� If unsure ask your doctor as you need to answer truthfully

� You have been advised by a physician not to travel at this time;

� You have had or are waiting for an organ or bone marrow transplant (excluding corneal transplant);

� You require kidney dialysis; d. In the last twelve (12) months, you have been prescribed or taken Lasix or furosemide for any reason or you have had heart failure;

� You have been diagnosed with a medical condition for which a physician has estimated that you have less than six (6) months to live;

� In the past twelve (12) months you have used or been prescribed home oxygen;

� You have ever been diagnosed with AIDS (Acquired Immune

Medically Underwritten Plans (MUPS)
