Cancer care at Marshfield Clinic Health System is continuously evolving. We are now providing more cancer treatment options than ever before with six cancer centers and nine outreach locations. The enclosed information provides a snapshot of the cancer care options our Health System can provide you right in your backyard. I hope it provides the information you and your doctor need to make an important decision – where you are going to receive your cancer care.

To our cancer survivors…We are here for you. We have assembled a team of specialists that are world-renowned for cancer care right here in northern Wisconsin. You don’t have to travel three hours for premier cancer care.

Marshfield Clinic Health System has six cancer centers that feature the best people and latest technologies. We are here to help you fight this the entire way.

To those doctors helping our cancer survivors…Cancer care at Marshfield Clinic Health System includes diagnostic and therapeutic surgical services; radiation oncology services; medical oncology services like chemotherapy, palliative care and hospice services; and inpatient services for diagnostic and supportive needs. Our patients also have access to clinical trials through the newly-formed Cancer Care and Research Center, which has further strengthened our 35-year history of clinical trials at the Health System.

In 2017, new cancer cases diagnosed and treated by the Health System totaled 4,210 cases. Similar to national data, the top five cancer diagnoses were breast cancer, lung cancer, colorectal cancer, prostate cancer and melanoma. A total of 12,909 patients received medical oncology services, while 1,373 patients received radiation oncology services. In 2017, our cancer centers served patients in 31 counties in Wisconsin with additional patients served from the upper peninsula of Michigan.

So much is going on in oncology overall, including advanced multi-diagnostic methods and new advances in biology and chemotherapy. We have been flexible and adaptive to these changes so we continue to provide top-of-the-line cancer care.

Cancer Care at Marshfield Clinic Health SystemAt Marshfield Clinic, we have a team of specialists that provide a complete lineup of cancer care options and deliver a specific plan for each patient.

Medical oncology and hematology servicesOur doctors provide a complete lineup of medical oncology and hematology, and cancer care options that include chemotherapy, immunotherapy, and hormonal, biological and targeted therapies.

Chemotherapy and infusion services are provided by experienced, certified oncology nurses in outpatient and inpatient settings. Our nursing staff is certified to safely administer chemotherapy and biotherapy agents by the Oncology Nursing Society. All of our sites are certified by the Quality Oncology Practice Initiative (QOPI). We are one of the few programs with this designation in the state of Wisconsin.

Medical support staff members are on site each day to facilitate your needs and the needs of your cancer care team.

To complement our infusion service, specialty oncology pharmacies provide services and support for mixing cancer-based medications at our cancer centers.

We provide support services that are holistic for the mind and body including assisting with difficult medical decisions, coordinating care with other providers through a complex health care system and providing emotional support.

We also perform cancer research to help fight and better understand cancer and collect data about cancer and tumor diseases through our Cancer Registry Program.

Radiation oncology servicesRadiation oncology continues to advance as a treatment option for cancer patients, which is why we continue to offer more radiation oncology services throughout our cancer centers. Three new radiation oncology sites have opened in the last 18 months. We also will be upgrading our linear accelerator at our Minocqua Center in early 2018.

Surgical oncology servicesAll cancers, including rare and complex tumors, are addressed within a complete cancer care treatment plan for our patients.

Our surgical oncologists are a part of your cancer care team, participating in weekly meetings with Medical Oncology/Hematology and Radiation Oncology. This approach provides the most advanced care and has shown to result in the best patient outcomes.

Our oncology team includes three fellowship-trained surgical oncologists. Dr. Rohit Sharma and Dr. Jessica Wernberg are based at Marshfield Center. Dr. Sharma also sees patients in Wausau, while Dr. Wernberg also sees patients in Stevens Point. Dr. Sanjoy Saha is based at Eau Claire Center and also sees patients in Chippewa Falls.

Specialized cancer surgery also is offered by other surgical sub-specialists including gynecologic oncology, ENT oncology, dedicated breast surgery, Mohs dermatology, colorectal surgery, urology and others. Marshfield Medical Center is home to a high-volume pancreas cancer center with outcomes comparable to the best cancer centers.

For those with cancers that affect the abdomen, Marshfield Medical Center surgical oncologists are trained in a procedure that delivers promising results. This procedure includes heated treatments aimed at abdominal cancer, called hyperthermic intraperitoneal chemotherapy (HIPEC). This is done after noticeable cancerous tumors are removed.

