(Carnow Conibear) was retained by the Public Building Commission of Chicago (PBCC) to perform a hazardous material survey prior to renovations at Charles G. Hammond Elementary School in Chicago, Illinois. The survey was limited to materials and areas which could be disturbed during planned renovation activities. The list of the proposed renovations was provided on the design architectural scope sheets and cost estimate from the PBCC, attached. Carnow Conibear reviewed the renovation items prior to the survey to determine which areas of the school may be affected by the renovation activities. The hazardous materials survey was conducted on May 14, 2013 and consisted of visually inspecting the school building proposed renovation areas to determine the presence and location of polychlorinated-biphenyl (PCB) containing equipment (lighting ballasts, switchgears, transformers, and hydraulic fluids), mercury-containing equipment (mercury lamps, thermostats, switches, thermometers, regulators, and gauges), and other hazardous chemical wastes, such as chlorofluorocarbons (CFCs) or equipment. All items listed were assumed to contain the related hazardous materials since no sampling or internal investigatory inspection of electrical or other equipment was performed as part of the survey. Based upon the survey and limitations of the scope sheets, Carnow Conibear identified the following materials that will require special handling during their removal as part of the renovation work. Carnow Conibear did not include the movable equipment, computers, and other class room supplies or equipment. Carnow Conibear understands that these items will be relocated out of renovation areas as deemed necessary by the Chicago Public Schools. Since the renovation scopes do not include the details of the final design and all potential areas that could be affected by the future Design Builders work, Carnow Conibear has included the following list of materials commonly located in areas of the school.

Incorporate data from this report into the design and renovation documents. Identify all hazardous materials requiring removal/handling within the final design

renovation scope. Include additional items from Table II as applicable. Perform internal inspections and sampling of all materials requiring removal to ensure

proper characterization of waste materials. Coordinate inspection of electrical equipment with a licensed electrician. Properly dispose of all materials which are removed as part of renovation work in

accordance with local, state and federal, regulations. Properly document the handling and disposal of all wastes generated as part of

renovation work.
