Chiropractic versus Medical Coding

Question:I recently switched from coding & billing with a 97.5% collection rate to a Chiropractic Group.

Now 50% of my claims are being denied- particularly with Medicare and I am not doing anything differently than I did at my prior job. ANSWER

Question: What E/M code should I use when a new patient comes in to the physician office and the doctor documents the 99201-99205 codes along with the preventive codes or well visit codes? I believe the 99201-99205 would be appropriate even though the physician states he did the well visit too.

Question:I am getting confused trying to code for ALIF in ICD-10-PCS.

It seems so different from ICD-9-CM vol 3 codes. ANSWER

Question: Would you be able to bill 2 separate CPTs? Comprehensive preventive medicine and a problem visit? In situations when patient comes in for wellness visit and problem is identified.

Global Surgical Package

Question: Global surgical package; what would neuroplasty median nerve be considered; minor or major? Also, can provider bill for application of short arm splint?

25 Modifier

Question: Must the 25 modifier be linked to a different diagnosis than the procedure?

57 Modifier and Surgery

Question: Question on 57 modifier, an auditor told me that if the decision for surgery was made 4 days prior to the actual surgery we must put the 57 on that E/M. I have not found anything regarding this, only that it is for the day before or day of surgery. Do you have any information on this? Thanks!

E/M Discharge Do’s and Dont’s

Question: Can you explain E/M Discharge do's and dont's ..example DC note written on a day OTHER than the actual DC date. What is acceptable and what isn't.