Marshfield Clinic Health System is collecting data on pancreatic, esophageal and HIPEC surgeries to track patient results. This provides information to improve surgical outcomes.

Minimally invasive and robotic surgery using the da Vinci Surgical System® robot is offered by an experienced team.

Cancer Care and Research CenterAfter more than 35 years offering clinical trials through the National Cancer Institute’s Community Clinical Oncology Program, Marshfield Clinic and Marshfield Clinic Research Institute partnered March 2018 to create the Cancer Care and Research Center. This combines oncology research and clinical care so every patient receiving cancer care from us can participate in a clinical trial.

More than 50 percent of cancer patients are cured largely because of laboratory and clinical research, which leads to new cancer treatments. Clinical trials provide access to new anti-cancer therapies, often long before they are available elsewhere.

The clinical trials offered to our patients include: • New drug therapies including targeted therapy • Surgical studies • Radiation therapies • Autologous bone marrow transplantation • Genetic advancements • Cancer control studies

We partner with national research centers such as: • National Cancer Institute funded NCORP • Alliance for Clinical Trials in Oncology • Cancer Trials Support Unit • Children’s Oncology Group • ECOG-ACRIN Cancer Research Group • National Cancer Institute funded NCORP • NRG Oncology • South West Oncology Group • URCC Research Base • Wake Forest Research Base • Wisconsin Oncology Network (University of Wisconsin)

In addition to the latest clinical trials, the Cancer Care and Research Center studies the delivery of cancer care, making it more effective, patient-centered and affordable.

The Center partners with researchers from other centers in the Research Institute to prevent cancer and diagnose it early, which could change cancer care. For example, these partnerships are identifying genetic biomarkers for breast cancer progression, optimizing the clinical management of lung nodules, improving efforts to help smokers quit and finding support interventions to ease financial and psychological distress after a cancer diagnosis.

At the August 2017 National Cancer Institute Community-Based Participation NCORP Annual Meeting, we were awarded physician accrual awards to the following:

• Gold Certificate of Excellence Award to Dr. Adedayo Onitilo and research staff

• Silver Certificate of Excellence Award to Dr. Arlene Gayle and research staff

• Certificate of Appreciation Award to Dr. Michael McManus and research staff

Specialty oncologyGynecologic oncology Gynecologic cancers involving the female reproductive tract present unique challenges. These cancers include those of ovaries, fallopian tubes, uterus, vagina, vulva and cervix. Implementation of cutting-edge treatments has brought us to a place where many of these cancers can be prevented, detected earlier, controlled or cured.

Comprehensive gynecologic oncology services have been provided at Marshfield Medical Center-Marshfield for more than 30 years. Our team offers a full menu of surgical therapies including classic, time-honored techniques as well as the most modern and up-to-date options for appropriate candidates. Minimally invasive surgery and treatments designed to lessen side effects and speed recovery are routinely employed.

Our surgical team is based at MMC-Marshfield, but outpatient management is offered through outreach and by Telehealth. Our partnership with other Marshfield Clinic Health System professionals allows for chemotherapy and radiation therapy to be provided locally. Referrals are welcome and patients will receive prompt, comprehensive service while keeping primary and other treating providers updated throughout.

Gastroenterology and endoscopic oncologyEach cancer center has a gastroenterologist on staff to help diagnose colon cancer and remove colon polyps. These gastroenterologists can also help diagnose cancer in the esophagus, stomach, duodenum, rectum, pancreas and gall bladder.

We also have a fellowship-trained endoscopic oncologist, Sabo Tanimu, M.D., FACP. He is specially-trained to treat specific cancers using targeted radiation therapy and radiofrequency ablation.

Neuro-oncologyNeuro-oncologists diagnose and treat neurologic complications of cancer outside and inside the nervous system.

Neuro-oncology is a multidisciplinary service that includes neurologists, neurosurgeons and neuro-radiologists that area part of your cancer care team.

Neuro-oncology offers the latest cancer therapies, including surgery, radiosurgery (Gamma Knife® and linear accelerator), chemotherapy and combinations of all three.

