Dear Mr. Erickson:

The Minnesota Department of Health assists the Centers for Medicare and Medicaid Services (CMS) by

surveying skilled nursing facilities and nursing facilities to determine whether they meet the requirements

for participation. To participate as a skilled nursing facility in the Medicare program or as a nursing facility

in the Medicaid program, a provider must be in substantial compliance with each of the requirements

established by the Secretary of Health and Human Services found in 42 CFR part 483, Subpart B.

Based upon your facility being in substantial compliance, we are recommending to CMS that your facility

be recertified for participation in the Medicare and Medicaid program.

Effective November 20, 2015 the above facility is certified for:

65 Skilled Nursing Facility/Nursing Facility Beds

Your facility’s Medicare approved area consists of all 65 skilled nursing facility beds.

You should advise our office of any changes in staffing, services, or organization, which might affect your

certification status.

If, at the time of your next survey, we find your facility to not be in substantial compliance your Medicare

and Medicaid provider agreement may be subject to non-renewal or termination.

Dear Mr. Erickson:

On November 10, 2015, we informed you that we would recommend enforcement remedies based on

the deficiencies cited by this Department for a standard survey, completed on October 29, 2015. This

survey found the most serious deficiencies to be isolated deficiencies that constituted no actual harm

with potential for more than minimal harm that was not immediate jeopardy (Level D), whereby

corrections were required.

On December 10, 2015, the Minnesota Department of Health completed a Post Certification Revisit

(PCR) by review of your plan of correction and on November 17, 2015 the Minnesota Department of

Public Safety completed a PCR to verify that your facility had achieved and maintained compliance with

federal certification deficiencies issued pursuant to a standard survey, completed on October 29, 2015.

We presumed, based on your plan of correction, that your facility had corrected these deficiencies as

of November 20, 2015. Based on our PCR, we have determined that your facility has corrected the

deficiencies issued pursuant to our standard survey, completed on October 29, 2015, effective

November 20, 2015 and therefore remedies outlined in our letter to you dated November 10, 2015,

will not be imposed.

Please note, it is your responsibility to share the information contained in this letter and the results of

this visit with the President of your facility's Governing Body.

On October 29, 2015, a standard survey was completed at your facility by the Minnesota Departments of Health

and Public Safety to determine if your facility was in compliance with Federal participation requirements for

skilled nursing facilities and/or nursing facilities participating in the Medicare and/or Medicaid programs.

This survey found the most serious deficiencies in your facility to be isolated deficiencies that constitute no

actual harm with potential for more than minimal harm that is not immediate jeopardy (Level D), as evidenced

by the attached CMS-2567 whereby corrections are required. A copy of the Statement of Deficiencies

(CMS-2567) is enclosed.

Please note that this notice does not constitute formal notice of imposition of alternative remedies or

termination of your provider agreement. Should the Centers for Medicare & Medicaid Services

determine that termination or any other remedy is warranted, it will provide you with a separate formal

notification of that determination.

This letter provides important information regarding your response to these deficiencies and addresses the

following issues:

Opportunity to Correct - the facility is allowed an opportunity to correct identified deficiencies

before remedies are imposed;

Electronic Plan of Correction - when a plan of correction will be due and the information to be

contained in that document;

Remedies - the type of remedies that will be imposed with the authorization of the

Centers for Medicare and Medicaid Services (CMS) if substantial compliance is not attained at

the time of a revisit;

Potential Consequences - the consequences of not attaining substantial compliance 3 and 6

months after the survey date; and

Informal Dispute Resolution - your right to request an informal reconsideration to dispute the

attached deficiencies.

OPPORTUNITY TO CORRECT - DATE OF CORRECTION - REMEDIES

As of January 14, 2000, CMS policy requires that facilities will not be given an opportunity to correct before

remedies will be imposed when actual harm was cited at the last standard or intervening survey and also cited

at the current survey. Your facility does not meet this criterion. Therefore, if your facility has not achieved

substantial compliance by December 8, 2015, the Department of Health will impose the following remedy:

• State Monitoring. (42 CFR 488.422)

ELECTRONIC PLAN OF CORRECTION (ePoC)

An ePoC for the deficiencies must be submitted within ten calendar days of your receipt of this letter. Your

ePoC must:

- Address how corrective action will be accomplished for those residents found to have been

affected by the deficient practice;

