1:1
• But not reduced spasticity measured by physician
•Central pain: OCE may reduce central pain
•Urinary incontinence: • OCE & THC not prove to reduce urinary frequency
• Nabiximol may reduce urinary frequency if use in short period (10 weeks)
•Tremor: no benefit

ขอควรระวง
•สารออกฤทธในกญชา โดยเฉพาะ THC มผลตอจตประสาท ซงจ าเปนตองท าการศกษาหาอตราสวนของสารออกฤทธทชดเจน ถกตอง เพอสามารถตดตามอาการและผลขางเคยงได
•กญชา หรอ สารสกดจากกญชา ไมใชยาครอบจกรวาล แมแตการศกษาวจยอยางมระบบในโรค MS ทมขอมลมากทสด ผลลพธหรอประโยชนทได คอ การคมกลมอาการบางกลม (เกรง ปวด) ได แตไมไดครอบคลมอาการอน ๆ ทพบในโรค MS ได เชน กลามเนอออนแรง ตามองไมเหน และยงไมมขอมลวาสามารถยบยงหรอชะลอการด าเนนโรคได
Medical cannabis in MS
•Oromucosal oil:
•THC 2.7 mg CBD 2.5 mg (ratio 1:1)
•Max dose: THC < 25-30 mg/day

How to use THC:CBD oil in MS
•First week: 1 drop (THC ~ 2.7 mg) FU for side effect
•Week 2-12: titrate dose in q 2-3 days with no more than 50% of previous dose with maximum THC ≤ 10.8 mg (4 drops / 3 hours) or maximum of daily dose of THC ≤ 27-32.4 mg (10-12 drops)
•When clinical response reaches benefit or maximum should not increase the dosage
Monitoring side effect during use
•In MS, one of the symptom of MS is fatigue and other medication for symptomatic Rx in MS can produce sedation, drowsiness
•Cannabis side effect : sleepiness, drowsiness, dizziness especially use with clonazepam, lorazepam
•Cannabis + fluoxetine : jitteriness, hypomania
•Other cannabis side effect : dry mouth, fatigue, paranoid thinking

Mechanism of reducing pain
• At periphery: inhibit of ascending nociceptive transmission
• At centrally:
– Spinal cord : Inhibit of neuronal sensitization at dorsal horn and DRG
– Brainstem : Activation of descending inhibitory pathway via nucleus raphe magnus & periaqueductal gray
– Brain : Inhibition of ascending nociceptive transmission (thalamus) & modification of emotional (limbic) & cognition (frontal cortex)

•No high-quality evidence for the efficacy of any cannabis-based medicine, plant-derived THC/CBD combination, in any condition with chronic neuropathic pain
•Some adverse events (somnolence, confusion, psychosis) may limit the clinical usefulness of cannabis-based medicine
Clinical correlations: Neuropathic pain
• Neuropathic pain– ∆9 THC : CBD 1:1 same preparation and concentration as in MS

Laboratory monitoring
• AST, ALT, bilirubin before Rx then 1, 3, 6 month after Rx
(consider to check LFT when changing the dose or
combine use with potential hepatotoxic drug e.g.
valproate)
• If clinical of cannabis toxicity is suspected
Toxicity management
• Symptomatic Rx as presentation symptoms: e.g.
hypotension, arrhythmia
– If overdose is suspected : may consider activated charcoal 30-
100 g in adult or 1-2 g/kg in pediatrics via NG tube
– Acute psychotic: diazepam 5-10 mg oral or IV + keep in quiet
environment
• No anti-dote, gradually improve within 24-48 hours
(cannabis can be withdrew without withdrawal symptoms)