Supporting youPalliative Care - a holistic approach to mind, body and spiritPalliative Care is holistic care for the mind, body and spirit that is provided at every stage of cancer care by the cancer care team.

We aim to give you the best possible quality of life after diagnosis. This may include:

• Relieving pain and other symptoms.

• Assisting with making difficult medical decisions.

• Coordinating care with other providers through a complex health care system.

• Guiding you in making an advanced care plan based on your needs, concerns and goals.

• Providing emotional and spiritual support and guidance.

We provide palliative care even if a patient is enrolled in hospice. Hospice is a home care program that provides important services to our patients who need end-of-life care. It is designed to help families care for their loved ones wherever they reside and is provided a few hours every day, but a nurse is always available 24 hours a day.

Navigating your journey Our Oncology nurse navigators will be with you during your journey. Navigators will stay in close contact with you and your family while you go through the care continuum to help with care coordination, efficient care delivery and removal of any barriers. These individuals are patient-centered and ensure your needs are met.

Our Oncology financial navigators are with you at the beginning of your journey. They give peace of mind and allow you to focus on healing. This team verifies insurance and provides financial counseling to patients. In addition, they connect you with drug assistance, co-pay support and insurance coverage options.

Social services and behavioral health supportOur licensed social workers embedded at our cancer centers help provide you with resources and coping strategies along your journey. Additionally, our behavioral health services support our patients with any level of psychological or social distress.

WINGS® Cancer Survivorship ProgramWe offer a unique program called WINGS (We Inspire, Nurture and Give Support). This cancer survivorship program offers holistic care for adult patients throughout their cancer treatment and is part of our commitment to quality patient care.

WINGS provides an environment that nurtures healthy survivorship for individuals experiencing cancer.

Survivorship coordinators are located throughout the system to meet with patients after they have finished their last treatment to continue our journey together.

WINGS offers an array of tailor-made programs and services to help all who are affected by cancer and their loved ones. We also provide various support groups to provide additional help during your journey.

CLIMB® (Children’s Lives Include Moments of Bravery)Our CLIMB program helps kids find the courage to deal with cancer in their families. This course, developed by the non-profit Children’s Tree House Foundation, gives emotional support to kids who have a parent or other loved one diagnosed with cancer. CLIMB helps children moderate their feelings without having to “stuff” them or feel overwhelmed. Participants bond quickly with other children going through a similar experience.

Our CLIMB program consists of weekly group sessions. Specially-trained leaders encourage open discussions about cancer, treatments and possible side effects.

CLIMB is offered over a series of weeks free of charge. The program is geared toward children 5 to 12 years old. You can ask your cancer care team how to register.

Hope Lodge and partnership with American Cancer SocietyMarshfield Clinic Health System partners with the American Cancer Society in various manners to better meet the needs of our patients.

The Hope Lodge, run by the American Cancer Society, in Marshfield serves as a home away from home for our adult cancer patients. It is free of charge and available to our patients that have to travel to Marshfield to receive life-saving cancer treatment. It is the only American Cancer Society Hope Lodge in Wisconsin and serves patients from as far away as Michigan.

The Living with Cancer Support Group funded by the American Cancer Society provides valuable “Care and Share” meetings to provide support for each of our patients through their cancer journey.

We also partner with the American Cancer Society in their ride share program and various support programs such as Look Good Feel Better, I Can Cope and Reach to Recovery.

10

Genetics and your risk for cancerAbout 5 to 10 percent of cancers are related to your genetics. A genetic mutation can lead to an increased risk of getting cancer.

It is important to identify families that have this genetic mutation because medical management can be personalized to better manage the risks. This can be done through screenings, earlier detection and risk-reducing options. This can then provide opportunities to participate in research.

Medical Genetics has genetic counselors that work with our providers to find people that may benefit from genetic testing. Patients who should talk to their doctor about having a cancer genetic test completed include those that have had:

• Breast, uterine or colon cancer diagnosed at younger than 50-years-old.

• Multiple close relatives, on the same side of the family, who have been diagnosed with cancer.

• More than one primary cancer, for instance bilateral breast cancer or a primary colon cancer and a primary stomach cancer.

• A rare type of cancer, for instance ovarian, male breast or metastatic prostate cancer, or tumor pathology.