- Address how the facility will identify other residents having the potential to be affected by the

same deficient practice;

- Address what measures will be put into place or systemic changes made to ensure that the

deficient practice will not recur;

- Indicate how the facility plans to monitor its performance to make sure that solutions are

sustained. The facility must develop a plan for ensuring that correction is achieved and

sustained. This plan must be implemented, and the corrective action evaluated for its

effectiveness. The plan of correction is integrated into the quality assurance system;

- Include dates when corrective action will be completed. The corrective action

completion dates must be acceptable to the State. If the plan of correction is

unacceptable for any reason, the State will notify the facility. If the plan of correction is

acceptable, the State will notify the facility. Facilities should be cautioned that they are

ultimately accountable for their own compliance, and that responsibility is not alleviated

in cases where notification about the acceptability of their plan of correction is not made

timely. The plan of correction will serve as the facility’s allegation of compliance; and,

- Submit electronically to acknowledge your receipt of the electronic 2567, your review and your

ePoC submission.

The state agency may, in lieu of a revisit, determine correction and compliance by accepting the facility's ePoC

if the ePoC is reasonable, addresses the problem and provides evidence that the corrective action has occurred.

If an acceptable ePoC is not received within 10 calendar days from the receipt of this letter, we will recommend

to the CMS Region V Office that one or more of the following remedies be imposed:

• Optional denial of payment for new Medicare and Medicaid admissions (42 CFR

488.417 (a));

• Per day civil money penalty (42 CFR 488.430 through 488.444).

Failure to submit an acceptable ePoC could also result in the termination of your facility’s Medicare and/or

Medicaid agreement.

PRESUMPTION OF COMPLIANCE - CREDIBLE ALLEGATION OF COMPLIANCE

The facility's ePoC will serve as your allegation of compliance upon the Department's acceptance. Your

signature at the bottom of the first page of the CMS-2567 form will be used as verification of compliance. In

order for your allegation of compliance to be acceptable to the Department, the ePoC must meet the criteria

listed in the plan of correction section above. You will be notified by the Minnesota Department of Health,

Licensing and Certification Program staff and/or the Department of Public Safety, State Fire Marshal Division

staff, if your ePoC for the respective deficiencies (if any) is acceptable.

VERIFICATION OF SUBSTANTIAL COMPLIANCE

Upon receipt of an acceptable ePoC, an onsite revisit of your facility may be conducted to validate that

substantial compliance with the regulations has been attained in accordance with your verification. A Post

Certification Revisit (PCR) will occur after the date you identified that compliance was achieved in your plan of

correction.

If substantial compliance has been achieved, certification of your facility in the Medicare and/or

Medicaid program(s) will be continued and remedies will not be imposed. Compliance is certified as of the

latest correction date on the approved ePoC, unless it is determined that either correction actually occurred

between the latest correction date on the ePoC and the date of the first revisit, or correction occurred sooner

than the latest correction date on the ePoC.

Original deficiencies not corrected

If your facility has not achieved substantial compliance, we will impose the remedies described above. If the

level of noncompliance worsened to a point where a higher category of remedy may be imposed, we will

recommend to the CMS Region V Office that those other remedies be imposed.

of a higher category of remedy, we will recommend to the CMS Region V Office that those remedies be

imposed.

If new deficiencies are found at the revisit, the remedies specified in this letter will be imposed. If the

deficiencies identified at the revisit require the imposition of a higher category of remedy, we will recommend

to the CMS Region V Office that those remedies be imposed. You will be provided the required notice before

the imposition of a new remedy or informed if another date will be set for the imposition of these remedies.

FAILURE TO ACHIEVE SUBSTANTIAL COMPLIANCE BY THE THIRD OR SIXTH MONTH

AFTER THE LAST DAY OF THE SURVEY

If substantial compliance with the regulations is not verified by January 29, 2016 (three months after the

identification of noncompliance), the CMS Region V Office must deny payment for new admissions as

mandated by the Social Security Act (the Act) at Sections 1819(h)(2)(D) and 1919(h)(2)(C) and Federal

regulations at 42 CFR Section 488.417(b). This mandatory denial of payments will be based on the failure to

comply with deficiencies originally contained in the Statement of Deficiencies, upon the identification of new

deficiencies at the time of the revisit, or if deficiencies have been issued as the result of a complaint visit or

other survey conducted after the original statement of deficiencies was issued. This mandatory denial of

payment is in addition to any remedies that may still be in effect as of this date.

