Documenting Supplementary Aides and

Services

• Be sure the accommodation clearly states what the student needs and includes:

• When the student needs it (i.e. which classes or subjects)• How often they need it (Frequency)• For how long the need it (Duration)

• One should be able to clearly show whether the student has received this accommodation.

• “As Needed” should never be used.

A Document has been added to the SEIS document library that contains examples of various Supplementary Aides and Services.

• The examples in this document should be adjusted to fit the varying needs of your individual students.

• If you have questions on how to write up a particular accommodation, please contact your Program Specialist.

To access this document, hover over “Reference Materials” on the grey navigation bar and click on “Document Library”.

Scroll down to the “Resources” section of the document library and click “Documenting Supplementary Aides and Services” to download the document.