• A family history of a hereditary cancer syndrome or genetic mutation.

Patients are then referred to our genetic counselors to determine if a cancer genetic test should be completed. This meeting can be done in person at the Marshfield Center or using our telehealth services, which can reach more than 30 Marshfield Clinic Health System sites.

In addition to helping find people at risk for cancer, genetic counselors:

• Determine what genetic tests should be completed for patients and their families.

• Identify the most informative family member to test.

• Discuss the pros, cons and possible outcomes of testing.

• Help match patients with testing options that are financially feasible.

• Interpret genetic test results.

• Discuss preventative treatment plans for those identified to be at higher risk.

• Provide information and support resources.

Our genetic counselors help educate and empower individuals so they can make informed decisions regarding their health care. This leads to opportunities for more personalized and proactive medical management.

• Child psychology

• Pediatric neurologists

• Pediatric physical medicine and rehabilitation

• Pediatric oncology nursing

• Child life specialists

• Pediatric sedation service

• Pediatric pain management and palliative care

Our pediatric oncology team is a member of the Children’s Oncology Group, which is the world’s largest organization devoted exclusively to childhood and adolescent cancer research.

We also offer childhood cancer services in our Eau Claire Cancer Center.

Treyden’s CabinTreyden’s Cabin opened September 2015 and transformed the waiting and patient rooms in Outpatient Pediatric Oncology at Marshfield Medical Center into forest murals with bright colors and woodland scenes to put children’s minds at ease during treatment. This theme continues to the Inpatient Pediatric Oncology Unit, where each room has a different nature-focused mural. The unit’s solarium offers pediatric cancer patients a fun get-away from their hospital rooms.

Latest innovations at your fingertips

Gamma Knife®Gamma Knife radiosurgery, also called stereotactic radiosurgery, is a type of radiotherapy treatment that is offered at Marshfield Medical Center. Even though it’s called surgery, a Gamma Knife procedure does not use surgery, and it isn’t a knife. Gamma Knife uses very precise beams of gamma rays to treat an area of cancer.

It’s most often used in the brain. Beams of gamma radiation send a very intense dose of radiation to a small area without a need to make an incision, destroying the cancer cells. The beams of radiation are precisely focused to reach the cancer cells, with little effect on nearby healthy tissue.

HDR brachytherapyHigh-Dose Rate (HDR) brachytherapy uses temporary implants that attack the cancer cells from inside the body. This treatment is available at Marshfield and Stevens Point centers.

The temporary implants are placed into the tumor or nearby tissue and give off radiation. With HDR brachytherapy, the implants stay in the body for several minutes and then are removed.

Autologous stem cell transplantsMarshfield Medical Center is one of three locations in Wisconsin that provides autologous stem cell transplants. This procedure allows a patient to undergo extensive chemotherapy treatment to kill the cancer. A patient’s stem cells are then used to reproduce blood-producing cells that were destroyed during chemotherapy.

The treatment is offered to patients with certain hematologic cancer types such as lymphoma and multiple myeloma. The program has a less than 5 percent mortality rate a year after the transplant, which is in line with the national average.

The stem cell transplant program is now under the leadership of Arlene Gayle, M.D. after Dr. Richard Mercier served in that role for more than 15 years. The program is accredited through the Foundation of the Accreditation of Cellular Therapies (FACT).

Leaders in wound healingMarshfield Medical Center features a hyperbaric chamber that helps heal wounds, which can be used after a patient undergoes cancer surgery or radiation therapy.

Hyperbaric medicine uses increased oxygen levels to heal wounds faster than in a normal environment, making hyperbaric medicine an important tool when fighting a time-sensitive disease like cancer. A new chamber opening in 2018 can treat up to 10 patients at a time or 36 patients a day, making it the largest chamber in central Wisconsin.

“I am passionate about encouraging my patients with cancer to have hope and focusing that hope into living each day to the fullest.”

- Dr. Arlene Gayle, oncology

Leader in breast cancer treatmentThe number one cancer diagnosis we see is breast cancer, so we are continuously working to make breast cancer screening and treatment a priority. This includes bringing new ways to treat breast cancer and more screening options to your doorstep.