We will also recommend to the CMS Region V Office and/or the Minnesota Department of Human Services

that your provider agreement be terminated by April 29, 2016 (six months after the identification of

noncompliance) if your facility does not achieve substantial compliance. This action is mandated by the Social

Security Act at Sections 1819(h)(2)(C) and 1919(h)(3)(D) and Federal regulations at 42 CFR Sections 488.412

and 488.456.

This REQUIREMENT is not met as evidenced by: Based on observation, interview and document review the facility failed to develop a care plan related to the routine use of a hypnotic medication for 1 of 1 resident (R73) reviewed for unnecessary medications and who received a hypnotic, daily.

Findings Include:

R73's significant change minimum data set (MDS) dated 5/21/15, identified R73 had a moderate cognitive impairment and diagnoses which included non- Alzheimer dementia, depression and insomnia.

R73's current physician's orders dated 7/2/15, directed staff to administer R73's Trazodone HCL (hypnotic) tablet 50 mg by mouth at bed time for insomnia.

R73's care plan dated 10/29/15, lacked identification of the use of Trazadone and adverse reactions related to the use of the medication.

On 10/28/15, at 7:06 a.m. R73 was observed in the living room seated in a wheelchair.-At 7:15 a.m. R73 independently wheeled self with his feet to the nurse's station by the main dining room. -At 7:40 a.m. was independently eating.

All resident charts will be reviewed for the use of pharmacological management for sleep. Care Plans will be reviewed to ensure that the use of hypnotic medication is addressed in the care plan in addition to non-pharmacological interventions. Care plans will be reviewed initially weekly for the first 3 months by DON or designee to ensure compliance with all charts reviewed and compliant no later than 11/13/15. After the 3rd month if compliance has been consistent than every other week for 2 months and then monthly checks their after. Staff has been educated on importance of addressing both pharmacological and non-pharmacological interventions related to sleep in the care plan with continued education through staff meetings and 1:1 training. Education of staff by DON is to be completed no later than 11/20/15. Pharmacy consultants will be providing education in addition to nursing education in relation to psychotropic medications to be completed no later than December 20th. Quality Assurance Committee members have been made aware of citation and will review POC with next QA meeting in December 2nd, 2015.

-At 8:41 a.m. R73 was observed in the therapy room exercising. Following the exercising, R73 left the building with family and remained gone the rest of the day.

On 10/29/15, at 10:22 a.m. register nurse (RN) case manager said R73 was on Trazadone prior to coming here and the wife asked the doctor to put him on it and he did. RN stated she did not do a sleep study for R73. RN verified there was no documentation or a summary note for effectiveness or adverse reaction from the medication.

On 10/29/15, at 10:30 a.m. the director of nursing (DON) verified there was nothing on R73's care plan related to the use of the hypnotic.

Patient Care Planning Policy (undated) indicated each resident would have a written plan of care. The care plan would be implemented by all departments and individuals assigned to responsibilities in the care of the resident. In addition, the nurse managers were responsible for the coordination of the overall plan of care.

by: Based on observation, interview and document review, the facility failed to ensure care plan interventions for repositioning were followed for 1 of 3 residents (R20) identified at risk for pressure ulcer development and failed to ensure timely toileting was provided for 1 of 1 resident (R20) who was incontinent of urine.

Findings include:

R20's Order Summary Report dated 10/29/15, identified R20's diagnoses as hemiplegia (paralysis on one side of the body) and hemiparesis (severe weakness) following a cerebral vascular accident (stroke), hypertension, heart failure, urinary incontinence, and chronic obstructive pulmonary disease.

R20's care plan dated 10/27/15, directed staff to offer toileting and turn/repositioning every two hours and per R20's request.

The CNA care plan (undated) directed the NA's to offer R20 to be toileted and turn/repositioned every two hours.

