• Filling the skip was difficult because it was in the wrong place.

• Local supervisor was not paying attention to the work.

• Labourer decided to move the skip on his own.

• He positioned his hands without thinking about possible hazards.

• Finger was trapped between lifting handle and rim of large steel bucket. Top section was crushed.