Accelerated Partial Breast IrradiationAccelerated Partial Breast Irradiation (APBI) is offered at the Stevens Point and Marshfield centers. Patricia Lillis, M.D., M.H.A., radiation oncologist, and Anna Seydel, M.D., FACS, surgical oncologist, are pleased to be able to offer this treatment to their patients. APBI differs from traditional radiation treatments in that only the portion of the breast that contained the tumor receives the high dose of radiation, as opposed to the entire breast with conventional treatments. Additionally, APBI patients are treated twice a day for five days, as opposed to the traditional treatments of once per day for approximately six weeks.

For APBI treatments, Dr. Seydel will temporarily place a device in the area of the breast in which the tumor was removed. This device, called the SAVI® (Strut Adjusted Volume Implant) from Cianna Medical, is used to deliver the high dose of radiation to the tumor site. The SAVI is left in place for the duration of the treatments – five days. This device has various struts or channels for which the radiation can be delivered. Once the device is put in place, the struts are expanded to fill the space where the tumor was located. This device comes in four different sizes so it can be customized for each patient.

Once the device is in place, Dr. Lillis plans the radiation treatments. Utilizing CT scans, Dr. Lillis and her team will plan how the dose is to be delivered.

The plan is done once and used for all ten treatments. The plan is verified before every treatment to assure quality and accuracy. The radiation is then delivered using a High-Dose Rate (HDR) brachytherapy unit that attaches to the SAVI device. The treatments are delivered twice per day, once in the morning and once in the afternoon. Once the final treatment is completed,

Dr. Lillis removes the SAVI device and places a dressing on the area. The patient returns to the Clinic in a few days to check the catheter device exit site.

Not all patients are good candidates for this type of treatment. We encourage you to contact Dr. Seydel or Dr. Lillis to see if you or someone you know is eligible for this treatment.

Mobile mammographyMobile mammography is a powerful tool to expand access to important screenings for women in the fight against breast cancer.

We have three mobile units that provide a variety of services including mammography, bone density screenings and molecular breast imaging. The unit provides precise integration of image data for the best possible image, less patient discomfort and efficient transfer of imaging information to doctors.

Our mobile fleet has traveled to 70 of Wisconsin’s 72 counties. In 2016, 5,785 patients were seen in our mobile mammography units and 26 positive tests for breast cancer were found during these screenings.

High Risk Breast ClinicsFour of our cancer centers are High Risk Breast Clinics that provide breast cancer screenings, chemoprevention and genetic risk evaluations for individuals at high risk for breast cancer. The centers are accredited through the National Accreditation Program for Breast Centers (NAPBC).

Your impact on Cancer Care Every day we hear stories of bravery, hope and survivorship from cancer patients throughout our system of care. While each cancer journey is different, all of them have been touched in some way by philanthropy.

Gifts to Marshfield Clinic Health System Foundation provide consistent support for important cancer care and research programs throughout our health system. All of the gifts to cancer care and research stay local to fund important programs, like:

• WINGS (We Inspire, Nurture and Give Support): A cancer survivorship program offering holistic care for adults navigating cancer treatment.

• CLIMB (Children’s Lives Include Moments of Bravery): A program helping children understand a parent or adult loved one’s cancer diagnosis.

• Support groups: Women Living with Hope and Man to Man support groups provide needed support during cancer treatment.

• Community events: Our staff is involved in community events including Relay for Life, Making Strides and American Cancer Society as a part of the fight against cancer.

• Mobile mammography: Three mobile units that travel throughout central and northern Wisconsin, delivering preventive services to patients across our service area.

• Clinical trials: Innovative treatment options for cancer patients, including participation in national research studies.

• Angel funds: Resources offered for patients in need to assist with non-medical expenses during treatment, such as gas cards, electric bills, mortgage and rent payments, and more.

• New diagnostic and treatment equipment: The latest tools to diagnose and treat cancer patients for the best chance of remission.

• Patient support items: Hats, wigs, scarves, blankets and educational material are among the comfort items offered through patient care funds and are offered at our cancer center boutiques.

In addition to ongoing support for our cancer programs, recent donations funded new, thoughtfully-designed spaces in our cancer centers. New cancer consult rooms in Marshfield, Stevens Point and Eau Claire, and an expansion of the Shine on Boutique in Marshfield, offer comforting, healing areas for patients and families.