On 10/28/15, at 7:40 a.m. R20 was observed seated in an electric wheelchair and drove himself down the hall toward the dining room. -At 8:42 a.m. R20 had finished eating breakfast and drove himself out of the dining room and returned to his room and remained seated in the

Staff will be educated through staff meetings as well as 1:1 education on importance of following plan of care specific to toileting and repositioning. Staff will be educated on where to find resident specific care plan information in relation to toileting and repositioning. Staff education to be completed by no later than 11/20/15. Audits will be in initiated by DON or designee to ensure compliance with repositioning and toileting on every shift for residents at risk for pressure ulcer development. The audits will consist of a monitored timed observation by cart nurse designated by DON or designee on select residents unannounced to NAR. The audits will begin every shift and daily then will change to randomized audits daily after 1 month if consistent compliance is observed. Findings of audits will be reported to DON or designee when observation was delegated to cart nurse. This will be implemented no later than 11/20/15. Quality Assurance Committee members have been made aware of citation and will review POC with next QA meeting in December 2nd, 2015.

NA-A had not offered or provided R20 toileting and/or repositioning assistance. R20 remained seated in the wheelchair in his room until 9:41 a.m. when at this time, R20 steered himself to the activity room for bingo.-At 10:38 a.m. R20 steered himself back to his room, situated himself in front of the television set and remained seated in the wheelchair. -At 10:41 a.m. R20 put his call light on for assistance. -At 10:46 a.m. NA-A entered R20's room, gathered some clean clothes items and supplies and followed R20 to the sauna room across the hall. -At 10:52 a.m. R20 remained in the sauna room, seated in the wheelchair awaiting his turn to enter the sauna/shower. -At 11:10 a.m. (3 hours and 30 minutes since R20 had been changed and repositioned) NA-B and NA-C transferred R20 to a semi-standing position using a mechanical lift. NA-C removed R20's sweat pants and incontinent brief. R20's bottom was reddened on both buttocks and gluteal fold areas, no open areas seen. NA-B confirmed R20 had been incontinent of urine. NA-B stated R20 wasn't usually incontinent.

On 12/28/15, at 12:46 p.m. registered nurse (RN)-A confirmed R20 should be offered to be toileted every two hours and per his request.

On 10/28/15, at 1:41 p.m. the director of nursing (DON) confirmed R20 was at risk for the development of a pressure ulcer and stated she

Comprehensive Resident Care Plans Policy (undated) identified each resident's care plan would include measurable objectives and timetables to meet all resident needs identified in the comprehensive assessment.

Patient Care Planning Policy (undated) indicated each resident would have a written plan of care. The care plan would be implemented by all departments and individuals assigned to responsibilities in the care of the resident. In addition, the nurse managers were responsible for the coordination of the overall plan of care.

This REQUIREMENT is not met as evidenced by: Based on observation, interview and document review, the facility failed to provide repositioning assistance according the the assessed need in order to minimize the development of pressure related ulcers for 1 of 3 residents (R20) reviewed who was at risk for pressure ulcers.

Findings include:

R20's quarterly Minimum Data Set (MDS) dated 8/5/15, indicated R20's cognition was intact, however required extensive assist with bed mobility, transferring, toileting and personal hygiene. R20 had range of motion impairment on one side of his body on both upper and lower extremities. In addition, the MDS indicated R20 was frequently incontinent of urine. R20's Pressure Ulcer Care Area Assessment (CAA) dated 2/10/15, indicated R20 was at risk for the development of a pressure ulcer and that R20 was turned/repositioned every two hours.

R20's Tissue Tolerance SittingTest dated 7/2/15, directed staff to reposition R20 every two hours.

R20's care plan, dated 10/27/15, directed staff to turn and reposition R20 every two hours and per R20's request.

Staff will be educated through staff meetings as well as 1:1 education on importance of following plan of care specific to toileting and repositioning including specifically pressure ulcer development. Staff will be educated on where to find resident care plan information in relation to repositioning and toileting. Staff education to be completed by no later than 11/20/15. Audits will be in initiated by DON or designee to ensure compliance with repositioning and toileting on every shift for residents at risk for pressure ulcer development. The audits will consist of a monitored timed observation by cart nurse designated by DON or designee on select residents unannounced to NAR. The audits will begin every shift and daily then will change to randomized audits daily after 1 month if consistent compliance is observed. Findings of audits will be reported to DON or designee when observation was delegated to cart nurse. This will be implemented no later than 11/20/15. Quality Assurance Committee members have been made aware of citation and will review POC with next QA meeting in December 2nd, 2015.

R20's Braden Scale for Predicting Pressure Sore Risk dated 9/24/15, indicated R20 was at risk for the development of a pressure ulcer and the interventions listed included to keep skin clean and dry and to turn and reposition every two hours.

