MAYNARD STEEL QUALITY ASSURANCE PROGRAMPERTAINING TO TRANSPORTATION PACKAGING FOR

TRANSPORTATION OF BY-PRODUCT MATERI ALAS PEQUIRED BY 10 CFR 71

I. PURPOSE OF QUALITY ASSURANCE PROGRAM:

1.0 Maynard Steel Casting Company establishes this QualityAssurance Program to assure that all requirements forcontrol procedures and package design, as required by10CFR71 and approved by the Nuclear Regulatory Commis-si'n, shall be implemented.

2.0 The program shall assure that specific control procedureswill be formulated and followed whenever a shippingpackage containing by-product material is received by,or is shipped from, Maynard Steel Casting Company.

II. ORGANIZATION:

1.0 The final responsiblity for the Quality Assurance Programfor Part 71 Requirement rests wi th Maynard Steel CastingComp any . No design and f abric. on shall be conductedunder this program. The organization of the programshall be 2s listed on the organization chart listed inthis document.

2.0 The Radiographic Administrator shall be responsible forthe overall administration of the program, training,certification and document control.

3.0 The Radiographer shall be responsible for handling,storing, and shipping, inspection, testing, operating andrecord keeping.

4.0 The Personnel Department shall be responsible for auditingthe Quality Assurance Program, including the f unctions ofthe Adminis trator and Radiographers. Audits shall beconducted -t leas t once per year, except in those yearswhen no transfer of radioactive material is made tc orfrom the Maynard Steel f acility. The Personnel Dept.shall be independent frcm and have no responsibility tothe Radiography f acility.

Specifically, the fol.'.owing areas shall be included in the audit:

4.1 All procedures listed in the program art followed(Sections III, IV, V, and VI.)

4.2 Current certification of Radiographers

4.3 Current certification of Shipping package being used(Certified by Supplier) . . <g

4.4 Current certification of instrument calibration.

4.5 Radiation Survey Report of Transportation Package.

4.6 Radiographic log book entrees pertaining to receiptand shipment of radioactive material to or fromfacility.

4.7 All records of purchase r.n i shipment of radio-active materials to or ??am facility.

4.8 Assure that all pertenint records are on file inthe Radiographic f acility files and that they arein a readily retrievable status.

III. QUALITY ASSURANCE PROGRAM:'

l.0 The management of Maynard Steel Casting Company establishesand implements this Quality Assurance Program.

2.0 All by product material received by, or sent out fromMaynard Steel shall be shipped in an N.R.C. approved shippingpackage.

3.0 Only shipping packages owned and supplied by certifiedsuppliers shall be used for transportation of by-productmaterial by Maynard Steel Radiographic facility.

4.0 All shipments transferred as in (2.61 above shall betagged, identified and marked in accordance with10CFR71 requirements.

5.0 Prior to receipt, or shipment by Maynard Steel, of ashipping package supplied by a certified supplier acertification f rcm the supplier will be furnished toMaynard Steel statinc that all regulations pertainingto design, and engineering, identification. labe ling ,and all other items recuired have been approved by theN.R.C. and that said package is capable of transportingthe specified by-product material safely and in accord-an ce wi th N . R. C . criteria. The supplier certificationand copies of all shipping instructions shall be filedat the Maynard Steel Radiographic Facility and shallbe readily retrievable.

6.0 Transfer at Maynard Steel of by-prcduct material into,or out of, the shipping package shall be done by acertified radiographer in accordance with suppliersreccmmended procedure, and instructions as stated inthe operating manual of the Maynard Steel By-ProductLicense.

7.0 Shipping precautions and procedures:

7.1 Incoming or outgoing shipments containing productmaterial shall be monitored with a calibratedsurvey reter to ascertain that .:adiation level isaccept able . The survey results shall be enteredin the log book of the Radiographic Facility.

7.2 Visual inspection shall be made to package toascertain if any physical damage of package isevident. Results of findings shall be recordedin log book of Radiographic Facility.

7.3 "isual inspection shall be made to package toascertain that all labels, markings and identifi-cation are present as required by N.R.C. regulation.Entry of findings shall be entered in log boox ofRadiographic Facility.

7.4 All lecords required shall be readily retrievable.

7.5 Any shipping container arriving at the Maynard Steelfacility which shall be found not to meet all N.R.C.criteria for shipping packages shall be placed"

undisturbed in safe storage in the RadiographicFacility and the N.R.C. immediately notified. Thepackage will remain undisturbed until specificinstructions are received from the N.R.C. as to

proper action to be taken. The facility Radiographerwill maintain security of package until N.R.C. instruc-tions are forthcoming.

7.6 No transfer of by-product material to cr from ashipping package, or receipt, or shipment of ashipping package shall be made to or f rom theMaynard Radiographic Facility except under thedirect supervision of a qualified Radiographerand with full knowledge of the Administrator.

7.7 All transfers of by product material shall be inaccordance wid1 step by step standard proceduresas stated in the Maynard Steel Radiographiccperating manual which is part of the Maynard SteelBy-Product License.

8.0 The administrator shall be notified whene/er a shippingpackage is received by, or sent out from, Maynard Ste 1.

9.0 The administrator shall ascertain that said shipmentsare in compliance with requirements of 10CFR71 and theMaynard Quality Assurance Program for TransportationPackaging.

10.0 The administrator shall enter statement in the MaynardRadiographic Facility log book that compliance has been met.

11.0 No change may be made in this program without approvalof the Administrator, and unless final approval is madeby the Nuclear Regulatory Commission.

IV. DOCUMENT CONTROL:

1.0 All documents pertmining to the transfer to or from MaynardSteel Casting Company shall be under the surveillance andcontrol of the Administrator of the program. The f llowingdocuments shall be included:

1.1 Purchase order for replacement source.

1.2 shipping papers (bill of lading)

1.3 Certification of packaging (f rom certified supplier)

1.4 Radiation survey report of incoming or outgoing packageStatus ~ tags, 13belv and marking in conformance torequiren. ..ts.

1.5 Entry in Radiographer Log Book pertaining to insta.lationor removal of source material to . cping container.

2.0 The above documents are to be kept on file in the Radio-graphers Fa 7ility and readily accessable for inspection.

V. HANDLING, STORAGE AND SHIPMENT:

1.0 Only storage and shipping containers aanufacutred and/orfurnished by certified suppliers of By-Product materialwill be used to trans fer by-product material to or fromMaynard Steel f acilities.

2.0 A shipment will not be received by, or sent from, Maynardfacilities until a certification is received frcm saidsupplier of by-product material stating that the containerbeing used is in complete compliance with requirements of10CFR71.

3.0 Upon reccipt of By-Product material, the package will ceinspected for damage, radio-activity and a radiation surveyreport made.

4.0 Actual transfer of By-Product material to or from thecontainer shall be made in accordance with step-by-stepprocedure listed under Section IX of the Maynard SteelOperating and Procedures for Radiography - March, 1978,which forms part of the current license.

5.0 All of the ri*L;91 handling, stcrage and shipping operationsshall be performed oy certified radiographic personnel.

6.0 All by-product containers received by, or sent from, MaynardSteel shall be surveyed for safe limits of radiation andinspected for proper markings in compliance with require-ments of 10CFR71.

VI. Immediately following a shipment te or from the Maynard SteelRadiography facility, an audit will be made Dy the PersonnelDepart: rent to assure that all necessary document. are on filein the Radiography Department.
