1. Notes & Reminders: 1.1. The base unit weighs approximately 110 lbs; be careful when lifting or

moving! 1.2. Required installation tool (1/8” Allen Wrench) is supplied in Hardware

Kit. 1.3. After complete assembly of the BLU-U, initiate (1) cycle (16 minutes 40

seconds) per installation instructions to ensure proper set-up and installation.

1.4. Check serial numbers of all assemblies to ensure proper installation procedure is followed.

1.5. Please remove all packaging from the customer’s office before you leave!

2. Safety Precautions: 2.1. Always wear appropriate blue light blocking eye protection when

operating the BLU-U.

3. Tool List: 3.1. ⅛” Allen Wrench (supplied in Hardware Kit) 3.2. Packaging tape 3.3. Straight slot screwdriver

4. Installation of the BLU-U® 6000 Series Head with 10,000 Series Post & Base:

Step 4.1 Locate and verify the integrity of the three (3) boxes in which the BLU-U modules are shipped:

• Stand Post (approx. 40 lbs)

• Base (approx. 140 lbs)

• Head (approx. 48 lbs)

Step 4.2 Locate the box labeled “Stand Post Assembly”. Remove the post, yoke, hardware kit, and installation instructions from the box. Place post and yoke on a carpet or other non-scratch surface.

Step 4.3 Verify contents of the Hardware Kit (ASD-1419), clockwise from top: (2) Keys (2) Handles (to replace head knobs) (3) #10-32UNF x 5/8” Lg Flat Head Screws (spare screw provided) (2) Bottles of Touch-up Paint (1-White; 1-Blue) (1) 1/8” Allen Wrench

Step 4.4 Locate the box labeled “Base Assembly”. Remove box cover, top foam packaging, and plastic covering. Open the ramp end of the box in preparation to roll the base out. Grasp the sides of the base and lift slightly while rolling the base out of the box. The side support blocks will slide out along with the base. When the base is completely out of box the side support blocks can be removed.

Note: Be careful when handling the base due to heavy weight.

Step 4.5 Position the bottom of the Post near the Base. Connect the Post and Base electrical receptacles.

Note: Ensure the keyed connectors are properly aligned.

Step 4.6 Position the Post over the Base. Feed the slack wires into the post mounting shaft and up into the Post.

Step 4.7 Slide the Post onto the post mounting shaft.

Note: Ensure the wires are not pinched between the post and base.

Step 4.8 Position the post as shown and tighten the screw at the bottom of post until seated. Required 1/8” Allen Wrench is provided in the Hardware Kit.

Step 4.9 Position the Yoke near the yoke mounting shaft. Connect the Post and Yoke electrical receptacles.

Note: Ensure the keyed connectors are properly aligned.

Step 4.10 Slide the Yoke onto the yoke mounting shaft. To assist in mounting the Yoke, the head cable may be gently pulled out of the yoke arm approximately 1” – 2” while sliding the Yoke onto the yoke mounting shaft.

Note: Ensure the end cap does not disassemble from the yoke arm while pulling the cable.

Step 4.11 Rotate the Yoke upwards until the screw holes are aligned. Fasten the Yoke to the yoke mounting shaft with the (2) #10-32UNF x 5/8” Lg Flat Head Screws. Required 1/8” Allen Wrench is provided in the Hardware Kit.

Step 4.12 If required, lift the Pivot Latch and rotate the post until the pivot pin locks the post in the open position.

Step 4.13 Rotate brake handle counter-clockwise until the brake disengages (approximately ¼ - ½ turn). Rotate Yoke downwards, and then rotate brake handle clockwise until the brake engages.

Note: Do not over-tighten the brake handle.

Step 4.14 Locate the box labeled “Head Assembly”. Remove the head and place on a carpet or other non-scratch surface.

Step 4.15 Remove the knobs from the head and install the (2) handles provided in the Hardware Kit. Unscrew both handles approximately ¾ of their travel (approximately 1 inch). Ensure the spacer washers on each bolt are recessed into the depressions in the plastic housing on both sides of the unit. Ensure the two (2) tabbed washers on each side are spread out (one against the handle, the other against the spacer washer).

Step 4.16 Mount the Head onto the Yoke. Ensure the spacer washer and (1) tabbed washer are located closest to the Head. Ensure there is (1) tabbed washer located closest to the handle. Tighten the head handles.

Step 4.17 Insert the circular connector of the head cable into the circular connector on the head and tighten.

Step 4.18 Verify unit is assembled correctly as shown in photo at right:

• Yoke and Head are to right of base.

• Head has new Handles provided in Hardware Kit.

Step 4.19 Functionality Checks:

• Loosen the Pivot Latch and rotate the post 90° clockwise to the storage position.

• Loosen the Pivot Latch and rotate the post 90° counter-clockwise to the open position.

• Release the Break Handle and raise/lower the head several times.