R20's Order Summary Report dated 10/29/15, identified R20's diagnoses as hemiplegia (paralysis on one side of the body) and hemiparesis (severe weakness) following a cerebral vascular accident (stroke), hypertension, heart failure, urinary incontinence, and chronic obstructive pulmonary disease.

On 10/28/15, at 7:40 a.m. R20 was observed seated in an electric wheelchair and drove himself down the hall toward the dining room. R20 had a pressure relieving cushion in the wheelchair and R20's left arm was rested in a sheep skin lined trough, fastened with Velcro straps securing the left arm in place. -At 8:42 a.m. R20 had finished eating breakfast and drove himself out of the dining room, returned to his room and remained seated in the wheelchair.-At 8:47 a.m. nursing assistant (NA)-A entered R20's room and asked R20 if he wanted to take a shower today. On 10/28/15, at 12:26 p.m. trained medication aide (TMA)-A stated R20 should be checked, changed every two hours.

On 12/28/15, at 12:46 p.m. registered nurse (RN)-A confirmed R20 should be offered to be toileted every two hours and per his request.

On 10/28/15, at 1:41 p.m. the director of nursing (DON) confirmed R20 was at risk for the development of a pressure ulcer. DON stated she expected staff to follow R20's care plan with regards to timing of toileting. The DON confirmed R20 should have been at least offered toileting every two hours.

On 10/28/15, at 2:12 p.m. NA-A verified she had not offered to toilet R20 when she had entered R20's room that morning (at 8:47 a.m.).

The Greenwood Connections Policy and Procedure for Incontinent Management (undated) indicated all incontinent residents would be evaluated for proper incontinence control management techniques. In addition, for those residents on a check and change program, they would be checked for wetness and changed and cleaned if wet.

483.25(l) DRUG REGIMEN IS FREE FROM UNNECESSARY DRUGS

Each resident's drug regimen must be free from unnecessary drugs. An unnecessary drug is any drug when used in excessive dose (including duplicate therapy); or for excessive duration; or without adequate monitoring; or without adequate indications for its use; or in the presence of adverse consequences which indicate the dose should be reduced or discontinued; or any combinations of the reasons above.

Based on a comprehensive assessment of a resident, the facility must ensure that residents who have not used antipsychotic drugs are not given these drugs unless antipsychotic drug therapy is necessary to treat a specific condition as diagnosed and documented in the clinical record; and residents who use antipsychotic drugs receive gradual dose reductions, and behavioral interventions, unless clinically contraindicated, in an effort to discontinue these drugs.

Based on observation, interview and document review the facility failed to ensure a comprehensive sleep assessment / sleep monitoring was completed and failed to identify non pharmacological interventions for sleep for 1 of 5 residents (R73) reviewed for unnecessary medications.

Staff education will occur on importance of sleep observation monitoring implementation prior to and/or with the initiation of any medication that would be used for sleep. Findings Include:

R73's significant change minimum data set (MDS) dated 5/21/15, identified R73 had a moderate cognitive impairment and diagnoses which included non- Alzheimer dementia, depression and insomnia. The MDS also identified under the mood section D0200 that R73 had no trouble falling or staying asleep, or sleeping too much. R73's quarterly minimum data set (MDS) dated 8/13/15, indicated R73 had a severe cognitive impairment, diagnoses which included non-Alzheimer's dementia and anxiety. The MDS also identified under the mood section D0100 that R73 had no trouble falling or staying asleep, or sleeping too much.

R73's current physician's orders dated 7/2/15, directed staff to administer R73's Trazodone HCL tablet 50 mg by mouth at bed time for insomnia.

R73's care plan dated 10/29/15, lacked identification and monitoring of individualized insomnia symptoms and lacked evidence that non-pharmacological approaches were tried before the administration of the medication. In addition, R73's nursing assistant (NA) care plan did not include any non-pharmacological interventions to promote sleep.

