Control of Hazardous Energy Policy

Personal locks & keys will be issued to employees by their Safety/Training Coordinator prior to their hands on training on machinery or equipment. (Locks & Keys will be engraved with employees names & clock card numbers)

UNDER NO CIRCUMSTANCES ARE YOU ALLOWED TO USE SOMEONE ELSE’S LOCK!!!!!!!!!

If multiple people are working on a machine, each individual must be LOCKED OUT.

Definitions

• Affected Employees: Any employee who performs the duties of his/her job in an area which the LOTO Procedure is implemented.

• Authorized Employees: A trained individual to whom authority and responsibility is given to implement the LOTO Procedure on machines or equipment.

• Energized: Residual (left over) energy, stored energy or machines and equipment connected to an energy source.

• Energy Source: Any electrical, mechanical, hydraulic, pneumatic, chemical, thermal, or other energy source that could cause injury to personnel.

• Energy Isolation Device: A mechanical device that physically prevents the transmission or release of energy (on/off switch)

• Lockout/ Tag out: The placement of a lock and/or tag on the energy isolating device in accordance with an established procedure, indicating the machinery/ Equipment is not to be operated until removal of the lock/tag in accordance with an established procedure.

LOCKOUT / TAGOUT IS REQUIRED FOR THE FOLLOWING OPERATIONS:

• Anytime an employee may be exposed to a hazard.• When a guard is removed.• When employees must place a body part inside equipment or an area where they could contact an energy source. • When an electrical guard or panel is removed.• Installation and set up operations.• Any repair of equipment or machines.• Preventative maintenance of machines or equipment.• A spill in equipment that requires cleaning.• A equipment jam.• When troubleshooting, repairing or installing electrical circuits.• When performing maintenance on moving parts.

(cycle stop, depressing the stop button, etc.)

Isolate the energy by disconnecting any/allsources of power that apply to area of the machine that needs to be worked on.

After making sure that no one is exposed, push the START button to be certain that energy is properly isolated, stored energy is

released and equipment will not operate.

Now that the equipment is properly lockedout, any needed work can be performed.

If you do not have a lock and are not personally locked out, you are not permitted to be in any part of the machine, no matter how much you

want to help out.

resume.