Step 4.20 On the back side of the base, verify the power inlet module is set for the correct line voltage.

Step 4.21 To change the voltage setting, carefully open the cover with a straight slot screw driver, slide the red fuse block out using the straight slot screw driver, turn 180°, replace the red block, and then close the cover. Reference photos below.

Note: The desired voltage reading should be right side up and visible through the window of the power inlet module.

Step 4.22 Locate the power cord and plug the IEC320 end into the inlet on the back of the base. Ensure the main power switch on the base is in the “0” or off position (“0” pressed down) and that the fuse block is fully seated.

Step 4.23 Plug the power cord into a standard wall outlet. Switch the main power switch on the base to the “l” or on position (“l” pressed in).

Step 4.24 Note: Wear appropriate blue light blocking eye protection during the following steps. Locate the key supplied in the Hardware Kit, insert it into the key switch and turn to the “l” or on position.

Step 4.25 The timer should display the time 16:40. If not, set the timer to 16:40 by pressing the “up” or “down” arrow as required. Press the “Start/Stop” button to initiate timer countdown. Let the timer count down to zero (i.e. the unit should be run through a full 16 minute 40 second cycle).

Note: Reference troubleshooting Section 5 if Timer displays “Err” or has a loss of functionality. If problem persists, contact DUSA Customer Service to resolve the issue.

Step 4.26 When pressing the “Start/Stop” button the first time:

• Ensure the neon status indicator lamp on the control panel flashes three times (3X).

• All seven (7) lamps light

• The lamps turn off when the timer reaches zero (“0”).

Turn the key switch to the “0” or Off position.

Step 4.27 Complete the appropriate section of FORM043 Installation/Service Form BLU-U® Blue Light Photodynamic Therapy Illuminator Model 4170.

Step 4.28 Train customer per section 8.0.

Step 4.29 Remove the packaging material from the customers office.

5. Troubleshooting Symptom Possible Cause What To Do

Timer display is illuminated. Lamps and fans not operating.

Improper orientation of fuse holder in the Power Inlet Module.

• Verify proper orientation of fuse holder in the Power Inlet Module, located at rear of the base, see Step 4.21 above.

• If normal operation does not resume call customer service to receive further instructions.

No Power One or more fuses in the Fused Power Entry Module have blown.

• Install new fuses in the Power Inlet Module, located at rear of the base.

• If this is a new installation, a fuse kit (D1015) has been provided with the unit.

• If normal operation does not resume, or a fuse kit is not available, call customer service to receive further instructions.

Timer display is illuminated. Fans are running Lamps not operating.

F2 fuse in head is blown.

• Install new F2 fuse in the head. • If this is a new installation, a fuse kit

(D1015) has been provided with the unit.

• If normal operation does not resume, or a fuse kit is not available, call customer service to receive further instructions.

System Status Indicator does not flash three (3) times when the Start/Stop button on the Timer is pressed.

Neon light or control circuitry is not functioning properly.

Call customer service to receive further instructions.

System Status Indicator flashing, on steady, or on intermittently during timing cycle.

Power output is above/below specified range. Control circuitry is not functioning properly.

Call customer service to receive further instructions.

6. De-Installation of the BLU-U®: 6.1. Identify the 4170 BLU-U® system. 6.2. Identify that the packaging for the 4170-1 Head, 4170-2 Post and

4170-3 Base are on site. 6.3. Turn the 4170 system “on” and “off” to verify that the unit is

functioning properly. Record on FORM043. 6.4. Disconnect and package the unit(s) in the appropriate boxes. 6.5. Once the unit(s) is packaged and taped, move to a safe location. 6.6. Complete FORM043 and send and/or fax to DUSA Customer

Service.

7. Return of BLU-U® Component (Head, Post or Base): 7.1. Identify that the packaging for the 4170-1 Head, or 4170-2 Post, or

4170-3 Base are on site. 7.2. Package the unit in the appropriate box. 7.3. Tape the box for shipment and move to a safe location. 7.4. Complete FORM043 and send and/or Fax to DUSA Customer

Service.

8. On-site Personnel Training: 8.1. Review and demonstrate with Clinician(s) operation of the system

using Operating Manual BLU-U® Blue Light Photodynamic Therapy Illuminator Model 4170 (MAN-1211) concerning: 8.1.1. How to turn power on at power inlet module. 8.1.2. How to operate the timer. 8.1.3. How to place post in storage mode. 8.1.4. How to raise and lower the head, and lock/unlock brake.

8.2. Have the Clinician sign the Installation/Service Form BLU-U® Blue

Light Photodynamic Therapy Illuminator Model 4170 (FORM043) confirming that he/she has been trained.

8.3. Turn the keyswitch to the OFF position. 8.4. Return signed Installation/Service Form BLU-U® Blue Light

Photodynamic Therapy Illuminator Model 4170 (FORM043) to DUSA Pharmaceuticals Corporate Office, Customer Service Department via fax.