On 10/28/15, at 7:06 a.m. R73 was observed in the living room seated in a wheelchair.-At 7:15 a.m. R73 independently wheeled self with his feet to the nurse's station by the main dining room.

interventions for sleep will be conducted to ensure compliance of sleep study implementation. DON or designee will also be alert to any new medications or sleep trouble that may be reported in daily report. Audits will occur initially weekly for the first 3 months by DON or designee to ensure compliance with all charts reviewed. Compliance to be met by no later than 11/13/15. After the 3rd month if compliance has been consistent than every other week for 2 months and then monthly checks their after. Pharmacy consultants will offer additional education to nursing staff on psychotropic medication to be completed by RNs not later than December 20th. Quality Assurance Committee members have been made aware of citation and will review POC with next QA meeting in December 2nd, 2015.

-At 7:40 a.m. was independently eating.-At 8:41 a.m. R73 was observed in the therapy room exercising. Following the exercising, R73 left the building with family and remained gone the rest of the day.

On 10/29/15, at 10:22 a.m. register nurse (RN) case manager said R73 was on Trazadone prior to coming here and the wife asked the doctor to put him on it and he did. RN stated she did not do a sleep study for R73. RN verified there was no documentation or a summary note for effectiveness or adverse reaction from the medication.

On 10/29/15, at 10:30 a.m. the director of nursing (DON) stated she would expect a sleep monitor to be initiated. The DON said she would look at the sleep monitor to to see if the resident was sleeping or up all night, to see if the medication was effective. The DON stated the sleep monitor was not implemented and it should have been done, it is protocol. The DON verified there was nothing on the care plan for sleep or non -pharmacological interventions to be used.

The facilities policy titled Assessment dated 5/11/12, indicated any resident on a sedative or medication to help them sleep either routinely or as needed needs to have a sleep study for at least one week. The RN then needs to assess the information and make recommendations and if the resident remains on the medication for insomnia the sleep study needs to be done quarterly.

The drug regimen of each resident must be reviewed at least once a month by a licensed pharmacist.

The pharmacist must report any irregularities to the attending physician, and the director of nursing, and these reports must be acted upon.

This REQUIREMENT is not met as evidenced by: Based on interview and record review, the facility consultant pharmacist failed to ensure sleep patterns were monitored in order to determine sleep medication effectiveness and in addition, nonpharmacological interventions were used for sleep promotion for 1 of 1 resident (R73) who received medication for sleep.

Findings Include:

R73 received a daily dose of Trazadone medication for sleep, the record lacked identification and monitoring of individualize insomnia symptoms, and lacked evidence that non-pharmacological approaches were tried before the administration of the medication. R73's care plan with a print date of 10/29/15, lacked identification and monitoring of individualized insomnia symptoms and lacked evidence that non-pharmacological approaches were tried before the administration of the Trazadone medication. In addition, R73's nursing assistant (NA) care plan did not include any non-pharmacological interventions to promote sleep.

R73's consulting pharmacist report dated 6/9/15, indicated R73 had a recent fall and Elavil ( medication used for sleep) had potential side effects and recommended the physician considered changing the medication to Remeron or Trazadone which was used for sleep. The pharmacy review lacked any documentation for monitoring the effectiveness of Elavil use or if non-pharmacological interventions were in place.

On 10/29/15, at 10:22 a.m. register nurse (RN) -A stated R73 was on trazadone prior to admission to the facility and said R73's spouse had asked the doctor to put him on it and he did. RN-A stated she had not completed a sleep study for R73 and verified there was no documentation or

training. Staff education by DON to be completed no later than 11/20/15. Pharmacy consultants will be providing education in addition to nursing education in relation to psychotropic medications to be completed no later than December 20th. Quality Assurance Committee members have been made aware of citation and will review POC with next QA meeting in December 2nd, 2015. Pharmacy consultants after monthly visit will review and share any concerns with DON after review of resident records including specifically concerns related to psychotropic medications.

On 10/29/15, at 10:30 a.m. the director of nursing (DON) stated she would have expected a sleep monitor be initiated. The DON stated she would look at the sleep monitor results in order to see if the resident was sleeping or up all night to see if the medication was effective. On 11/2/15, at 1:20 p.m. via phone the consulting pharmacist (CP) stated she was not sure if the trazadone was checked by another pharmacist or not as she had not reviewed R73's medications. The CP stated they usually checked for things for insomnia and if not found they would make recommendations to nursing which was their normal process. The CP stated she would have expected nursing to monitor sleep and look for non-pharmacological interventions. The CP also stated if a medication had been changed they would expect documentation for an improvement, the same or worse. The CP indicated if they were unable to find any documentation, then the pharmacist should have identified that.
